To assess longitudinal microvascular changes in eyes treated with I-125 episcleral plaque brachytherapy (EPB).
High resolution OCT angiograms of the central 3×3mm macula were obtained from I-125 episcleral plaque brachytherapy treated and untreated fellow eyes of 61 patients. Capillary density (vessel skeleton density, VSD) and caliber (vessel diameter index, VDI) were quantified using previously validated semi-automated algorithms. Nonperfusion was also quantified as flow impairment regions (FIR). Exams from treated and fellow eyes obtained pre-treatment and at 6-month, 1-year, and 2-year intervals were compared using generalized estimating equation linear models. Dosimetry maps were used to evaluate spatial correlation between radiation dose and microvascular metrics.
At 6 months, treated eyes had significantly lower VSD (0.145 ± 0.003 vs 0.155 ± 0.002;
= 0.009) and higher FIR (2.01 ± 0.199 vs 1.46 ± 0.104;
= 0.010) compared to fellow eyes. There was a significant decrease in VSD and a corresponding increase in FIR even for treated eyes without clinically identifiable retinopathy at 6 months. VDI was significantly higher in treated eyes than in fellow eyes at 2 years (2.92 ± 0.025 vs 2.84 ± 0.018;
< 0.001). When our cohort was categorized into low dose radiation (<15Gy) and high dose radiation (>45Gy) to the fovea, there were significant differences in VSD and FIR between groups.

However, there have been no reports of artery fenestration causing renal aneurysm. We present the case of a 58-year-old man who developed renin-dependent hypertension. He was aware of heaviness of the head, and his blood pressure was 196/134 mm Hg on 5 mg of amlodipine. Laboratory tests showed hypokalemia, hyperreninemia, and hyperaldosteronemia. An enhanced 3-dimensional computed tomography scan showed a 19-mm renal aneurysm in a branch of the left renal artery, and renal arteriography showed a fenestration in the aneurysm-forming branch. Coil embolization was performed on the central side of the artery forming the aneurysm and fenestration, after which blood pressure, serum potassium, and plasma renin levels improved. The patient in the present case had renin-dependent hypertension as a result of decreased renal blood flow caused by the renal aneurysm and fenestration, which is considered an extremely rare etiology of hypertension.
Hypoparathyroidism is a rare disease and, as such, its natural history, long-term complications, and correct clinical management remain unclear.
To describe the natural history and clinical characteristics of the disease.
To present a retrospective observational analysis from 7 specialized centers in Buenos Aires, Argentina.
Chronic hypoparathyroid patients followed-up between 1985 and December 2018.
Data on demographics, etiology, clinical complications, biochemical parameters, dual-energy x-ray absorptiometry (DXA) values, and treatment doses were collected.
A total of 322 subjects with chronic hypoparathyroidism were included; 85.7% were female, the mean age was 55.2 ± 16.8 years, and the mean age at diagnosis was 43.8 ± 16.8 years. Prevalence of surgical hypoparathyroidism was 90.7%, with the most common causes being thyroid carcinoma and benign thyroid disease. A history of hypocalcemia requiring hospitalization was present in 25.7% of the whole group and in 4.3% of patients who had a historydisease. Almost 70% of the study's group of patients met the experts' indications for considering the use of rhPTH 1-84. Being aware of this fact is the 1st step in improving our medical management of this disease in the future.Patterns of brain structural connectivity (SC) and functional connectivity (FC) are known to be related. In SC-FC comparisons, FC has classically been evaluated from correlations between functional time series, and more recently from partial correlations or their unnormalized version encoded in the precision matrix. The latter FC metrics yield more meaningful comparisons to SC because they capture 'direct' statistical dependencies, that is, discarding the effects of mediators, but their use has been limited because of estimation issues. With the rise of high-quality and large neuroimaging datasets, we revisit the relevance of different FC metrics in the context of SC-FC comparisons. Using data from 100 unrelated Human Connectome Project subjects, we first explore the amount of functional data required to reliably estimate various FC metrics. We find that precision-based FC yields a better match to SC than correlation-based FC when using 5 minutes of functional data or more. Finally, using a linear model linking SC and FC, we show that the SC-FC match can be used to further interrogate various aspects of brain structure and function such as the timescales of functional dynamics in different resting-state networks or the intensity of anatomical self-connections.
