Arthroscopic treatment of the iliopsoas may be an example of how treatment patterns and trends can shift with limited evidence-based medicine. A cross-sectional survey of 16 high-volume hip preservation surgeons was conducted to gather perspectives and opinions on how and why the arthroscopic management of the iliopsoas has evolved. All participants completed the survey in person and anonymously. Of the surveyed surgeons, the mean career hip preservation volume was 1031.25 cases (250 to >3000) with an average annual volume of 162.08 cases (75-400). Of the surveyed surgeons' caseload, 16.1% involved an iliopsoas tenotomy or fractional lengthening mostly commonly (75%) for recalcitrant internal snapping. Labral repair/reconstruction is performed concomitantly 87.5% of the time. Seventy-five percent of surgeons indicated a decrease in frequency of iliopsoas tenotomy over the course of their practice most commonly (56.3%) because of hip flexion weakness; however, 0% of the surgeons could cite literature evidence to support their practices. Perceived poor outcomes in individual practices was the most common (56.3%) source of this complication. Surgeons were less inclined to perform tenotomy on patients with borderline dysplasia (75%) or ligamentous laxity (56.3%).Capsulotomy in different modalities has been used to provide adequate exposure to access both the central and peripheral compartment in hip arthroscopy. Even though the hip joint has inherent bony stability, soft tissue restraints may be important in patients with ligaments hyperlaxity or in some cases with diminished bony stability. Biomechanical studies and clinical outcomes have shown the relevant role of the capsule in hip stability, mainly the role of the iliofemoral ligament. Although is not very common, iatrogenic post-arthroscopy subluxation and dislocation have been reported and many surgeons are concerned about the role aggressive capsulotomy or capsulectomy in this situation, thus capsule repair has become very popular. We present a novel technique to access the hip without cutting the iliofemoral ligament. We describe the use of and provide the software code for a technique to better define the location and magnitude of acetabular pathology. We have developed software code that allows the end user to obtain detailed measurements of the acetabulum using traditional computed tomography data. We provide the code and detailed instructions on how to use it in this article. The methodology was validated by having an unbiased observer (that was not involved in this project but has been trained in this software measurement methodology) to perform the entire acquisition, reconstruction and analysis procedure and compare their measurements to the measurements of one of the authors. The author then repeated the procedure 2 months later to determine intra-observer reliability. Inter- and intra-observer reliability for version, tilt, surface area and total acetabular coverage angles ranged from an intra-class correlation coefficient of 0.805 to 0.997. The method provided in this manuscript gives a reproducible objective assessment of three-dimensional (3D) acetabular morphology that can be used to assist in the diagnosis of hip pathology and to compare the morphological parameters of subjects with and without hip pathology. It allows a surgeon to understand the 3D shape of each individual's acetabulum, share these findings with patients and their parents to demonstrate the magnitude and location of the clinical abnormality and perform patient-specific surgical corrections to optimize the shape and coverage of the hip.Hip microinstability is a recognized cause of hip pain in young patients. Intra-operative evaluation is used to confirm the diagnosis, but limited data exist associating magnetic resonance arthrography (MRA) findings with hip microinstability. To determine if a difference exists in the thickness of the anterior joint capsule and/or the width of the anterior joint recess on MRA in hip arthroscopy patients with and without an intra-operative diagnosis of hip laxity. Sixty-two hip arthroscopy patients were included in the study. Two musculoskeletal radiologists blinded to surgical results reviewed the MRAs for two previously described findings (i) anterior joint capsule thinning; (ii) widening of the anterior joint recess distal to the zona orbicularis.
