Autosomal dominant polycystic kidney disease (ADPKD) is associated with distinct cytopenias in observational studies; the most consistent and strongest association is seen with alternations in the lymphocytic lineages. Although the underlying mechanism of these associations is unclear, it has been hypothesized to be secondary to sequestration of white blood cells in cystic organs, or related to the uremic environment in chronic kidney disease (CKD). However, since mutations in PKD1 or -2 affect several immunomodulating pathways, cytopenia may well be an unrecognized extrarenal manifestation of ADPKD. Furthermore, many important questions on the clinical implications of this finding and the effect on the disease course in these patients are unanswered. In this review article, we provide an overview of the current evidence on cytopenia in ADPKD and explore the underlying mechanisms of this association and its potential prognostic implications. Based on the current literature, we hypothesize that polycystin deficiency can disturb immune cell homeostasis and that cytopenia is thus an intrinsic feature of ADPKD, related to genetic factors. Taken together, these findings warrant further investigation to establish the exact etiology and role of cytopenia in patients with ADPKD.
Leg length discrepancy (LLD) after total hip arthroplasty (THA) prevents functional recovery and reduces patient satisfaction. selleck We investigated impact of changes in patient-perceived LLD on patient satisfaction and walking ability.
one hundred and forty-nine patients with unilateral hip osteoarthritis undergoing THA from 2014 to 2017, (125 women, 24 men; average age, 68.5years) with an objective LLD < 1cm were included. Outcome measures included the patient-perceived LLD, Western Ontario and McMaster Universities Osteoarthritis (WOMAC) index, 10-m walking speed, and affected leg loading rate, assessed preoperatively and at 3weeks, 3months, and 1year postoperatively.
The absolute patient-perceived LLD (mean ± SD) (the number of patients with perceived LLD > 5mm) were 6.4 ± 9.6mm [88 patients (59%)] preoperatively; and 2.2 ± 4.0mm [48 (32%); p < 0.001], 0.7 ± 2.3mm [17 (11%); p < 0.001], and 0.4 ± 1.6mm [10 (7%); p = 0.095] at 3weeks, 3months, and 1year postoperatively, respectively. All outcome measures improved over time. One year postoperatively, a weak positive correlation between the patient-perceived LLD and WOMAC or 10-m walking speed (r = 0.24, 0.23, respectively) was found. The risk of patient-perceived LLD persisting > 1year postoperatively was 5.5-fold higher in patients who exhibited it at 3months and those with a WOMAC score > 10 at 3months postoperatively, using multivariate logistic regression.
Achieving a post-THA objective LLD < 1cm significantly reduced the patient-perceived LLD up to 3months postoperatively. The residual patient-perceived LLD at 1year postoperatively was predicted from the WOMAC score or the presence of patient-perceived LLD at 3months after THA.

Therapeutic level IV.Soil moisture content is a key factor affecting surface evaporation in the hydrologic cycle, with application in water resources management, early drought warning system, irrigation management, and estimation of crop yield. The subject of soil moisture content has long been of interest in hydrology, agriculture, forestry, and soil mechanics engineering. The cracking of surface soil due to dryness is often used to describe the drought condition of the land based on the certain relationship between soil cracking and moisture content. This study establishes the relation between fractal dimensions of soil cracking patterns and moisture content of surface soil. Sixteen soil samples were prepared and subjected to drying under different temperature levels to crack in the laboratory. Photographs of the cracks were digitized and an algorithm for calculating fractal dimensions of cracking patterns was developed. The results demonstrated that the fractal dimensions of cracking patterns are highly correlated with the surface soil moisture content; the lower the soil moisture content, the larger the fractal dimension is. Therefore, the surface soil moisture content-fractal dimension (DB-w) rating curves can be established. Once crack structures occur on surface soil, the moisture content of the surface soil can be estimated quickly and accurately by mean of remote sensing technique such as UAV together with the fractal analysis.Anxiety and depressive disorders are the most common mental health disorders in adolescents, yet only a minority of young people with these disorders access professional help. This study aims to address this treatment gap by improving our understanding of barriers and facilitators to seeking/accessing professional help as perceived by adolescents with anxiety/depressive disorders identified in the community. Twenty-two adolescents, aged 11-17 years, who met diagnostic criteria for a current anxiety and/or depressive disorder were identified through school-based screening. In-depth qualitative interviews were conducted one-to-one with each adolescent and adolescents' parents were interviewed separately for the purpose of data triangulation. Data were analysed using reflexive thematic analysis. We identified four themes capturing adolescent perceived barriers and facilitators to seeking/accessing professional help for anxiety and depressive disorders (1) making sense of difficulties, (2) problem disclosure, (3) ambivalence to seeking help, and (4) the instrumental role of others. Barriers/facilitators identified within each theme reflect important developmental characteristics of adolescence, such as a growing need for autonomy and concerns around negative social evaluation. At the same time, the results highlight adolescents' dependency on other people, mainly their parents and school staff, when it comes to successfully accessing professional help for their mental health difficulties. This study identifies a number of barriers/facilitators that influence help-seeking behaviour of adolescents with anxiety and/or depressive disorders. These factors need to be addressed when targeting treatment utilisation rates in this particular group of young people.
