National Health Service (NHS) Emergency Department (ED) attendances are at the second highest level ever recorded, (RCEM, 2021a) and as they soar, performance plummets, putting patient safety at risk (RCEM, 2021b). Managing patient flow in the ED is critical to reduce patient safety incidents and crowding, however, this needs effective leadership (Jensen and Crane, 2014). This paper aims to introduce an innovative form of managing patient flow in ED, which is a two hourly "Board Rounds", providing a managed process to pull patients through the system meeting pre-determined time critical standards and preventing patient harm. Board Rounds combined with effective leadership can play a contributory role preventing crowding in the ED.
An evaluation of two hourly ED Board Rounds was undertaken using the hospitals' ED Board Round Standard Operating Procedure to develop a series of short questions. As leadership is the responsibility of all clinicians (Darzi, 2008; Moscrop, 2012), a separate survey was undertakealuate perceptions of Board Rounds in the ED and to engage clinicians and managers in a self-assessment of their own leadership styles to reflect on optimum leadership styles for use in ED.

Paediatric patients account for less than 5% of total COVID-19 patients worldwide, and hence studies involving such subjects are unlikely in the near future. As a result, there are inadequate data about COVID-19 infection in children. Therefore, we present this systematic review to understand the epidemiology, clinical features, diagnostic outcomes, and available options for the management of COVID-19 in paediatric populations in order to advise clinical and public health policy choices for safety of children worldwide.The goal of this study is to assess the use of a score composed of markers of inflammation and organ failure to predict positive cultures for preterm newborns with clinical suspicion of late-onset sepsis. The score was calculated at the first suspicion and 24-48 hours later. We retrospectively compared score results between neonates with positive and negative cultures. Neonates with positive cultures had a significantly higher score at the second instance; the receiver operator characteristics curve presented an area under the curve of 0.798 (p=0.007). A score for early prediction of sepsis could be an important tool for prognostic improvement in the future.
Healthcare expenditures of older United States (US) adults with different pain severity levels are important to investigate given the increasing prevalence of pain in this population. link2 This study assessed the correlation of healthcare expenditures among older US adults with different pain severities, hypothesizing that expenditures would increase as pain severity increased.
This retrospective cross-sectional database study used 2018 Medical Expenditure Panel Survey (MEPS) data and included US adults aged≥50 with self-reported pain in the past four weeks and positive healthcare costs. Adjusted linear regression models with logarithmically transformed expenditure data compared differences in total; office-based; outpatient; emergency room; inpatient; other; and prescription medication expenditures, between those with little, moderate, quite a bit, and extreme pain. Analyses were weighted to account for the complex MEPS design and to obtain nationally representative estimates. The
alpha level was 0.05.

This study found total healthcare costs increased as pain severity increased, yet this was not always the case when analyzing subcategories of healthcare costs. Further research is needed to investigate why some types of healthcare costs are greater with increasing pain severity, yet others are not.Previous research has acknowledged the effect of prosody on inter-gestural coordination, but specifically the effect of tones is still understudied. This paper has a two-fold purpose. First, it aims to explore effects of the Swedish word accents on word-initial consonant-vowel (CV) coarticulation. Second, it aims to revisit the existing evidence for tonal integration. Based on Articulatory Phonology, it has been suggested that tones - in tone languages - are integrated in the gestural organization of a syllable-initial CV sequence in the same manner as would an additional consonant (CCV), indicated by a time lag between the gestural onsets of the C and the V gesture (CV onset time lag). However, we argue that the existing evidence is inconclusive, because previous cross-linguistic research has used small-scale data sets (one to seven speakers), and we still lack a well-grounded consensus on how gestural onsets are to be measured. This study uses Electromagnetic Articulography (EMA) to investigate word-initialrch needs to compare data from a variety of languages using a considerable number of speakers. The present study provides new reference values for such comparisons.
Given the cardiac pathological remodeling following to anabolic androgenic steroids (AASs) consumption, we examined the effect of chronic administration of nandrolone decanoate with high-intensity endurance exercise on the left ventricular hypertrophy index, levels of hydroxyproline, tumor necrosis factor-alpha (TNF-α), adiponectin (APN) and its receptors (AdipoR1 and AdipoR2) expression in rats' hearts.
The male Wistar rats randomly divided to six groups included the control (CTL), exercise (Ex), nandrolone (Nan), vehicle (Arach), trained vehicle (Ex+Arach), and trained nandrolone (Ex+Nan) groups that were treated for eight weeks.
Nandrolone consumption significantly enhanced the hypertrophy index (p<0.05) and exercise intensified this effect. It also increased the level of cardiac hydroxyproline (p<0.001), however exercise completely masked this effect. The values of TNF-α protein and AdipoR1 protein significantly increased in trained nandrolone-treated (Ex+Nan) group in comparison with CTL grouinst nandrolone induced heart remodeling may apply through modulation of APN system.COVID-19 infection started in China in December 2019 and was declared a global pandemic of international concern by the WHO in March 2020. The present review briefs concerning the impact of COVID-19 and HIV/AIDS pandemics on children and pregnant women worldwide.
The representation of a child is an important element of mother-child relations, which allows the mother to empathise with and respond to the child's needs. A child's cancer may be reflected in the mother's representation of the child. The aim of this study was to see whether there were differences in a mother's representation of healthy and oncologically ill children.
The participants in the study include 30 mothers of oncologically ill children and 25 women with healthy children as the control group. The study used a self-constructed questionnaire containing questions about the mothers' representations of their children as well as demographic information.
Women with oncologically ill children less frequently described their children as independent, impulsive, needing food and the recognition of others, than mothers of healthy children. They also felt fear more frequently when thinking about the child. There were, however, common elements in representations of ill and healthy children. Impulsiveness and interest in computer games and movies was more often attributed to boys than girls in both categories, while helpfulness was attributed more often to older children than younger ones. Single mothers felt regret more often when thinking about the child than mothers who had some helpers.

Thrombosis with thrombocytopenia has been noted as a rare side effect of COVID 19 vaccine. Such side effect of COVID 19 vaccine in patients of hypercoagulable disorder may prove to be fatal.
