CONCLUSIONS Our findings showed that psychological problems such as anxiety and high unmet supportive needs are correlated with delayed initiation of adjuvant chemotherapy in colorectal cancer patients.STUDY DESIGN Retrospective analysis. OBJECTIVE Analysis of a standardized, pre-surgical psychological evaluation program for complex spine surgery. Adult spinal deformity (ASD) patients have a high rate of comorbid mental health conditions. Although there is a body of literature demonstrating the impact of psychological factors, including anxiety and depression, on spine surgery outcome, it is estimated that spine surgeons utilize a psychological assessment only about one third of the time prior to a patient's spine surgery. At this time, there is not a widely reported pre-surgical psychological evaluation program for ASD patients. METHODS 129 consecutive complex spine surgery candidates receiving a pre-surgical psychological evaluation were analyzed between January 1st 2014 and December 31st 2018. Based on the available literature and professional experience in our facility, a color code for patients was developed from Green (low psychological or psychosocial co-morbidity) to Red (high psychological or psychg evaluated for surgical reconstruction of adult spinal deformities. LEVEL OF EVIDENCE 3.The original article has been corrected.Hypoxic-ischemic encephalopathy, also referred as HIE, is a type of brain injury or damage that is caused by a lack of oxygen to the brain during neonatal period. The incidence is approximately 1.5 cases per 1000 live births in developed countries. In low and middle-income countries, the incidence is much higher (10‒20 per 1000 live births). The treatment for neonatal HIE is hypothermia that is only partially effective (not more than 50% of the neonates treated achieve an improved outcome). HIE pathophysiology involves oxidative stress, mitochondrial energy production failure, glutaminergic excitotoxicity, and apoptosis. So, in the last years, many studies have focused on peptides that act somewhere in the pathway activated by severe anoxic injury leading to HIE. This review describes the pathophysiology of perinatal HIE and the mechanisms that could be the target of innovative HIE treatments.PURPOSE OF REVIEW Heart failure with preserved ejection fraction (HFpEF) or diastolic heart failure (DHF) makes up more than half of all congestive heart failure presentations (CHF). With an ageing population, the case load and the financial burden is projected to increase, even to epidemic proportions. CHF hospitalizations add too much of the financial and infrastructure strain. Unlike systolic heart failure (SHF), much is still either uncertain or unknown. Specifically, in epidemiology, the disease burden is established; however, risk factors and pathophysiological associations are less clear; diagnostic tools are based on rigid parameters without the ability to accurately monitor treatments effects and disease progression; finally, therapeutics are similar to SHF but without prognostic data for efficacy. RECENT FINDINGS The last several years have seen guidelines changing to account for greater epidemiological observations. Most of these remain general observation of shortness of breath symptom matched to static echocardiographic parameters. The introduction of exercise diastolic stress test has been welcome and warrants greater focus. HFpEF is likely to see new thinking in the coming decades. This review provides some of perspective on this topic.Augmented Reality (AR) in monocular liver laparoscopy requires one to register a preoperative 3D liver model to a laparoscopy image. This is a difficult problem because the preoperative shape may significantly differ from the unknown intraoperative shape and the liver is only partially visible in the laparoscopy image. Previous approaches are either manual, using a rigid model, or automatic, using visual cues and a biomechanical model. We propose a new approach called the hybrid approach combining the best of both worlds. The visual cues allow us to capture the machine perception while user interaction allows us to take advantage of the surgeon's prior knowledge and spatial understanding of the patient anatomy. The registration accuracy and repeatability were evaluated on phantom, animal ex vivo and patient data respectively. The proposed registration outperforms the state of the art methods both in terms of accuracy and repeatability. An average registration error below the 1 cm oncologic margin advised in the literature for tumour resection in laparoscopy hepatectomy was obtained.The risk of fatigue damage within the L5-S1 spinal disc was calculated for a sample of 52 individuals with unilateral limb loss (26 transtibial; 26 transfemoral) and 26 uninjured controls using a non-linear multi-axial fatigue model of the spine motion segments. Time to complete damage was calculated for each participant and walking pace; the influences of walking activity were determined by varying daily step counts. Assuming similar activity across groups (10,000 steps per day), times to failure were not different between persons with and without limb loss (50 [23] vs. 46 [24] years, respectively); walking faster was associated with shorter times to failure. Greater daily step counts similarly decreased time to failure in all groups. While such a similarity in fatigue damage evolution does not necessarily downplay the role of biomechanical factors, it may highlight the important influences of psychosocial factors in the development of low back pain in persons with limb loss. Nevertheless, with additional work to include activities beyond walking, computational damage models can provide a predictive platform for evaluating specific clinical interventions (e.g., behavior modification, movement retraining, prosthetic devices) that are ultimately intended to mitigate physical risk factors for low back pain following limb loss.Gastric Cancer epidemics have changed over recent decades, declining in incidence, shifting from distal to proximal location, transforming from intestinal to diffuse histology. Novel chemotherapeutic agents combined with modern surgical operations hardly changed overall disease related survival. This may be attributed to a substantial inherent geographical variation of disease genetics, but also to a failure to standardize and implement treatment protocols in clinical practice. To overcome these drawbacks in Greece and Cyprus, a Gastric Cancer Study Group under the auspices of the Hellenic Society of Medical Oncology (HeSMO) and Gastrointestinal Cancer Study Group (GIC-SG) merged their efforts to produce a consensus considering ethnic parameters of healthcare system and the international proposals as well.
