Each paradigm involves a particular perspective on the purpose of education, the nature of knowledge, what knowledge is valued and included in the curriculum, what it means to learn and how learning is assessed, and the roles of teachers and learners in the learning process. We aim to foster awareness of how these different paradigms look in practice and to illustrate the importance of alignment between teaching, learning and assessment practices with paradigmatic values and assumptions. Finally, we advocate for a pluralistic approach that purposefully and meaningfully integrates paradigms of education, enhancing our ability to drive quality in HPE.Environmental pollution by potentially toxic element (PTE) and the associated health risks in humans are increasingly becoming a global challenge. The current study is an in-depth assessment of PTEs including the often studied lead (Pb), manganese (Mn), zinc (Zn), arsenic (As) and the less-studied titanium (Ti), rubidium (Rb), strontium (Sr), zirconium (Zr), barium (Ba) and thorium (Th) in highly polluted floodplain topsoil samples from the Litavka River, Czech Republic. Soil chemical properties including carbon (Cox) and reaction (pH_H2O) together with iron (Fe) were assessed in the same soils. A portable X-ray fluorescence spectrometer (p-XRFS) (Delta Premium) was used to measure the PTEs and Fe contents of the soils. Soil organic carbon and reaction pH were determined following routine laboratory procedures. The concentration level of each PTE was compared against world average and crustal values, with the majority of elements exceeding the aforementioned geochemical background levels. Distributions of the PTEs were mapped. Two pollution assessment indices including enrichment factor (EF) and pollution index (PI) levels were calculated and their means for Zn (43.36, 55.54), As (33.23, 43.59) and Pb (81.08, 103.21) show that these elements were enriched. The EF and PI distribution maps corresponded with the concentration distribution maps for each PTE. On health risk assessment, hazard quotients (HQ) in different human groups varied. Children had the highest HQs for all PTEs than adults (women and men). PTEs with high HQ levels in distinct human groups were As, Zr and Pb. Zirconium is a less likely element to pose a health risk in humans. Nonetheless, it should be kept in check despite its low pollution occurrence.Polycyclic aromatic hydrocarbons (PAHs) are an important group of compounds of major environmental concern, which are in the class of persistent organic pollutants. Therefore, the key purpose of this research was to analyze seasonal fluctuations and to determine the probability of polycyclic aromatic hydrocarbons in coastal sediments of the Iranian Marine Resource Center based on the evaluation of 16 US-EPA important PAH compounds. These compounds have been collected from intertidal sediments located in the marine resources of southern Iran in different seasons. These samples of the surficial sediment were collected at the PSEEZ area using a stainless steel grab sampler in four seasons, from depths between 0.5 and 30 m. Surface sediment samples were removed by spoons and carefully placed in an aluminum foil; they were taken to the laboratory on ice and held at 20° C until their study. After extraction, by using a rotary evaporator apparatus, samples were condensed. The assay was added to roughly 2 g of activaontamination studies and the level of PAH contamination. Therefore, this study will provide a scientific background, planning, and policies for PAH pollution control and environmental protection in Iran and similar regions around the world.
Spasticity is a common and disabling symptom in patients with multiple sclerosis (PwMS) as highlighted by many epidemiological studies, it is often a severe and not well treated. Despite the availability of evidence-based spasticity management guidelines, there is still great variability in everyday therapeutic approach, especially for the most complex cases.
In our single-centre study, we retrospectively evaluated PwMS-treated nabiximols and botulinum toxin injections (BTI) from July 2015 to April 2019. Clinical and demographic data were collected. The severity of spasticity and spasms was recorded by modified Ashworth Scale (mAS) and Penn Spasm Frequency Scale (PSFS) at baseline and after 1 month of treatment.
We evaluated 64 treatments for MS-related spasticity 28 patients were treated with BTI and 36 patients with nabiximols. We found that both BTI and nabiximols are effective in reducing mAS (nabiximols, BTI p < 0.001), PSFS frequency (nabiximols p = 0.001, BTI p = 0.008) and intensity (nabiximols p = 0.001, BTI p = 0.016). No differences were found when directly comparing the efficacy of the two treatments, except for a statistical trend favouring BTI on spasms intensity (p = 0.091). Eleven patients were treated with both BTI and nabiximols, and only four patients continued both treatments. All dropouts were due to inefficacy of at least one of the two therapies.
Our single-centre experience highlights that both BTI and nabiximols are effective in treating multiple sclerosis-related spasticity; however, BTI treatment may be more effective on spasms intensity. Combined nabiximols and BTI treatment could represent a therapeutic option for severe spasticity.
