The gold standard public databases present images with segmentations of the disc and cupping made by experts and division between training and test groups, serving as a reference for use in deep learning architectures. However, the data offered are not interchangeable. The quality and presentation of images are heterogeneous. Moreover, the databases use different criteria for binary classification with and without glaucoma, do not offer simultaneous pictures of the two eyes, and do not contain elements for early diagnosis. In recent years, hepato-pancreato-biliary (HPB) cancers have been increasing their incidence [...].Context-specific information, including differences in geographical areas, such as distinct altitudes, can be important to explain variations in physical growth. We aimed to compare the estimation of maximum growth velocity and pubertal growth-spurt parameters of children and adolescents living at low and moderate altitudes in Colombia. A cross-sectional study, including a representative cohort of 30.305 (51% boys) children and adolescents aged 1-18 years from Colombia, was performed. The heights were measured with standardized techniques. The Preece-Baines growth model was used to estimate the mathematical and biological parameters of the height-growth velocities and growth spurts for both sexes. The altitudes were categorized as low (18 to 564 m above sea level) or moderate (2420 to 2640 m above sea level). There were no differences in final height (h1), peak height velocity size (hθ), age at peak height velocity (APHV), or peak height velocity PHV (cm/y) between the subjects living in both altitudes (p > 0.05). The APHV was estimated at 12.75 ± 0.75 years in the boys and at 10.05 ± 0.65 years in the girls. The girls reached the APVH 2.70 years earlier than the boys. Regarding the PHV, the boys reached higher growth velocity, which was 6.85 ± 0.55 cm/y. In conclusion, there were no significant differences in final height, peak height, APHV, or PHV between the children and adolescents living at distinct altitudes in Colombia. The PHV occurred approximately 3 years earlier in the girls than in the boys. Furthermore, the girls' estimated PHV, APHV, and final height were lower than those of the boys. This study allows additional insight into pubertal growth-spurt parameters and also provides a valuable reference database for the assessment of Colombian children and adolescents.Patient Blood Management (PBM) is a patient-centered, systemic and evidence-based approach. Its target is to manage and to preserve the patient's own blood. The aim of PBM is to improve patient safety. As indicated by several meta-analyses in a systematic literature search, the cell salvage technique is an efficient method to reduce the demand for allogeneic banked blood. Therefore, cell salvage is an important tool in PBM. Cell salvage is widely used in orthopedic-, trauma-, cardiac-, vascular and transplant surgery. Especially in cases of severe bleeding cell salvage adds significant value for blood supply. In cardiac and orthopedic surgery, the postoperative use for selected patients at the intensive care unit is feasible and can be implemented well in practice. Since the retransfusion of unwashed shed blood should be avoided due to multiple side effects and low quality, cell salvage can be used to reduce postoperative anemia with autologous blood of high quality. Implementing quality management, compliance with hygienic standards as well as training and education of staff, it is a cost-efficient method to reduce allogeneic blood transfusion. The following article will discuss the possibilities, legal aspects, implementation and costs of using cell salvage devices in an intensive care unit. The aim of this study was to assess the impact of dimensional distortion and its changes with modification of exposure setting parameters on the measurements of peri-implant bone margin. Ten titanium dental implants (InKone Primo, Global D, Paris, France) were placed in two prepared bovine ribs. Two bone models and an implant-with-transfer model were scanned with 3shape E4 (3shape, Copenhagen, Denmark) laboratory scanner. Cone beam computed tomography (CBCT) images of two bone models were taken with different values of voltage (60, 70, 80, 90 kV), tube current (4, 10 mA) and voxel size (200, 300 µm). All the data were superimposed using planning software, and the measurements of buccal bone thickness in two selected regions were performed both using CBCT and scan cross-sections. The mean squared error (MSE) being the squared differences between measurements was used in the accuracy assessment of the CBCT device. A one-way ANOVA revealed significant differences between voltage and MSE (p = 0.044), as well as implant position and MSE (p = 0.005). The distortions of measurements depend on bone margin thickness, and the higher the distance to measure, the higher the error. Accurate measurements of buccal bone thickness (MSE below 0.25) were achieved with voltage values of 70, 80, and 90 kV.The trend of Total Hip Arthroplasty (THA) is projected to grow. Therefore, it has become imperative to find new measures to improve the outcomes of THA. Several studies have focused attention on the influence of psychological factors and sleep quality on surgical outcomes. The consequences of depressive states may affect outcomes and also interfere with rehabilitation. In addition, sleep quality may be an essential factor in determining surgical outcomes. To our knowledge, few articles focus on the influence of these factors on THA results. The present study investigates a possible correlation between preoperative depression or sleep quality and postoperative outcomes of THA. This study was conducted with 61 consecutive patients undergoing THA from January 2020 to January 2021. Patients were assessed preoperatively using GDS and PSQI, and six months postoperatively using FJS-12, SF-36, WOMAC, PSQI, and GDS. To simplify comparisons, the overall scores were normalized to range from 0 (worst condition) to 100 po
During the last years, the COVID-19 pandemic meets the pandemic generated by obesity, raising many questions regarding the outcomes of those with severe forms of infection.
The present systematic review summarises and analyses the data providing evidence for or against the "obesity-paradox" in COVID-19 patients. After applying the inclusion and exclusion criteria, 23 studies were included. We also analysed the presumably underlying basic mechanisms.
The patients with a body mass index (BMI) of 30-40 kg/m
presented severe symptoms that led to intensive care unit (ICU) admission but not increased death rate. Those with a higher degree of obesity, with a BMI higher than 40 kg/m
, led to a rise in the death rate, particularly in young patients. Obesity was associated with a higher rate of ICU admission but was not determined as an independent predictor of increased mortality. In contrast, some studies suggest a strong association between obesity or morbid obesity and the risk of death.
The existence of "obesity-paradox" cannot be stated; our study presents obesity as a critical risk factor in the evolution of COVID-19.

Exposure to high altitudes determines several adaptive mechanisms affecting in a complex way the whole cardiovascular, respiratory, endocrine systems because of the hypobaric hypoxic condition. The aim of our study was to evaluate the circulatory adaptive mechanisms at high altitudes, during a scientific expedition in the Himalayas.
Arterial distensibility was assessed measuring carotid-radial and carotid-femoral pulse wave velocity. Tests were carried out at several altitudes, from 1350 to 5050 m above sea level, on 8 lowlander European researchers and 11 highlander Nepalese porters.
In Europeans, systolic blood pressure and pulse pressure increased slightly but significantly with altitude (
< 0.05 and
< 0.001, respectively). Norepinephrine showed a significant increase after the lowlanders had spent some time at high altitude (
< 0.001). With increasing altitude, a progressive increase in carotid-radial and carotid-femoral pulse wave velocity values was observed in lowlandes ethnic group. Further studies are needed to evaluate the role of aortic stiffening in the pathogenesis of acute mountain sickness.Despite its high prevalence, the mechanisms underlying non-alcoholic fatty liver disease (NAFLD) in people living with HIV (PLWH) are still unclear. In this prospective cohort study, we aim to evaluate differences in plasma fatty acid profiles between HIV-infected and HIV-uninfected participants with NAFLD. We included participants diagnosed with NAFLD, both HIV-infected and HIV-uninfected. Fatty acid methyl esters were measured from plasma samples. Ratios ([product]/[substrate]) were used to estimate desaturases and elongases activity. Cytosporone B price We used linear regression for adjusted analyses. We included 31 PLWH and 22 HIV-uninfected controls. We did not find differences in the sum of different types of FA or in FA with a greater presence of plasma. However, there were significant differences in the distribution of some FA, with higher concentrations of ALA, trans-palmitoleic, and behenic acids, and a lower concentration of lignoceric acid in PLWH. PLWH had lower C240/C220 and C160/C140 ratios, which estimates the activity of elongases ELOVL1 and ELOVL6. Both groups had similar fatty acid distribution, despite differences in traditional risk factors. PLWH had a lower proportion of specific ratios that estimate ELOVL1 and ELOVL6 activity, which had been previously described for other inflammatory conditions, such as psoriasis.Mycoplasma hominis is a habitual colonizing microorganism of the lower genital tract but can exceptionally be the causal agent of blood culture-negative infective endocarditis (IE). Only 11 cases of this entity have been published to date. The study objectives were to describe the first case diagnosed in our center of IE by M. hominis on pacemaker lead and to carry out a narrative review. Among published cases of IE by this microorganism, 72.7% were male, with a mean age of 45 years and a history of valve surgery; the diagnosis was by culture (54.5%) or molecular technique (45.5%), and the prognosis was favorable in 72.7% of cases. The most frequently prescribed antibiotics were doxycycline, quinolones, and clindamycin.Premorbid adjustment (PA) has classically been defined as psychosocial functioning in the areas of education, occupation, social and interpersonal relationships prior to evidence of characteristic positive symptomatology. It is a concept which possesses ample evidence regarding its predictive nature for the course of Schizophrenia. The study aimed to analyze the latent profiles of premorbid adjustment and their relationship with symptomatology, functionality, subjective recovery, stigma resistance and years of untreated psychosis. Latent class analysis (LCA) was used to elaborate a solution of three premorbid adjustment profiles in a sample of 217 patients diagnosed with Schizophrenia from Public Mental Health Centers in the city of Arica, Chile. The results show that premorbid adjustment was significantly correlated with recovery indicators and that latent profiles of better premorbid adjustment predict better outcomes in subjective recovery and stigma resistance. The results show that premorbid adjustment not only has implications for the severity of the disorder, but that psychosocial functioning prior to psychosis affects the patient's subjectivity, the representation of the disorder and the recovery process.
