Chronic obstructive pulmonary disease (COPD) has high morbidity and mortality rates. COPD impairs body functioning, reduces quality of life, and creates a great economic burden for society. Pulmonary rehabilitation (PR) has become an important nonpharmacological treatment for COPD. This paper systematically reviews economic evaluations of PR in COPD patients in different settings.

We aimed to understand the cost-effectiveness of PR in different settings for COPD to provide economic evidence for decision-makers.

We searched eight databases from their inception to 23 November 2019. We used the Consensus on Health Economic Criteria to assess study quality.

This review included ten studies that matched the selection criteria. Five studies compared PR with usual care in primary healthcare or outpatient departments. Two studies compared community-based PR with hospital PR or usual care. In the other studies, PR was mainly carried out at home. Compared with usual care, PR was cost-effective in primary healthcare institutions or outpatient departments. According to CEACs, community-based PR had a 50% probability of cost-effectiveness at £30,000/quality-adjusted life year (QALY) compared with hospital PR in the UK. Based on the ICER, community-based PR was "moderately" cost-effective, with a ratio of €32,425/QALY compared with usual care in the Netherlands. Home-based PR was dominant compared with usual care, and tele-rehabilitation was dominant compared with traditional home PR.

This challenge is exemplified in Myanmar, where an active shark fishery exists despite a nationwide ban on targeted shark fishing. We used the Kipling method (5W1H) to gather a complete story of non-compliance in five small-scale fishing communities in the Myeik Archipelago. Among 144 fishers surveyed, 49% were aware of the nationwide ban. Shark fishers (24%) tended to be younger individuals who did not own a boat and perceived shark fishing to be prevalent. Compliant fishers were motivated by a fear of sharks and lack of capacity (equipment, knowledge), whereas food and income were cited as key motivations for non-compliance. The results of our study emphasize that in resource-dependent communities, improving compliance for effective shark conservation may require addressing broader issues of poverty, food security and the lack of alternatives.Necrotizing soft tissue infections (NSTI) are multifactorial and characterized by dysfunctional, time dependent, highly varying hyper- to hypo-inflammatory host responses contributing to disease severity. In order to achieve clinically relevant results and to guide preclinical and clinical research the vast amount of fragmented clinical and experimental datasets, which often include omics data at different levels (transcriptomics, proteomics, metabolomics, etc.), need to be organized, harmonized, integrated, and analyzed taking into account the Big Data nature of these datasets. In this chapter, we address these matters from a systems perspective and yet personalized approach. The chapter provides an overview on the increasingly more frequent use of Big Data and Artificial Intelligence (AI) to aggregate and generate knowledge from burgeoning clinical and biochemical information, addresses the challenges to manage this information, and summarizes current efforts to develop robust computer-aided clinical decision support systems so to tackle the serious challenges in NSTI diagnosis, stratification, and optimized tailored therapy.In necrotizing soft tissue infection (NSTI) there is a need to identify biomarker sets that can be used for diagnosis and disease management. We introduce the fundamentals of top-down systems biology approaches including analysis tools and we review the use of current methods and systems biology approaches to biomarker discover. In this chapter, we discuss the advanced recombinant inbred (ARI) BXD mouse model that mimics the genetic diversity as seen in humans and underpins the feasibility to map multiple genes (genetic loci) modulating GAS NSTI. GAS produces a myriad of virulence factors, including superantigens (SAg). Superantigens are potent immune toxins that activate T cells by cross-linking T cell receptors with human leukocyte antigen class-II (HLA-II) molecules expressed on antigen-presenting cells.
