Cytological diagnosing microfilariae throughout technically unsuspected cases: A new retrospective review of Twelve situations.
. These results suggested that combined use of anti-HIF-1α nanobodies with first-line treatment may in the future be an effective treatment for pancreatic cancer. This article is issued from a reflective approach developed from a case observed during the internship of Dr Van Nieuwenhove in the neuro-resuscitation department of the intensive care unit of the Geneva University Hospitals (Switzerland) in July 2018. This article sought to analyse the similarities and differences between two major attitudes towards assisted deaths, namely euthanasia in Belgium and assisted suicide in Switzerland. The relevant legislations and ethical considerations are presented with the value at stake in these particular end-of-life management. The whole reflective process discussed here is based on the assumption that the patient has made the request to die or had requested the end of his suffering. The reflections and questions are therefore only simple speculations allowing to open a discussion.Compared with the general population, oncology patients face a higher morbidity and mortality caused by the COVID-19 pandemic. As a result, health systems had to quickly adapt cancer care in order to maintain the best quality and patient safety. From March to May and from October to December 2020, 254 patients diagnosed with cancer and tested positive for SARS-CoV-2 benefited from a tele-health monitoring at the Oncology Department at CHUV. This article describes the key points of the development, implementation and operation of this tele-health monitoring, enabled by an interdisciplinary and inter-professional collaboration between different units and healthcare professionals.Small fiber neuropathy (SFN) causes damage to small-calibre nerve fibers (unmyelinated C fibers and myelinated A-delta fibers). The symptoms of SFN usually are sensitive including paresthesia, dysesthesia or burning pain, and protopathic deficits, sometimes associated with dysautonomia. The causes of SFN can be classified in six main groups idiopathic, toxic, metabolic, immunological, infectious and hereditary. In this article, we present the diagnostic approach to SFN, the most common autoimmune aetiologies, as well as elements of their therapeutic management. Many vaccine strategies have been developed to control the COVID-19 pandemic. This article presents the mechanisms of action and the efficacy of different vaccines including mRNA- and adenovirus-based vaccines. We will discuss the different vaccine targets, immune responses and allergic reactions which have been reported during the vaccination campaigns. Finally, the latest recommendations for the prevention and management of severe allergic reactions will be summarized.Systemic lupus erythematosus is a complex autoimmune disease that remains challenging to treat. Recent advances in the understanding of the pathogenesis of SLE pave the way for the evaluation of biologic medicine. The most promising therapeutic targets in SLE are those that interfere with B cells count or normal function, interferon inhibitors, JAK inhibitors and biologicals that alter the cytokines imbalance that characterizes SLE. Recent phase 3 clinical trials have evaluated the role of belimumab in lupus nephritis and the usefulness of anifrolumab in the treatment of moderate to severe SLE. Many more trials are currently underway and may improve the level of care of patients with SLE in the near future.Cannabis-fruit and vegetable syndrome is of recent discovery and linked to lipid transfer protein (LTP) sensitization. It is thought that the primary sensitization originates from the cannabis LTP (Can s 3). Sensitized patients can cross-react to others LTP homologs such as peach LTP (Pru p 3). Diagnosis may be challenging, as consumption of cannabis is often omitted by the patient and needs to be specifically addressed during the interview. Thus, meticulous history taking is mandatory. Laboratory workup includes LTP-specific IgE and skin testing. Management relies on allergen eviction.Hen's egg allergy is the second major cause of food allergy in children and is rare in adults. It can present with diverse and sometimes severe symptoms that can be challenging for patients and clinicians. Allergy history represents the main diagnostic tool, and can be completed with skin tests, specific IgE testing and in selected cases with an oral challenge test. All patients with egg anaphylaxis should benefit from an emergency kit in case of reaction and specialized allergologic evaluation. Strict eviction of eggs can be difficult to achieve because of hidden allergens. this website For safety purpose, an oral tolerance induction is helpful selected patients. A pre-vaccination assessment is required in rare selected cases.Food hypersensitivity represents a group of adverse immunological reactions linked to food ingestions such as anaphylaxis or eosinophilic esophagitis. Identification of the responsible food is currently based on a detailed history and complementary exams, mostly prick testing and assessment of serum specific IgE, either by ImmunoCAP or multiplex assay. Other skin and laboratory tools such as functional in vitro or patch testing are currently not validated although some show promising results in the field of food allergy. This article presents the different diagnostic procedures that are commonly and less commonly used in the workup of food allergies and their potential for future use in the workup of food-induced hypersensitivity reactions. Non-immunological reactions, rare entities (Heiner syndrome) and celiac disease will not be addressed in this review. In the first article we described how pharmaceutical industry is facing major challenges in general. In this second part, we intend to focus on new concepts and developments, which might sound «futuristic» for most of us. However, this seems definitely «work in rapid progress», especially concerning, for example, the use of the pharmacogenome (prerequisite for treatment personalization), «clever» medication, 3D printing and labelling with QR code. The future is bright but we cannot omit to quote the ever growing role of incumbent actors, both in the fields of medication production as well as distribution.
