Estos resultados son útiles para profesionales de la prevención y gestión de la ITCC.in English, Spanish OBJETIVO El objetivo es valorar la efectividad de una intervención de mejora de hábitos posturales para prevenir los trastornos musculoesqueléticos en el sector limpieza. MÉTODO Se diseñó una intervención específica para el colectivo de limpieza, mediante una serie de acciones continuadas en el tiempo dirigidas a fomentar una buena higiene postural y el ejercicio físico. Se aplicó la intervención en dos años distintos (estudio original y estudio de replicación), a una muestra de trabajadores del subsector Limpieza general de edificios, pertenecientes a distintos centros de trabajo de una misma empresa. Se incorporaron herramientas de valoración de las intervenciones basadas en la autopercepción y el registro de los hábitos por un observador externo. RESULTADOS En el primer estudio se obtuvo un aumento del 92% de las personas que manifestaban adoptar siempre o casi siempre buenos hábitos posturales durante las tareas de limpieza. En el estudio de replicación esta mejora fue del 67%. Las principales mejoras se observaron en flexionar las piernas, mantener la espalda recta, alternancia de brazos y buscar puntos de apoyo. En la valoración observacional de los hábitos posturales se constató la mejoría autopercibida. CONCLUSIONES El modelo de intervención fundamentado en una serie de acciones continuadas en el tiempo, favoreciendo el empoderamiento de los trabajadores puede ser efectivo para mejorar, a corto plazo, los hábitos posturales del colectivo de limpieza. Se precisa de más estudios para valorar si el cambio de hábitos se mantiene a largo plazo y su efectosobre la disminución de los trastornos musculoesqueléticos.BACKGROUND Beginning June 2019, Children's Wisconsin was the first hospital to identify a cohort of adolescent patients hospitalized with symptoms likely associated with e-cigarette use. Our report adds to the growing literature describing the radiographic, gross and cytopathologic bronchoscopic findings, and short-term lung function outcomes in this cohort of adolescents with e-cigarette or vaping product use associated lung injury (EVALI). METHODS We present 15 adolescents hospitalized from June to September, 2019 with confirmed EVALI. We abstracted data from inpatient hospitalization and first outpatient pulmonary clinic visit. RESULTS There were 15 patients (11 male, 12 White) with a mean age of 17.1 years. All patients presented with subacute pulmonary, gastrointestinal and constitutional complaints. Diagnostic workup was guided by the Centers for Disease Control criteria for confirmed EVALI case surveillance. Flexible bronchoscopy was performed in 13/15 patients with 10/13 demonstrating gross pathologic abnormalities. Seven of 15 patients required intensive care and 2 met criteria for pediatric Acute Respiratory Distress Syndrome. Patients had dramatic improvement with systemic glucocorticoid therapy and 14/15 were discharged on room air. Eleven patients were seen as outpatients. Despite 11/11 patients reporting resolved or improved symptoms, 7/11 had abnormalities on pulmonary function testing. This study aimed to assess the effectiveness of a newly developed mobile ACT device (mHFCWO-The Monarch Airway Clearance System) in patients with cystic fibrosis (CF). A standard nonmobile HFCWO device (sHFCWO) was used as a comparator. METHODOLOGY This was a randomized, open-label, crossover pilot study. CF patients were treated with each device. Sputum was collected during and after each therapy session, while spirometry tests, Brody score assessment and functional respiratory imaging were performed before and after treatments. RESULTS Wet weight of sputum collected during and after treatment was similar for mHFCWO and sHFCWO (6.53 ± 8.55 vs 5.80 ± 5.82; P = .777). Interestingly, the mHFCWO treatment led to a significant decrease in specific airway volume (9.55 ± 9.96 vs 8.74 ± 9.70 mL/L; P  less then  .001), while increasing specific airway resistance (0.10 ± 0.16 vs 0.16 ± 0.23 KPA*S; P  less then  .001). These changes were heterogeneously-distributed throughout the lung tissue and were greater in the distal areas, suggesting a shift of mucus. Changes were accompanied by an overall improvement in the Brody index (57.71 ± 16.55 vs 55.20 ± 16.98; P = .001). CONCLUSION The newly developed mobile device provides airway clearance for CF patients comparable to a nonmobile sHFCWO device, yielding a change in airway geometry and patency by the shift of mucus from the more peripheral regions to the central airways. © 2020 The Authors. Pediatric Pulmonology published by Wiley Periodicals LLC.Schmallenberg virus (SBV), an insect-transmitted orthobunyavirus that induces severe foetal malformation in calves and lambs, was detected for the first time in late summer 2011 in Central Europe. Thereafter, the virus spread rapidly across the continent causing a large epidemic in the ruminant population. In 2019, detection of virus was again reported more frequently in Germany. From March to November, infections of viremic adult animals were noticed. In September, SBV genome was also detected in newborn lambs. Altogether, affected species included cattle, sheep, a goat and a fallow deer. M-segment sequences were generated from viruses detected in viremic cattle and compared to viral sequences from previous years. The genome of viruses detected in the blood of acutely infected adult cattle and sheep, which represent the circulating SBV strains, seems very stable over the course of nine years and in various European countries. The nucleotide similarities of these viruses are as high as 99.4%-100%. The renewed SBV circulation in 2019 in the country, in which the virus was first detected in 2011 and where it circulated again in 2014 and 2016, suggests the establishment of an enzootic status in Central Europe with regular larger waves in a cycle of around 3 years.
