Preoperatively, the breast implants were located in a subpectoral pocket in 78.7% of the patients and, postoperatively, in a subglandular pocket in 74.8% of the patients. Within the first 2 years, 13 patients (8.7%) underwent reoperation for additional fat grafting. Among 45 preoperative breast capsular contractures, there were eight recurrences in the first 3 years and four reoperations. CONCLUSIONS Secondary breast augmentation cannot rely solely on implant exchange. Because the soft-tissue envelope also ages over time, fat grafting is mandatory in the vast majority of secondary cases. Reprints and permission [email protected] We investigated the effects of percutaneous transluminal renal angioplasty on left ventricular (LV) mass, and the impact of LV mass reduction on outcomes. METHODS A total of 144 hypertensive patients with renal artery stenosis (RAS) (mean age 69 years; 22.2% fibromuscular dysplasia [FMD]) who underwent angioplasty were included. Echocardiography was performed at baseline and after one year, and patients were thereafter followed up for a median of 5.6 years for primary composite outcomes. RESULTS In both the FMD and atherosclerotic stenosis (ARAS) groups, LV mass decreased after angioplasty, but the decrease in LV mass index (-15.4±18.3% versus -0.8±27.8%, p less then 0.01) as well as the regression rate of LV hypertrophy was greater in FMD. Multiple logistic regression analysis indicated that FMD (odds ratio [OR] 2.94, p less then 0.01), severe RAS (≥90%) (OR 2.94, p less then 0.05), and higher LV mass index at baseline (OR 2.94 for 1 standard deviation increase, p less then 0.001) were independent predictors of LV mass index decrease of at least 20%. The primary composite outcomes occurred in 45 patients (31.3%). In FMD, lower LV mass index after one year (hazard ratio 2.81 for 1 standard deviation increase, p less then 0.05) or regression of LV mass (hazard ratio 0.75 for 5% decrease, p=0.054) showed a tendency to be associated with better outcomes; however, these associations were not found in ARAS. CONCLUSION Hypertensive patients with ARAS have less regression of LV mass in response to angioplasty than those with FMD, and LV mass regression is less useful as a surrogate marker of outcomes especially in ARAS. © American Journal of Hypertension, Ltd 2020. We describe preclinical to clinical translation of TD-1473-an oral gut-selective pan-JAK inhibitor-from in vitro characterization through a Phase 1b study in patients with UC. METHODS TD-1473 JAK inhibition potency was evaluated in vitro; plasma pharmacokinetics, safety, and efficacy were assessed in mice. In the first-time-in-human study, plasma pharmacokinetics and safety were assessed after single and multiple (14 days) ascending doses administered orally to healthy subjects. The Phase 1b study randomized patients with moderately to severely active UC to receive once-daily oral TD-1473 20, 80, or 270 mg, or placebo for 28 days. Plasma and colonic tissue concentrations were measured, safety was assessed, and efficacy was evaluated by UC clinical parameters, disease-surrogate biomarkers, endoscopy, histology, and colonic tissue JAK signaling. RESULTS TD-1473 exhibited potent pan-JAK inhibitory activity in vitro. Oral TD-1473 administration to mice achieved high, biologically active colonic tissue concentrations with low plasma exposure, and decreased oxazolone-induced colitis activity without reducing blood cell counts vs placebo. TD-1473 administration in healthy human subjects and patients with UC yielded low plasma exposure and was generally well tolerated; treatment in patients with UC resulted in biologically active colonic tissue concentrations and descriptive trends toward reduced clinical, endoscopic and histologic disease activity vs placebo. CONCLUSION Gut-selective pan-JAK inhibition with TD-1473 administration resulted in high intestinal vs plasma drug exposure, local target engagement, and trends toward reduced UC disease activity. For the classification of DLBCL subtypes, as well as for providing a prognosis of the clinical outcome, the analysis of the tumor microenvironment and, particularly, of the different types and functions of tumor-associated macrophages is indispensable. Until now, however, most information about macrophages has been obtained either in a completely indirect way by gene expression profiling or by manual counts in immunohistochemically (IHC) fluorescence-stained tissue samples while automated recognition of single IHC stained macrophages remains a difficult task. In an accompanying publication, a reliable approach to this problem has been established, and a large set of related images has been generated and analyzed. RESULTS Provided image data comprise (i) fluorescence microscopy images of 44 multiple immunohistostained DLBCL tumor subregions, captured at 4 channels corresponding to CD14, CD163, Pax5, and DAPI; (ii) "cartoon-like" total variation-filtered versions of these images, generated by Rudin-Osher-Fatemi denoising; (iii) an automatically generated mask of the evaluation subregion, based on information from the DAPI channel; and (iv) automatically generated segmentation masks for macrophages (using information from CD14 and CD163 channels), B-cells (using information from Pax5 channel), and all cell nuclei (using information from DAPI channel). CONCLUSIONS A large set of IHC stained DLBCL specimens is provided together with segmentation masks for different cell populations generated by a reference method for automated image analysis, thus featuring considerable reuse potential. © The Author(s) 2020.
