Several observational studies have recently reported the outcomes of non-insulin anti-diabetic agents (ADA) in patients with T2DM and coronavirus disease 2019 (COVID-19). We sought to review the literature to appraise the clinicians on these outcomes.
A literature search using the specific keywords was carried out in the database of PubMed, MedRxiv and Google Scholar up till December 11, 2020 applying Boolean method. Full text of all the relevant articles that reported the outcomes of ADA in patients with T2DM and COVID-19 were retrieved. Subsequently, an appraisal of literature report was narratively presented.
Available studies that reported the outcomes of ADA are either case series or retrospective cohorts or prospective observational studies, in absence of the randomized controlled trials (RCTs). Results from these observational studies suggest that amongst all the non-insulin ADA, metformin users prior to the hospitalization had improved outcomes compared to the non-users. Data for dipeptidyl-peptnts with T2DM and COVID-19.
We describe the characteristics and short-term prognosis of in-patients with diabetes and COVID-19 admitted to a Belgian academic care center.
We retrospectively reviewed the data on admission from patients with known or newly-diagnosed diabetes and confirmed COVID-19. First, survivors were compared to non-survivors to study the predictive factors of in-hospital death in patients with diabetes. Secondly, diabetic patients with SARS-CoV-2 pneumonia were matched for age and sex with non-diabetic patients with SARS-CoV-2 pneumonia, to study the prognosis and predictive factors of in-hospital death related to diabetes.
Seventy-three diabetic patients were included. Mean age was 69 (±14) years. Women accounted for 52%. Most patients had type 2 diabetes (89.0%), long-term complications of hyperglycemia (59.1%), and hypertension (80.8%). The case-fatality rate (CFR) was 15%. Non-survivors had more severe pneumonia based on imaging (p 0.029) and were less often treated with metformin (p 0.036). In patients withstatistically different between patients with and without diabetes.Even though the pulse-echo ultrasonic technique is commonly used for the assessment of metal structures, it has some inherent limitations. Vibro-acoustography (VA) is a relatively new ultrasonic technique which has demonstrated a great potential in revealing defects in objects, however it is traditionally used to analyze the integrity of specimens made of low-stiffness materials. This work presents the evaluation of the performance of VA technique for the inspection of a steel structure, which was manufactured with defects of known geometry on its inner surface. All the inspection process was done automatically, from the data acquisition to the image processing for characterizing the defects. Experimental results show that VA was able to identify and characterize even the smallest defects in the structure, with a diameter of 2 mm. In addition, the results show that VA was able to characterize the artificial defects in the steel structure with almost the same errors as the traditional pulse-echo method, which indicates a potential use of VA in the inspection of materials with high mechanical stiffness.
Although multiple intracranial aneurysms (MIAs) are frequent and disastrous, determining treatment strategy for them is often complicated, especially in elderly patients (≥60 years old). We evaluated the safety and effectiveness of one-stage coiling versus surgical clipping for MIAs in elderly patients.
All elderly patients who underwent one-stage embolization or surgical clipping for more than two aneurysms were identified in our hospital between January 2012 and June 2018. Patient characteristics and clinical outcomes at discharge and follow-up were retrospectively evaluated.
A total of 32 patients with 76 IAs underwent one-stage coiling and 21 patients with 46 IAs underwent one-stage clipping were enrolled in this study. Patients who underwent clipping were younger (mean age 64.1 versus 67.2, P = 0.006) and had lower total hospital costs (mean14764 $ versus 24,620 $, P < 0.001) compared with patients who underwent coiling. Immediate posttreatment angiography showed complete occlusion in 56 aneurysms, near complete in 11 and incomplete in 2 in coiling group. Aneurysms wrapping was performed in 2, incomplete clipping in 1 and complete clipping in 41 aneurysms in clipping group. At discharge, 49 (92.5 %) patients were in a good condition (GOS score 4 and 5), 4 (7.5 %) were disabled. At 12 months after discharge, 25 (92.6 %) and 16 (94.1 %) patients had favorable outcomes in the coiling and clipping groups, respectively.
Both one-stage coiling and surgical clipping are safe, effective methods for treating MIAs in elderly patients.
Both one-stage coiling and surgical clipping are safe, effective methods for treating MIAs in elderly patients. The incidence and mortality of lung cancer account for first place all over the world. Lung cancer lacks early diagnostic biomarkers; lung cancer patients are usually diagnosed in both middle and advanced stages and have poor treatment outcomes. BTK inhibitor It is more important to find the first diagnostic tools for lung cancer with high specificity and sensitivity. Besides, exosomes are usually nanometer-sized bi-layered lipid vesicles formed and produced by various types of cells. As one of the main modes of intercellular communication, they can deliver multiple functional biomolecules, such as DNA, microRNAs, messenger RNA (mRNA), long non-coding RNA, and proteins, and the events as mentioned above affects different physiological processes of recipient cells. It has been reported that exosomes are involved in different types of cancer, including lung cancer. Various studies proved that exosomes are involved in multiple cancer processes such as cell proliferation, metastasis, epithelial-mesenchymal transition (EMT), angiogenesis, and the tumor microenvironment in lung cancer. Tumor-derived exosomes (TEX) contain a variety of stimulatory and inhibitory factors involved in regulating immune response, which can affect the tumor microenvironment (TME) and thus participate in the formation and progression of lung cancer. This review's primary purpose to review the latest research progress of exosomes in diagnosing and treating lung cancer.
