Rates have been demonstrated as high as 55%, but the diagnosis of malnutrition has historically been made in only a minority of inpatients. Laboratory studies, including serum prealbumin level, have been shown to have poor predictive value of malnutrition. In 2014, our institution embarked on a system-wide effort to improve diagnosis of malnutrition in hospitalized patients. We adopted the Academy of Nutrition and Dietetics/American Society for Parenteral and Enteral Nutrition (AND/ASPEN) Clinical Characteristics and implemented the Nutrition-Focused Physical Exam/Assessment into clinical practice. Dietitians recorded malnutrition diagnoses in a flow sheet in the electronic medical record (EMR) and alerted the primary team when a patient met criteria for malnutrition. An editable link to malnutrition diagnosis was created in the discharge summary templates in the EMR. Over 4 years, these efforts led to an increase in our rate of diagnosis of malnutrition from 6% to 12%, which was sustained over the last 2 years. We also found that the percentage of inpatients having serum prealbumin levels checked decreased from 13% to 8% over the study period. We found that a system-wide, stepwise approach to improving our diagnosis of inpatients with malnutrition was effective and appears sustainable over the period studied. Mitochondrial dynamics, coupled with mitophagy, is critical for the maintenance of mitochondrial fitness, involving many cellular processes, such as proliferation and apoptosis. Excessive mechanical stress induces chondrocyte apoptosis; however, the effects of mechanical stress on mitochondrial dynamics remain elusive. In this study, we performed fluorescence staining, flow cytometry, transmission electron microscope, Western blot analysis, and RNA-sequencing to assess the effects of different strength of mechanical stimulation on mitochondrial functions of chondrocyte treated with interleukin-1β (IL-1β). We found that moderate mechanical stress reduced the IL-1β-induced apoptosis by maintaining mitochondrial function and scavenging the reactive oxygen species, while excessive mechanical stress induced strong mitochondrial dysfunction and apoptosis. Moreover, RNAsequencing revealed that mitophagy and mitochondrial dynamics were involved in the regulation of mechanical stress on chondrocyte biology. In addition to the elevated mitophagy, moderate mechanical stress also promoted mitochondrial dynamics by enhancing the expression of MFN1/2 and OPA1 and the translocation of dynamin-related protein 1 from the cytoplasm to the mitochondria. However, an uncoupling of mitochondrial dynamics, characterized by strongly elevated fission, resulted in the unfavorable apoptosis of excessive mechanical stress-stimulated chondrocytes. To determine if various risk factors have the same effect on mortality in octogenarians as in younger patients.

The National Inpatient Sample data set from 2004 to 2014 was queried to select patients aged 65 years and older who underwent either coronary artery bypass grafting (CABG), valvular heart surgery (VHS), or both (CABG + VHS) within 10 days of hospital admission. The patients were divided into two groups 65-79 years and 80 years and greater. Hospital mortality, patient demographics, comorbidities, and type of hospital admission was evaluated and compared using χ
and multivariable logistic regressions.

About 397,713 patients were identified including 86,345 (21.7%) aged 80 and above. Octogenarians had higher in-hospital mortality for all procedures CABG (4.94% vs. 2.39%, p < .001), VHS (5.49% vs. 4.08%, p < .001), and CABG + VHS (7.59% vs. 5.95%, p < .001), and this relationship persisted ssociated with in-hospital mortality. Differing effect sizes suggest that certain risk factors, such as renal and heart failure, contribute more to mortality in younger patients.

The current study examined whether heritable influences on parenting and children's behavioural outcomes operate through child negative emotionality.

Using data from the Early Growth and Development Study, we examined associations among adoptive parent reports of child anger and sadness at 4.5years, adoptive parents' hostile and warm parenting at 6years and child behavioural problems and social competence at age 7. Birth parent temperament was included to test whether child effects on parents reflect evocative gene-environment correlation (rGE).

Child anger at 4.5years evoked hostile parenting from adoptive parents at 6years, which was subsequently related to child problem behaviours at 7years. Evocative rGE effects were identified for adoptive parents' hostile parenting.

By employing a genetically informed design, we found that birth parent temperament was related to child negative emotionality.
