Stimulant (cocaine and/or methamphetamine) use has increased among people with opioid use disorder. We conducted a systematic review of medications for stimulant use disorders in this population.

We searched for randomized controlled trials in multiple databases through April 2019, and dual-screened studies using pre-specified inclusion criteria. Primary outcomes were abstinence defined as stimulant-negative urine screens for ≥3 consecutive weeks; overall use as the proportion of stimulant-negative urine specimens; and retention as the proportion of participants who completed treatment. We rated strength of evidence using established criteria and conducted meta-analyses of comparable interventions and outcomes.

Thirty-four trials of 22 medications focused on cocaine use disorder in patients with opioid use disorder. Most studies enrolled participants stabilized on opioid maintenence therapy, generally methadone. None of the six studies that assessed abstinence found significant differences between grouprder are lacking in this population. Most of the medications studied for cocaine use were ineffective, although psychostimulants warrant further study.
Pain can return temporarily to old injury sites during opioid withdrawal. The prevalence and impact of opioid withdrawal-associated injury site pain (WISP) in various groups is unknown.

Using data from observational cohorts, we estimated the prevalence and correlates of WISP among opioid-using people who inject drugs (PWID). Between June and December 2015, data on WISP and opioid use behaviours were elicited from participants in three ongoing prospective cohort studies in Vancouver, Canada, who were aged 18 years and older and who self-reported at least daily injection of heroin or non-medical presciption opioids.

Among 631 individuals, 276 (43.7 %) had a healed injury (usually pain-free), among whom 112 (40.6 %) experienced WISP, representing 17.7 % of opioid-using PWID interviewed. In a multivariable logistic regression model, WISP was positively associated with having a high school diploma or above (Adjusted Odds Ratio [AOR] = 2.23, 95 % Confidence Interval [CI] 1.31-3.84), any heroin use in the last six months (AOR = 2.00, 95 % CI 1.14-3.57), feeling daily pain that required medication (AOR = 2.06, CI 1.18-3.63), and negatively associated with older age at first drug use (AOR = 0.96, 95 % CI 0.93-0.99). Among 112 individuals with WISP, 79 (70.5 %) said that having this pain affected their opioid use behaviour, of whom 57 (72.2 %) used more opioids, 19 (24.1 %) avoided opioid withdrawal, while 3 (3.8 %) no longer used opioids to avoid WISP.

Psychiatric illness complicates the clinical course of opioid use disorder (OUD) including treatment using medication for OUD (MOUD). The purpose of this study was to assess the relationship between psychiatric comorbidity and MOUD discontinuation, stratified by whether the client dropped out of treatment or whether MOUD was terminated by the addiction facility.

The study population consisted of individuals with OUD receiving MOUD. Data was derived from the 2015-2017 Treatment Episodes Dataset - Discharges (TEDS-D), which includes discharge records from addiction treatment centers across the United States. The association between psychiatric comorbidity and MOUD discontinuation (including client dropout and facility termination) was assessed using multivariable logistic regression models that included clinically relevant covariates (age, sex, race, education, employment status, living arrangement, prior addiction treatment, intravenous opioid use, primary opioid used at admission, polysubstance use, preving psychiatric illness when providing OUD treatment and suggests that measures to improve MOUD retention for individuals with psychiatric illness are required.Here, we describe DNA enrichment of the zein gene from maize using pyrrolidinyl peptide nucleic acid (PNA) immobilized on a magnetic solid support as a capture element.
