Leech hearts are modular two lateral segmented heart tubes running the length of the animal. Moving blood through the segmented heart tubes of leeches requires sequential constrictions, timed by a heartbeat CPG and relayed to each heart segment by likewise segmental motor neurons. The heartbeat CPG produces bilaterally asymmetric coordinations rear-to-front peristaltic on one side and nearly synchronous on the other, periodically switching sides. We examined the neuromuscular transform of isolated heart segments in response to electrical nerve stimulation to identify the range of parameters (burst duration, intraburst pulse frequency, period) allowing the heart to constrict continuously and reliably. Constriction amplitudes increased with increasing intraburst frequencies and decreased with decreasing burst durations. Similar amplitudes were achieved withe the neuromuscular transform. Decreasing the cycle period, e.g., to increase volume pumped over time, without altering motor burst duration and intraburst spike frequency shortens relaxation time and decreases amplitude. The likely strategy to preserve constriction amplitude is to shorten burst duration while increasing spike frequency.OBJECTIVE. Utilization of retrievable inferior vena cava filters (rIVCFs) has come under increased scrutiny because of historically high rates of placement, generalized lack of retrieval when the inferior vena cava filter (IVCF) is no longer indicated, and reports of device-related complications. These events have led to an increased interest in IVCF retrieval, including the development of advanced endovascular retrieval techniques and the proliferation of specialized clinical practices for rIVCFs. We aim to describe the indications for IVCF retrieval, patient selection, procedural planning, and procedural complications and management. CONCLUSION. IVCFs continue to have a role in the prevention of pulmonary embolism in select patients. Rising awareness of device-related complications paired with historically low retrieval rates has prompted renewed emphasis and interest in filter retrieval. Diligent follow-up and procedural planning permit prompt and safe filter retrieval.OBJECTIVE. The purpose of this study was to evaluate in a multicenter dataset the performance of an artificial intelligence (AI) detection system with attention mapping compared with multiparametric MRI (mpMRI) interpretation in the detection of prostate cancer. MATERIALS AND METHODS. MRI examinations from five institutions were included in this study and were evaluated by nine readers. In the first round, readers evaluated mpMRI studies using the Prostate Imaging Reporting and Data System version 2. After 4 weeks, images were again presented to readers along with the AI-based detection system output. Readers accepted or rejected lesions within four AI-generated attention map boxes. Additional lesions outside of boxes were excluded from detection and categorization. The performances of readers using the mpMRI-only and AI-assisted approaches were compared. RESULTS. The study population included 152 case patients and 84 control patients with 274 pathologically proven cancer lesions. The lesion-based AUC was 74.9% for MRI and 77.5% for AI with no significant difference (p = 0.095). The sensitivity for overall detection of cancer lesions was higher for AI than for mpMRI but did not reach statistical significance (57.4% vs 53.6%, p = 0.073). However, for transition zone lesions, sensitivity was higher for AI than for MRI (61.8% vs 50.8%, p = 0.001). Reading time was longer for AI than for MRI (4.66 vs 4.03 minutes, p less then 0.001). There was moderate interreader agreement for AI and MRI with no significant difference (58.7% vs 58.5%, p = 0.966). CONCLUSION. Overall sensitivity was only minimally improved by use of the AI system. Significant improvement was achieved, however, in the detection of transition zone lesions with use of the AI system at the cost of a mean of 40 seconds of additional reading time.OBJECTIVE. The purpose of this study was to add to evidence of the long-term prognostic value of coronary CT angiography (CCTA) when combined with nonimaging risk factors and to investigate how CCTA can contribute to the decision to start statin therapy. MATERIALS AND METHODS. Patients underwent CCTA in an outpatient setting for a variety of indications. The National Death Index from February 2004 through December 2018 was queried to identify the outcomes of death due to any cause (all-cause mortality) and death due to coronary artery disease. Framingham and machine learning risk estimation models were constructed. SUBJECTS AND METHODS. Seventeen patients who underwent cardiac MRI with gadolinium injection as part of follow-up of repaired tetralogy of Fallot from March 2017 to July 2018 were prospectively included in the initial study population; the final study cohort was composed of 15 of the 17 patients. Ventricle volume measurements were performed on a 3D cine kat-ARC sequence, a 4D flow kat-ARC sequence, and a 2D cine balanced steady-state free precession (bSSFP) sequence. The 2D cine bSSFP sequence was the reference standard in cardiac assessment. Intertechnique and interobserver analyses were performed. Bland-Altman analysis and correlation tests were used to compare quantitative measurements.
