The purpose of this study is to compare the early radiologic diagnosis of pulmonary infection between serial chest radiography (chest film) and single chest computed tomography (CT chest) in the first seven days of febrile neutropenia.

This study included 78 patients with hematologic malignancies who developed 107 episodes of febrile neutropenia from January 2012 to October 2017 and had a chest film performed within the first seven days. Demographic and radiographic data were retrospectively reviewed. Three radiologists independently and blindly evaluated chest films and CT chests. The sensitivity, specificity, and correlation of chest film with absolute neutrophil count were carried out.

A total of 222 chest films were performed during this period and found thirty-nine episodes (36.4%) of radiographic active pulmonary infection. The diagnosis of clinical positive for pulmonary infection is 44.8% (48/107). Sensitivity, specificity, positive predictive value, and negative predictive value of serial chestiologic diagnosis of pulmonary infection within the first seven days of febrile neutropenia has lower sensitivity with higher specificity as compared to a single CT chest. Conversely, CT chest may not only have a higher sensitivity in determining early pulmonary infection but also has a higher rate of false-positives.
Massive blood loss is the most common cause of immediate death in trauma. A massive blood transfusion (MBT) score is a prediction tool to activate blood banks to prepare blood products. The previously published scoring systems were mostly developed from settings that had mature prehospital systems which may lead to a failure to validate in settings with immature prehospital systems. This research aimed to develop a massive blood transfusion for trauma (MBTT) score that is able to predict MBT in settings that have immature prehospital care.

This study was a retrospective cohort that collected data from trauma patients who met the trauma team activation criteria. The predicting parameters included in the analysis were retrieved from the history, physical examination, and initial laboratory results. The significant parameters from a multivariable analysis were used to develop a clinical scoring system. The discrimination was evaluated by the area under a receiver operating characteristic (AuROC) curve. The calibration was demonstrated with Hosmer-Lemeshow goodness of fit, and an internal validation was done.

Among 867 patients, 102 (11.8%) patients received MBT. Four factors were associated with MBT a score of 3 for age ≥60 years; 2.5 for base excess ≤-10 mEq/L; 2 for lactate >4 mmol/L; and 1 for heart rate ≥105 /min. The AuROC was 0.85 (95% CI 0.78-0.91). At the cut point of ≥4, the positive likelihood ratio of the score was 6.72 (95% CI 4.7-9.6,
< 0.001), the sensitivity was 63.6%, and the specificity was 90.5%. Internal validation with bootstrap replications had an AuROC of 0.83 (95% CI 0.75-0.91).

The MBTT score has good discrimination to predict MBT with simple and rapidly obtainable parameters.

Poor nutritional status affects the normal process of the wound healing stage. There is limited evidence regarding the association between malnutrition and wound healing in Ethiopia.

To assess the association between nutritional status and wound healing progress among adult individuals who had undergone abdominal surgery at Public Hospitals, Ethiopia.

A prospective cohort study was conducted on 310 adult patients who had undergone abdominal surgery from August to December 2019. Data were collected using a standardized, structured, and pretested questionnaire. Anthropometric and serum albumin measurements were used to measure nutritional status. A multivariable Cox-regression analyses model was fitted to show the association between malnutrition and wound healing and
value < 0.05 was used to declare statistical significance value.

The cumulative incidence rate of good wound healing was 65.5% (95% CI 60.0-71.0). Patients who had normal preoperative body mass index (adjusted hazard ratio (AHR) = 2.22 (95% CI 1.55-3.19)) and normal range of serum albumin level (≥3.5) (AHR = 1.56 (95% CI 1.05-2.29)) were significantly associated with better wound healing outcomes.

Nutritional status had a strong association with good wound healing outcomes. Therefore, nutritional status screening should be done for all adult patients before undergoing abdominal surgery to improve wound healing outcomes and reduce hospital stays.

Recent developments in optical coherence tomography (OCT) have enabled detailed in vivo studies about the architectural characteristics of the LC. Structural characteristics of the LC have been widely explored in glaucoma management. However, information about which LC biomarkers could be useful for the diagnosis, and follow-up, of other diseases besides glaucoma is scarce. Hence, this literature review aims to summarize the role of the LC in nonophthalmic and ophthalmic diseases other than glaucoma. PubMed was used to perform a systematic review on the LC features that can be extracted from OCT images. All imaging features are presented and discussed in terms of their importance and applicability in clinical practice. A total of 56 studies were included in this review. Overall, LC depth (LCD) and thickness (LCT) have been the most studied features, appearing in 75% and 45% of the included studies, respectively. These biomarkers were followed by the prelaminar tissue thickness (21%), LC curvature index (5.4%), LC global shape index (3.6%), LC defects (3.6%), and LC strains/deformations (1.8%). Overall, the disease groups showed a thinner LC (smaller LCT) and a deeper ONH cup (larger LCD), with some exceptions. A large variability between approaches used to compute LC biomarkers has been observed, highlighting the importance of having automated and standardized methodologies in LC analysis. Moreover, further studies are needed to identify the pathologies where LC features have a diagnostic and/or prognostic value.
