Compared with non-binge drinkers pre- and post-Sandy (66.9%), the adjusted odds ratios (aOR) for being new binge drinkers and consistent binge drinkers among high Sandy exposure enrollees were 2.1 (95%CI 1.1-4.1) and 2.5 (95%CI 1.7-3.6), respectively. High Sandy traumatic experience alone was associated with consistent binge drinking (aOR 1.9, 95%CI 1.4-2.6). Among enrollees without 9/11 PTSD, those with Sandy PTSD were more likely to become new binge drinkers (aOR 4.4, 95%CI 1.4-13.9), while Sandy PTSD was not associated with any binge drinking behavior changes among those with 9/11 PTSD. Sandy exposure, Sandy traumatic experience, and Sandy PTSD were all associated with higher binge drinking intensity. Future natural disaster response should plan for treatment to address alcohol use and PTSD simultaneously.
Despite considerable achievements associated with the MDGs, under-five mortality, particularly in Sub-Saharan Africa, remains alarmingly high. Globally, intimate partner violence (IPV) affects one in three women within their lifetime. Little is known about the relationship between IPV and maternal care-seeking in the context of high rates of under-five mortality, particularly among young women and adolescent girls in low- and middle-income countries (LMICs).
Data from the Kenya Demographic Health Survey (2008-2009) were limited to a sample of women aged 15-24 years (n=1,406) with a child under-five who had experienced IPV in the last 12 months. Using multivariate logistic regression, we constructed three models 1) base model; 2) controlling for type of residence (urban/rural); and 3) controlling for wealth status and education attainment, to estimate odds ratios (ORs) for the association between IPV and ten maternal care-seeking behaviors.

We describe a 41-year-old man who visited the emergency room with hemoptysis and dyspnea. He had a 5-year history of hypertension, though he had not received any treatment. His blood pressure was 233/159 mmHg, his percutaneous oxygen saturation level was 88% on room air, and he had a serum creatinine level of 11.7 mg/dL. Laboratory data showed microangiopathic hemolytic anemia, thrombocytopenia, and severe kidney damage, suggesting thrombotic microangiopathy (TMA). Chest computed tomography and bronchoalveolar lavage revealed pulmonary alveolar hemorrhage. In addition to steroid treatment and plasma exchange, antihypertensive therapy was started immediately. On day 3, activity of a disintegrin and metalloproteinase with thrombospondin type 1 motif, member 13 (ADAMTS13) activity was not significantly reduced, and clinical markers for vasculitis and connective tissue disease were negative. Therefore, steroid administration and plasma exchange were discontinued. Although antihypertensive therapy centering on angiotensin II receptor blocker was effective for DAH and TMA, renal function did not recover, and maintenance hemodialysis was required. Renal pathological findings were consistent with malignant nephrosclerosis, and features suggestive of vasculitis were not found. The pathophysiology in this case was considered to be mainly hypertension and vascular endothelial injury with renin-angiotensin-aldosterone system (RAAS) activation. The use of RAAS inhibitor was effective in converging DAH and TMA, and it was expected to repair vascular endothelial damage associated with appropriate antihypertensive intervention. Magnetic refrigeration might answer some of the global challenges linked with the increasing demands for readily available cooling in almost every region of the world and the global-warming potential of conventional refrigerants. Important issues to be solved are, for example, the required mass and the ecological footprint of the rare-earth permanent magnets and the magnetocaloric material, which are key parts of the magnetic cooling device. The majority of existing demonstrators use Nd-Fe-B permanent magnets, which account for more than 50% of the ecological footprint, and Gd, which is a critical raw material. This work shows a solution to these problems by demonstrating the world's first magnetocaloric demonstrator that uses recycled Nd-Fe-B magnets as the magnetic field source, and, as a Gd replacement material, La-Fe-Mn-Si for the magnetocaloric heat exchanger. These solutions show that it is possible to reduce the ecological footprint of magnetic cooling devices and provides magnetic cooling as a green solid-state technology that has the potential to satisfy the rapidly growing global demands.
