To assess the efficacy and safety of holmium laser en-bloc resection (HolERBT) versus conventional transurethral resection of bladder tumor (cTURBT).

Between September 2015 and September 2018, 100 patients with non-muscle-invasive bladder cancer were randomly allocated to cTURBT or HolERBT.

The primary endpoint was detection of residual tumor in reTURBT specimens at 4 wk after the primary resection. Operative parameters, specimen quality, perioperative complications, and recurrence-free survival (RFS) were compared. Independent sample t tests, χ
tests, and Kaplan-Meier curves were used, as appropriate.

The patient and tumor baseline characteristics were comparable between the groups. Residual tumors were detected in 7% and 27.7% of cases after HolERBT and cTURBT, respectively (p=0.01). This in vitro study aimed to evaluate the influence of the radiographic image file format and the transmission application (app) on the diagnosis of proximal caries lesions.

Twenty bitewing radiographs of 40 posterior human teeth placed in phantoms were acquired using the Digora Toto digital sensor. All images were exported as TIFF (Tagged Image File Format), BMP (Windows Bitmap), PNG (Portable Network Graphics), and JPEG (Joint Photographic Experts Group) and transmitted online via WhatsApp and Messenger. Five examiners evaluated the radiographs with no online transmission and as transmitted through the 2 apps for the presence of proximal caries lesions using a 5-point scale. The reference standard for caries lesions was established using micro-computed tomography. Two-way analysis of variance compared values of sensitivity, specificity, accuracy, and area under the receiver operating characteristic curve (α=.05). The kappa test was used to assess intra- and interexaminer agreement.

Sensitivity, specificity, accuracy, and area under the receiver operating characteristic curve values showed no significant differences in the diagnosis of proximal caries lesions between the different image file formats (P ≥ .773) and transmission apps (P ≥ .608). Intraexaminer agreement was substantial (κ=0.742) and interexaminer agreement was moderate (κ=0.475).

In this quality improvement project, we analyzed surgeon focus and attention utilizing a visual tracking device. We hypothesized that surgeons maintained better focus working 12-hour shifts compared to 24-hour calls.

A prospective, quality improvement project was performed on surgery residents, medical students, and attending physicians working at a busy, tertiary referral, safety-net hospital with Level 1 trauma and burn centers. A visual tracking system was used to measure visual attention before and after a 12-hour, in-house shift and a 24-hour, surgical in-house call. A pupil tracker was utilized to measure attention impairment. Individual performance was rated on a scale of 0 to 6 Severely Impaired- 0; Impaired- 1; Low Average- 2; Average- 3; Above Average- 4; High Average- 5; Superior- 6. Data were analyzed for homogeneity. The Wilcoxon Ranked Sum was used to assess for statistically significant differences between focus scores in the same group of individuals before and after shifts and/or calls. Twelve-hour shifts appear to preserve focus in surgeons better compared with 24- hour, in-house calls. More research is needed to establish the optimal time of surgeon focus maintenance and shift duration.
