Percutaneous nephrolithotomy is one of the five interventions offered to a patient with renal stone. With the continuous development of noninvasive or minimally invasive techniques, these surgical procedures have been refined over time. This study was conducted to find the success rate of percutaneous nephrolithotomy in renal stone using Guy's score and complication by Modified Clavien score. METHODS This descriptive cross-sectional study was done among 114 patients who underwent percutaneous nephrolithotomy in a tertiary care hospital, from September 2016 to December 2018 after receiving ethical approval from the Institutional Review Committee. Convenient sampling was done. All patients were informed about the potential benefits and risks of the percutaneous nephrolithotomy procedure and patients signed an informed written consent form. Point estimate at 95% Confidence Interval was calculated along with frequency and proportion. Statistical analysis was done by using Statistical Package for Social Sciences version 22.2. RESULTS Forty-six (40.3%) patients had Guy's stone score I, 43 (37.71%) patients had a score of II, 15 (13.6%) patients had a score of III and 10 (8.77%) patients had a score of IV. The success rates of stone clearance were 97.8 %, 95.3%, 80% and 50% for Guy's stone score 1, 2, 3 and 4 respectively. A total of 114 patients were enrolled in the study out of which 66 were male and 48 were female. Eighteen patients experienced some form of complications out of which 3 patients needed surgical intervention with Modified Clavien score of III. CONCLUSIONS Using Guy's scoring system for percutaneous nephrolithotomy we evaluated the success rate. It is reproducible, easy and proves to be a useful tool to counsel patients about stonefree rate and prognosis for the surgical procedure. Modified Clavien score was helpful in evaluating complication rate.INTRODUCTION General anesthesia is feared to have adverse feto-maternal outcomes compared to neuraxial anesthesia. It is recommended to keep rate of caesarean sections under general anesthesia below 5% and 15% for elective and emergency caesarean sections respectively. This study was conducted to find out the prevalence of caesarean sections under general anesthesia at a tertiary care center in western Nepal. METHODS A descriptive cross-sectional study was conducted among caesarean sections conducted at Manipal Teaching Hospital, Pokhara, Nepal from January 2014 to December 2017. Ethical approval was taken from the Institutional Review Committee with reference number MEMG/IRC/GA/122. this website All the caesarean sections conducted during this study period were included in the study using whole sampling method. RESULTS Among 3613 cases, caesarean sections under general anesthesia was observed in 175 (4.84%) in our center over a period of four years at 95% Confidence Interval (4.13-5.55%). The yearly variations ranges from 2.83% to 8.99%. The rate of general anesthesia was found slightly higher in elective 31 (5.82%) as compared to emergency caesarean section 144 (4.67%). CONCLUSIONS The four year medical records of our institution showed fluctuating trend of caesarian sections under general anesthesia. The rate of general anesthesia for emergency caesarian section was within the recommended rate while it was slightly higher in elective caesarian section.INTRODUCTION Verbal abuse is the act of forcefully criticizing, insulting or denouncing another person. Verbal abuse can be devastating to doctors and may cause long lasting emotional and psychological damage. This study aims to find the prevalence of verbal abuse among doctors in tertiary care hospital. METHODS This descriptive cross-sectional study was conducted among doctors in a tertiary care hospitals, Chitwan from January to July, 2019 after taking ethical approval. Convenience sampling was done. Self administered questionnaire was distributed and data was collected. Point estimate at 95% CI was done for binary data along with frequency and proportion. Data were entered and calculations were done in Microsoft excel. RESULTS Verbal abuse was found among 80 (33.3%) respondents at 95% Confidence Interval (27.51- 39.09%) and most 51 (63.8%) of perpetrators were relatives of the patients. Most 38 (47.5%) of the doctors were often worried in workplace. Incident of the verbal abuse was more 35 (43.6%) in morning and least 14 (17.4%) in night. Most 22 (27.5%) of the doctors did not take any action for incident although most 42 (52.5 %) of the doctors were encouraged by colleagues to take action. CONCLUSIONS Prevalence of verbral abuse among the doctors were found out to be similar as the previous studies conducted in similar settings. This study has shown that doctors were frequently verbal abused by patient's relatives and were abused mostly in morning shift and were often worried in workplace.INTRODUCTION Acid base disorder is a condition characterized by alteration in blood pH by the imbalance between the components of blood leading to a life threatening situation. The main aim of this study was to find the prevalence of acid-base disorders and biochemical findings of such disorders in patients in a tertairy care hospital. METHODS This descriptive cross-sectional study was conducted in Nobel Medical College Teaching Hospital from 1st September, 2018 to 31st August, 2019. Ethical apporoval was taken from Institutional Review Committee. All the patients presented to emergency department, intensive care units and wards were included during the study period.
