Introduction The question of our study was to evaluate the incidence of coexisting outlet impingement among patients with calcifying tendinitis and a failure of the conservative treatment using intraoperative in addition to radiological criteria. This question is of clinical relevance as there is still a discussion about whether an additional arthroscopic subacromial decompression (ASD) should be performed when arthroscopic removal of the calcific deposits is needed. Materials and methods From February 2017 to 02/2018, we prospectively enrolled 50 patients who needed shoulder arthroscopy for calcifying tendinitis. We evaluated the x-rays for outlet impingement, measuring the lateral acromial angle (LAA), acromion slope, acromion index (AI) and Bigliani-type of the acromion. During shoulder arthroscopy, we evaluated the acromial undersurface for a bony impingement using a standardized view. If impingement was present, we performed an additional ASD. Before surgery, three months later and at final follow-up 20 months after surgery, we determined the relative Constant Score (rCS). Results 92.5% of the patients had an intraoperatively confirmed outlet impingement, whereas 82.5% of the patients had a radiological outlet impingement. The sensitivity of the radiographs was 83.8%, the specificity 33.3% when combining all parameters. The mean rCS improved significantly from 45% to 100% at final follow-up. Conclusion Our results show that coexisting outlet impingement is very common in patients with calcifying tendinitis and failure of the conservative treatment. As the radiological diagnosis is uncertain, an intraoperative assessment for impingement is rational and justifies an additional ASD. © 2020 Professor P K Surendran Memorial Education Foundation. With excellent outcomes, primary UCL reconstruction has allowed many overhead athletes to return to their pre-injury sport. However, the subjective factors influencing this decision to return to sport have yet to be studied. The aim of this study is to understand the factors influencing an athlete's decision to return to pre-injury level of sport after primary UCL reconstruction. Methods An experienced interviewer conducted qualitative, semi-structured interviews of patients aged 18-35 years who had undergone primary UCL reconstruction by one fellowship-trained, Major League Baseball (MLB) team orthopaedic surgeon. All subjects were throwing athletes prior to injury and had a minimum two-year follow-up without revisions. Qualitative analysis was then performed to derive codes, categories, and themes. Patients were surveyed to assess familiarity with UCL reconstruction as well as to obtain Kerlan-Jot in their care team's reputation, intrinsic personality traits, anecdotal evidence about post-operative outcomes, and lifestyle. This study emphasizes the importance for health care providers to educate patients toward realistic expectations upon return to sport. On a larger scale, this study illustrates the effects the media and anecdotal experiences of a growing population of players undergoing UCL reconstruction have had on the game of baseball and players' decisions to return to sport. However, despite this growth, only a small number of NSAs have been able to maintain a prolonged global presence over a substantial period of time. Ferrostatin-1 purchase To explain why some NSAs are more active, we rely on resource dependence theory. We demonstrate that sustained advocacy over time can be explained by a density dependence mechanism, namely the more NSAs mobilize, the lower the chance that individual NSAs will prolong their global advocacy efforts. Analysis of data stemming from a unique data set of 5,627 NSAs active at the global climate conferences demonstrates that much advocacy in this field is indeed of an incidental nature, namely a large number of groups attend once and never return. © 2019 The Authors. Governance published by Wiley Periodicals, Inc.A rapid, facile and selective detection of anti-H5 subtype avian influenza virus (AIV) antibody in serum by fluorescence polarization immunoassay (FPIA) was achieved. A fragment of recombinant H5 subtype AIVhemagglutinin was produced and labeled with fluorescein to use it as a labeled antigen in FPIA. This labeled antigen was mixed with anti-AIV sera (H1-H16 subtypes) and FP of the mixture was measured using a portable FP analyzer on a microdevice. It was found that FP increased in proportion to the concentration of anti-H5 AIV antibody (serum) and was significantly higher than FP obtained with the other sera. The selective detection of anti-H5 subtype AIV antibody was confirmed. The required volume of original sample was 2 μL and analysis time was within 20 min. This detection system realizes an efficient on-site diagnosis and surveillance of AIV. © 2020 Published by Elsevier B.V.Fuel cell vehicles fueled with renewable hydrogen is recognized as a life-cycle carbon-free option for the transport sector, however, the profitability of the H2 pathway becomes a key issue for the FCV commercialization. By analyzing the actual data from the Zhangjiakou fuel cell transit bus project, this research reveals it is economically feasible to commercialize FCV in areas with abundant renewable resources. Low electricity for water electrolysis, localization of H2 supply, and curtailed end price of H2 refueling effectively reduce the hydrogen production, delivery and refueling cost, and render a chance for the profitability of refueling stations. After the fulfillment of the intense deployment of both vehicles and hydrogen stations for the 2022 Winter Olympics, the H2 pathway starts to make a profit thereafter. The practices in the Zhangjiakou FCB project offer a solution to the hydrogen economy, which helps to break the chicken-egg dilemma of vehicles and hydrogen infrastructure. © 2020 Hydrogen Energy Publications LLC.
