This clinical report describes the treatment of a patient with a large defect area in the maxillary central incisor, left lateral incisor, canine, and first premolar area after a partial maxillectomy. Computer-aided design and computer-aided manufacturing (CAD-CAM) techniques including rapid prototyping and milling were applied together to fabricate a removable partial denture.
Although studies have reported on selective laser melting (SLM)-fabricated removable partial dentures (RPDs), research addressing the trueness of SLM-fabricated RPD metal frameworks is sparse.

The purpose of this invitro study was to evaluate the trueness of powdered cobalt-chromium (Co-Cr) or titanium-6 aluminum-4 vanadium (Ti-6Al-4V) alloy frameworks for RPDs fabricated by SLM.

A digital scan of a Kennedy class II mandible typodont was obtained to design an RPD framework by using a computer-aided design (CAD) software program (denoted as CRF). Two experimental groups of frameworks were fabricated from the CRF by using SLM in alloys of Co-Cr (SLM-Co-Cr, n=6) and in Ti-6Al-4V (SLM-Ti-6Al-4V, n=6) while a control group was fabricated by using traditional lost-wax casting following stereolithography (CA-Co-Cr, n=6). In total, 18 RPD frameworks were digitally scanned (denoted as CRF), with each scan then superimposed on the CRF and evaluated for discrepancies by using a 3D analysis software program. A nonparametric Kruskal-Wallis test was performed to determine differences in trueness among groups (α=.05).

The CA-Co-Cr group showed the highest discrepancy between CEF and CRF. Statistically significant differences were found between the CA-Co-Cr and SLM-fabricated groups (P=.03 for Co-Cr, and P=.016 for Ti-6Al-4V). However, no significant difference was found between the SLM-Co-Cr and SLM-Ti-6Al-4V groups (P=.787).

SLM-fabricated RPD frameworks exhibited higher trueness than CA-Co-Cr fabricated ones, indicating the potential of selective laser melting to produce the geometric shapes required for accurate dental restorations.

Partial upper sternotomy (mini-ER) can be used in some adult cardiac surgeries but is seldom performed in the treatment of acute type A aortic dissection (AAAD). This study aimed to assess the feasibility and short-term outcomes of complete stent-graft replacement of the arch with triple-branched stent graft for AAAD through a mini-ER.

From 2015 to 2018, 254 patients with AAAD underwent complete stent-graft replacement of the arch with a triple-branched stent graft. Replacement was performed with conventional full sternotomy (con-ER) in 142 patients and with mini-ER in the other 112 patients. Using propensity score matching, the clinical data were compared between 100 patients in the mini-ER group and 100 patients in the con-ER group.

After propensity score matching, there were no significant between-group differences in aortic cross-clamp time, cardiopulmonary bypass time, or total operative time. The amount of mediastinal drainage and number of red blood cell units were significantly lower in the mini-ER group compared with the con-ER group (P<.001). The intubation time was significantly shorter in the mini-ER group (P<.001). Selleckchem BLU-554 The treatment costs were also lower in the mini-ER group (P<.001). There were no significant between-group differences in 30-day mortality (9% vs 8%; P>.99) or postoperative complications.

This study shows that extensive repair of AAAD through a mini-ER is feasible. It was superior to con-ER in terms of blood loss, postoperative ventilation time, and treatment costs.

This study was conducted to determine the influence of early septal reduction therapy (SRT) after referral on survival in patients with obstructive hypertrophic cardiomyopathy.

We reviewed the patients with obstructive hypertrophic cardiomyopathy (resting pressure gradient ≥30mm Hg or provoked pressure gradient ≥50mm Hg) who were evaluated at our clinic from 2000 to 2012. Early SRT was defined as undergoing septal myectomy or alcohol septal ablation during the 6months after index evaluation. Survival after the 6-month landmark period was analyzed in a multivariable Cox model.

A total of 1351 patients were included in the landmark analysis. Patients who were more symptomatic and had received more medical treatment at index evaluation were more likely to undergo early SRT. Over a median follow-up period of 10.2years, the survival was comparable (P=.207) but patients undergoing early SRT had, on average, improved survival compared with the medical treatment group (hazard ratio, 0.66; 95% confidence intervaWheelset bearing is one of the crucial rotating elements in the train bogie. Detection of wheelset bearing defect comes with many challenges due to complex wheel/rail excitation and the horrible working condition. The parametric dictionary sparse representation provides a practical path to detect the weak fault of wheelset bearing. However, the parametric dictionary obtained by the classical correlation filtering algorithm (CFA) is hard to match the analyzed signal's underlying fault impact characteristic. A novel parametric dictionary design algorithm named fault impact matching algorithm (FIMA) combining Orthogonal matching pursuit (OMP) is proposed to address the problem in this paper. The core of the FIMA mainly comprises two stages partial segmentation and global analysis. Two indexes, correlation function (CF) and kurtosis, are used to comprehensively evaluate the partial and global matching degree between the Laplace wavelet and the signal. The proposed method's effectiveness is verified by the fault simulation analysis and the practical wheelset bearing fault signals (outer and inner race fault). Some comparison studies demonstrate that the proposed method outperforms the CFA-OMP, the K-SVD-OMP and some time-frequency decomposition methods, such as EWT and VMD, in detecting the bearing weak defects.
