All rights reserved.BACKGROUND AND OBJECTIVES The aim was to evaluate long-term prognostic factors in hepatocellular carcinoma (HCC) patients who survived over 10 years after hepatectomy and compare prognostic factors between patients with recurrence who died and survived 10 years after initial hepatectomy. METHODS We analyzed the HCC patients without recurrence over 10 years after hepatectomy (n = 35), those with recurrence who survived over 10 years (n = 48), and those who died within 10 years (n = 132). RESULTS The rate of recurrence was 16.3%, 10-year overall survival rate was 38.6%, and the 10-year recurrence-free survival (RFS) rate was 16.7%. Nonviral, solitary tumor, well differentiation, and without severe fibrosis were independent favorable factors for long-term RFS. High cholinesterase levels, small tumors and without portal vein invasion were independent favorable factors for long-term survival among patients with recurrence. Long-term survivors with recurrence showed significantly low early recurrence, extrahepatic recurrence, multiple intrahepatic recurrences. CONCLUSION Important factors for long-term prognoses in HCC patients were a solitary tumor, small tumors, and no advanced fibrosis. A treatment for nonviral hepatitis is needed to achieve long-term RFS. Even patients who relapse might survive long term if they have a late or solitary intrahepatic recurrence, nonsevere cirrhosis, and curative treatment at recurrence. © 2020 Wiley Periodicals, Inc.INTRODUCTION The effects of levator ani muscle (LAM) avulsion after instrumental delivery on the sexual function of patients are currently unknown. Therefore, the objective of our study was to use a validated questionnaire, namely, the Female Sexual Function Index (FSFI), to compare the sexual function in patients with and without LAM avulsion after instrumental vaginal delivery. MATERIAL AND METHODS This was a prospective observational study of 112 primiparous women after instrumental (vacuum or forceps) vaginal delivery. The obstetric and general characteristics of the population were studied. At 6 months postpartum, the contraceptive method used and the occurrence of LAM avulsion (using 4D transperineal ultrasound) were determined, and the FSFI was administered. RESULTS A total of 100 patients (62 without avulsion and 38 with avulsion) completed the study. Thirty-eight (38%) were diagnosed with avulsion (42.1% after Kielland forceps delivery, 57.9% after Malmström vacuum delivery; p=0.837). Women with LAM avulsion had significantly lower scores for desire (2.9±1.2 vs 3.4±1.1; p=0.049), arousal (2.8±1.7 vs 3.6±1.4; p=0.014), lubrication (2.3±1.4 vs 3.0±1.2; p=0.011), orgasm (2.6±1.6 vs 3.3±1.2; p=0.006) and satisfaction (3.1±1.8 vs 3.9±1.5; p=0.051) than did women without LAM avulsion. The overall FSFI score was lower in patients with avulsion (16.7±8.9 vs 20.7±6.9, p=0.033). These results were obtained after controlling for confounders (delivery mode, induced labor, birth weight, perineal tears, avulsion degree, contraceptive method and group assignment for the parent study) in the multivariate analysis (F=4.974, p=0.001). CONCLUSIONS Patients with LAM avulsion present a higher degree of sexual dysfunction than patients without avulsion at 6 months after instrumental vaginal delivery. Proteins and peptides in egg white have been found to alleviate inflammation, but the role played by the intestinal flora is still unclear. Using casein as a dietary protein control, we investigated the effects of diets composed of hen egg white, duck egg white, and preserved egg white on gut microbiota in the rat caecum. RESULTS The gut microbiota in rats was alterd post egg white consumption. The results showed that rats fed with egg white had a similar overall structure of caecal bacterial communities separated from those fed with casein. The relative abundance of Akkermansia in the group of rats fed with hen egg white was highest among all groups. Rats fed with duck egg white had significantly higher relative abundance of Proteobacteria and Peptostreptococcaceae, and lower relative abundance of Lachnospiraceae. In addition, the levels of lipopolysaccharide (LPS) and lipopolysaccharide-binding protein (LBP) in rats fed with duck egg white and preserved egg white were lower than that in rats fed with hen egg white. CONCLUSION Our results indicated the possible positive effect on homeostasis of the intestinal flora brought about by the intake of duck egg white. This study provides an insight into the potential positive impact of preserved egg white on the health by changing gut microbiota and affecting the antigen load. In this study, soybean soluble polysaccharide (SSPS) and high methoxylated pectin (HMP, a traditional PS) are used, at different ratios to the protein, to improve the colloidal stability of the acidified whey protein solution. Both heated and unheated complexes were studied. SSPS-whey protein complexes have shown exceptional stabilities in all ratios while HMP-whey protein complexes revealed coacervation after 72 hr of storage. read more The prepared complexes exhibited comparable sizes and ζ-potentials. The SSPS-whey protein complexes were less turbid than HMP-whey protein complexes at similar PS to protein ratios. Results also show that greater repulsive interactions occurred in SSPS-whey protein complexes when compared to HMP-whey protein complexes, as examined by free thiol content and intrinsic fluorescence intensity measurements.
