In the field of global health, there are very little data available regarding disability. Global health datasets including disability indicators operationalize the term disability in a variety of ways, making calculating global trends and cross-country comparisons by researchers difficult. The Washington Group on Disability Statistics has developed easily implemented questionnaires regarding disability to increase the availability of disability data. In 2014, the Demographic and Health Surveys program developed a disability data collection module based on the Washington Group Short Set of Questions. It is an optional module and has been implemented in fewer than 20 countries. Some countries had made adjustments to the questions while other countries have included disability using completely different questionnaires. This doesn't solve the problem of data consistency and cross-country comparison. Based on the data collected, there are also concerns regarding the quality of the available data and how the data is being collected.
While there are numerous studies comparing open-view autorefractors to subjective refraction or other open-view autorefractors, most studies between closed and open-view autorefraction tend to focus on children rather than young adults. The aim of this study was to determine the concordance in non-cycloplegic refractive error between two modern objective autorefractors the closed-view monocular Topcon TRK-2P and the binocular open-view Grand Seiko WR-5500.

Fifty young adults aged 20-29 years (mean age 22 ± 1.6 years) underwent non-cycloplegic autorefraction using the Grand Seiko WAM-5500 (open view) and Topcon TRK-2P (closed-view) autorefractors on both eyes. Findings were expressed as the isolated spherical component and were also converted from clinical to vector notation Mean Spherical Error (MSE) and the astigmatic components J0 and J45.

Mean MSE ± SD was -1.00 ± 2.40D for the Grand Seiko WAM-5500 compared to -1.23 ± 2.29D for the Topcon TRK-2P. Up to seventy-six percent of the cohort had mean spherical errors from the Topcon TRK-2P which fell within ±0.50D of the Grand Seiko reading and 58% fell within ±0.25D. Mean differences between the two instruments were statistically significant for all components (J0, spherical, and MSE) (p < 0.01), except J45 (p > 0.05).

The differences in non-cycloplegic MSE between these two instruments are small, but statistically significant. From a clinical perspective the Topcon TRK-2P may serve as a useful starting point for subjective refraction, but additional work is needed to help further minimise differences between the instruments.

Cadaverine, a 1,5-diaminopentane (DAP), is widely used as block chemicals for synthesis of biopolymer, which can be produced from lysine by lysine decarboxylase (EC 4.1.1.18) in Escherichia coli. However, the DAP will be further utilized into by-products through downstream genes of speE, puuA, speG and ygjG, which decrease the amount of product. In this study, two approaches including Lambda-Red system for gene knockout, and clustered regularly interspaced short palindromic repeats interference (CRISPRi) for gene knockdown; are explored to manipulate the metabolic flux among 26 genetic E. coli. As a result, CadA driven by inducible T7 promoter accumulated more DAP from CRISPRi targeted on single-gene repressive strains such as BT7AiE, BT7AiP, BT7AiG and BT7AiY. The highest DAP titer and productivity was obtained to 38 g/L and 2.67 g/L/h in BT7AiY (repression of ygjG). We also investigated the co-factor pyridoxal 5'-phosphate (PLP) effect on lysine consumption and DAP production from different E. coli derivatives. In contrast to CRISPRi-mediated strains, 4 genes knockout strain (BT7AdEPGY) deal with 98% lysine consumption and achieved 37.45 g/L DAP and 3.17 g/L/h DAP productivity. The metabolic regulation by CRISPRi is a simple strategy and the results are consistent with gene knockout to manipulate the pathway for DAP production.Hemoglobin-vesicles (Hb-V) are being developed as red blood cell (RBC) substitutes. In this study, we report on quantitative and qualitative alterations of hepatic cytochrome P450 (CYPs) and the pharmacokinetics of CYP-metabolizing drugs, with a focus on four CYP isoforms (CYP1A2, CYP2C11, CYP2E1 and CYP3A2), after Hb-V resuscitation from a massive hemorrhage. The results of proteome analysis and western blot data indicate that resuscitation with both Hb-V and packed RBC (PRBC) resulted in a decrease in the protein levels of CYPs. Along with a decrease in the protein expression of CYPs, pharmacokinetic studies showed that the elimination of CYP-metabolizing drugs was prolonged in the Hb-V and PRBC resuscitation groups. It is also noteworthy that the CYP-metabolizing drugs in the Hb-V resuscitation group was retained for a longer period compared to the PRBC resuscitation group, and this is attributed to the CYP isoforms having a lower metabolic activity in the Hb-V resuscitation group than that for the PRBC resuscitation group. These findings suggest that resuscitation with Hb-V after a massive hemorrhage has a slight but not clinically significant effect on drug metabolism via CYPs in the liver due to decreased protein levels and the metabolic activity with respect to the CYPs.
The COVID-19 pandemic has drastically transformed the healthcare community and medical education across the United States. The aim of this study was to evaluate the impact of COVID-19 on the surgical resident training experience, assess possible sources of stress or anxiety among surgery residents, and examine how patterns of anxiety vary by resident rank.

We developed and disseminated a survey, which included the Generalized Anxiety Disorder 7-Item Scale (GAD-7), to all general and integrated plastic surgery residents in their clinical years of training at the University of California, San Francisco. Statistical analysis of the survey responses was performed using the Kruskal-Wallis or Wilcoxon rank sum test. Post-hoc analysis was performed using the Bonferroni-corrected Dunn test. Survey data were combined with aggregated duty hour information and operative case numbers from select hospitals for March and April of 2019 (historical baseline) and 2020.

The overall survey response rate was 73.7% (n = 73).
