Fluid overload was prevalent in the early perioperative period of kidney transplantation. It did not promote renal recovery, but was associated with a high risk of complications. Our findings might be a useful indicator to optimize the perioperative fluid management of kidney transplant recipients.
This study aimed to investigate effects of workplace bullying on different post-traumatic stress symptoms and coping among hospital nurses.

Workplace bullying is a traumatic event that negatively affects the quality of patient care and nurses' mental health.

This cross-sectional, correlational study used an online survey among hospital nurses. Ordinary least square and quantile regression analyses were conducted using Stata version 16.

The study included 233 registered nurses from South Korea who had provided direct care to patients in a hospital for at least 6months. Overall, 28% self-identified as victims or witnesses and 37% as victims and witnesses simultaneously. 'Victim' and 'passive coping' were significantly associated with the 25th, 50th and 75th percentiles groups of post-traumatic stress symptoms, while 'witness' was significant in the 95th percentile group.

Our study findings explore nurses' workplace bullying, detect high-risk subgroups and suggest the development of coping interventions for reducing workplace bullying and post-traumatic stress symptoms.

This study aims to ascertain nurse managers' perception of nurses' governance and explore their perception of facilitating and hindering factors of shared governance.

Shared governance has been introduced in nursing as a plan to improve nursing service quality. There is limited understanding about how governance is implemented.

A mixed-methods design was used to elicit Korean nurse managers' views.

Quantitative data revealed the level of nurses' governance perceived by nurse managers as shared governance. However, among the subdomains, 'control over personnel' and 'participation in committee structures' indicate traditional governance. Analysis of qualitative data identified the factors that facilitate or hinder the subdomains 'stifled atmosphere', 'nurse manager's encouragement', 'lack of opportunity and perception of the given authority' and 'flexible organisational structure'.

Findings from the study can be used to inform shared governance programmes for hospitals.

Health care organisations, nurse managers and nurses must take action at each level to improve the current nurse governance situation. A flexible organisational culture with opinion windows for participation by nurses is important for shared governance. Supportive leadership of nurse managers and nurse-directed implementation could advance shared governance.

It can further lower blood pressure and reduce liver inflammation. Policosanol can also affect vascular calcification, however, its molecular mechanisms are not well understood. This study investigated the effect of policosanol on vascular calcification and its molecular mechanism. Policosanol decreased the expression of inorganic phosphate (Pi)-induced osteogenic genes such as distal-less homeobox 5 (Dlx5) and runt-related transcription factor 2 (Runx2). Ethyl 3-Aminobenzoate In addition, following policosanol treatment, adenosine monophosphate-activated protein kinase (AMPK) phosphorylation increased in a time-dependent manner. The constitutively active form of AMPK (CA-AMPK) dramatically suppressed Pi-induced Dlx5 and Runx2 protein levels. Inactivation of AMPK using compound C (Com. C; AMPK inhibitor) recovered policosanol-suppressed Alizarin Red S staining levels. Insulin-induced genes (INSIGs) were induced by CA-AMPK, their overexpression suppressed Pi-induced Dlx5 and Runx2 expression. Taken together, the results demonstrate that policosanol inhibits Pi-induced vascular calcification by regulating AMPK-induced INSIG expression in vascular smooth muscle cells.
Flecainide is a class 1C antiarrhythmic primarily usedfor the management of supraventricular arrhythmias. Adverse effects in patientswith pacemakers are poorly described in the current literature. We report acase of worsening intracardiac conduction in a patient with baseline atriallatency.

A patient with paroxysmalatrial fibrillation and recently implanted dual chamber pacemaker withresultant atrial latency underwent flecainide treatment resulting in worseningatrial latency, progressive dyspnea, and pacemaker syndrome physiology. The patientexperienced symptomatic improvement with pacemaker reprogramming to AAI andreduced flecainide dosages.

Prudence should be exercised when initiating flecainide therapy, particularly in pacemaker-dependentpatients, who may be at risk for increasedthresholds, inconsistent capture, and latency. In the setting of apparentcapture latency, changing to AAI mode may both confirm the diagnosis andprovide a symptomatic benefit.
