High-dose prednisolone is used in first-line treatment for lymphoma, but the potential adverse impact on bone health is unclear. Danish patients with diffuse large B-cell lymphoma or follicular lymphoma diagnosed between 2000 and 2012 were matched to the background population. Osteoporotic events (osteoporosis treatment or low-energy fracture) were identified using the Danish National Patient Registry and Prescription Registry. In total, 2589 patients and 12,945 controls were included. Lymphoma patients had increased risk of osteoporotic events compared to the matched population (hazard ratio 1.61 [95% confidence interval 1.40;1.84]). The 5- and 10-year cumulative risks of osteoporotic events for lymphoma patients were 10.0% [8.6;11.4] and 16.3% [13.8;18.7], whereas corresponding risks in the background population were 6.8% [6.3;7.3] and 13.5% [12.4;14.6]. Patients without osteoporotic event in the first two years after treatment were not at higher risk of osteoporotic events in subsequent years. Risk factors for osteoporotic events were female sex and age >70 years.Background/aim The effect of isotretinoin on soluble VEGFRs has not been previously investigated. This study evaluate the effects of isotretinoin (13-cis-retinoic acid) on soluble VEGFR1 (sVEGFR1), soluble VEGFR2 (sVEGFR2) and soluble VEGFR3 (sVEGFR3).Methods The study included 38 patients (28 females, 10 males) receiving systemic isotretinoin treatment and 38 healthy individuals (28 females, 10 males) with similar age and gender characteristics. The blood samples of the patient group at third months and blood samples of the control group were compared in terms of sVEGFR1, sVEGFR2 and sVEGFR3 concentrations.Results It was significant that sVEGFR1 was low and sVEGFR3 was high in patients receiving isotretinoin (p .038, p .021, respectively). There was no significant change in sVEGFR2 levels between the groups (p .519).Conclusions We think that the effect of isotretinoin on sVEGFR1, sVEGFR2 and sVEGFR3 may be secondary to its effects on the VEGF family. However, after clarifying the effect of isotretinoin on the VEGF family, we think that it can be used in some tumors and vascular diseases.Objective To study the efficacy and safety of fractional 2940 nm laser-assisted drug delivery of timolol maleate 0.5% solution for the treatment of deep IHs.Methods In this study, 30 deep IHs were included, and a fractional 2940 nm laser was applied with 2-weeks intervals. Topical timolol maleate 0.5% was applied under occlusion for 30 min four times/day for 24 weeks. The plasma concentration of timolol maleate was monitored for 0.5 h after the first treatment. The HAS scores and the depth, lateral diameter, vertical diameter of hemangiomas were evaluated before treatment, at 8 weeks and 24 weeks of treatment, and at 4 weeks after treatment.Results Twenty-three patients (76.7%) demonstrated excellent regression, 4(13.3%) showed good response, and 3(10%) experienced moderate regression. The HAS score was declined from 3.6 ± 0.7 to 2.3 ± 0.6 at 8 weeks, and from 1.3 ± 0.5 to 0.8 ± 0.5 at 24 weeks (p  less then  .05). Plasma timolol concentration was not detected in 11 infants, and the rest ranged from 1.580 pg/mL to 14.718 pg/mL, which were less then 1 ng/mL. A sequential mixed-method study was conducted in Zambia to explore provider perceptions of hypertension care and develop contextually relevant education for nurses. The Zambian and US nurse researchers interviewed 15 nurses and provided a hypertension education workshop. Thematic evaluation identified categories of infrastructure and belief & behaviors with a shared subcategory of medication. A statistically significant result was found in the health care providers knowledge and attitude toward hypertension, one of two pre and post-tests analysis (z = -2.17, p  less then  0.05, r = -.33) and (z = -.41, p =.67). Developing efficient and effective solutions to increase hypertension care must be made at the policy level and incorporate locally relevant information. Nurses, who have cultural and contextual expertise, are ideally situated to reach population health goals.Aim Trastuzumab prolongs progression-free and overall survival in HER2+ breast cancer (BC), but these are associated with increased distant recurrences and central nervous system metastases (CNSm). We retrospectively evaluated outcome and prognostic factors in CNSm and non-CNSm patients. Methods Records of HER2+ BC treated in 2000-2017 were reviewed. Results 283/1171 (24%) HER2+ BC patients developed metastatic disease. 109/283 patients (39%) have CNSm associated with worse prognosis and increased risk of death (hazard ratio 4.7; 95% CI 3.5-6.4). Prognostic factors were number of CNSm (single vs multiple lesions; 3-year overall survival 39 vs 18%; p = 0.003); brain radiation (30 vs 14%; p less then 0.001); new HER2-targeting therapies (30.6 vs 22.5%; p = 0.025). Conclusion Prognosis of BC patients with CNSm has improved using HER2-targeting therapies but remains poor.
