However, the long-term safety of the concurrent use of both agents has not been widely evaluated. We conducted a retrospective observational cohort study to evaluate the safety of SGLT2 inhibitors with loop diuretics versus SGLT2 inhibitors alone among diabetic patients. The primary endpoint was a composite of volume-depletion adverse events at 1 month and 12 months. Of the 400 patients included, 98 received SGLT2 inhibitors with a loop diuretic and 302 received SGLT2 inhibitors alone. The concurrent use of SGLT2 inhibitors and loop diuretics was tolerated at 1 month; however, it resulted in a significant increase in volume-depletion events at 12 months (10.2% vs. 1.7%; aHR= 7.03, 95% CI (1.80-27.37), p-value= 0.005). In conclusion, the long-term concurrent use of SGLT2 inhibitors and loop diuretics increases the risk of volume depletion, warranting frequent monitoring.Atrial fibrillation (AF) is the most prevalent arrhythmia in the United States. However, studies evaluating the impact of iron deficiency anemia on AF outcomes are limited. Therefore, we aimed to evaluate the association of iron deficiency anemia (IDA) on clinical outcomes in patients hospitalized with AF. A retrospective analysis of adult hospital discharges from the National Inpatient Sample (NIS) between 2004 and 2018 was conducted. Multivariable logistic regression was used to assess the association of IDA and other clinical outcomes i.e. inpatient mortality, acute myocardial infarction, cardiogenic shock, acute kidney injury, vasopressors use, length of stay, and other resource utilization. These models were adjusted for patient and hospital-level characteristics. A total of 5,975,241 weighted primary AF hospitalizations were identified. Out of these, 152,059 (2.5%) had diagnosis of IDA. After adjustment of variables, admissions with IDA were associated with higher rates of acute myocardial infarction (adjusted odds ratio [aOR] = 1.10, 95% CI 1.01-1.19 p=0.026), use of vasopressors (aOR= 1.30, CI 1.27-1.32, p less then 0.001), invasive mechanical ventilation (aOR= 1.26, CI 1.14-1.40 p less then 0.001) and acute kidney injury (aOR= 1.72, CI 1.66-1.79 p less then 0.001). There was no significant difference in all-cause mortality (aOR= 0.97, CI 0.87-1.07, p=0.513), cardiogenic shock, in-hospital cardiac arrest or use of mechanical circulatory support. Adjusted mortality in patients with AF and IDA decreased from 1.09% to 0.54% from 2004-2018 (p-trend less then 0.001). Among hospitalized patients with AF, our study did not show any difference in all-cause mortality between those with and without IDA.
With the growing utilization of transcatheter aortic valve replacement (TAVR) as an alternative option to surgical valve replacement (SAVR) in patients considered to be suboptimal for surgery, there is a need to explore the possibility of next day discharge (NDD) and its potential outcomes. The aim of our study is to compare outcomes and complications following NDD versus the standard early discharge (ED) (less than three days).

A comprehensive literature search was performed in PubMed, Embase, and Cochrane to identify relevant trials. Summary effects were calculated using a DerSimonian and Laird random effects model as odds ratio with 95% confidence intervals for all the clinical endpoints. Studies comparing same-day or next-day discharge vs. discharge within the next three days were included in our analysis.

Six studies with 2,672 patients were identified. The overall prevalence of FH is currently unknown and is usually underdiagnosed and undertreated worldwide. FH registry in different geographical area is a mission that helps early diagnosis of FH patients in the general population. PubMed, ISI Web of Science, Scopus, and Google Scholar were searched systematically for studies and reports on the FH registry using related keywords. Finally, 27 studies were included in this review. Most of the studies used the CASCADE screening method based on one or more than one of the three well-established FH criteria namely, the Dutch Lipid Clinic Network (DCLN), Simon Broome Register (SBR), or Make Early Diagnosis to Prevent Early Death (MEDPED) criteria. Except for a small number of studies that the genetic and molecular methods were used, in other studies only clinical diagnosis was applied. All these studies claimed that the FH registry causes the identification of many new cases as a result of used CASCADE screening and referral to lipid clinics. They concluded that the FH registry increases general and also physician awareness on FH prevalence and its related complications which in the long-term will improve FH management. The relative risk of 30-day readmission among those with cirrhosis but without renal disease (aOR, 1.3; 95% CI 1.3 to 1.3) was lower than those with both cirrhosis and renal disease (aOR, 1.8; 95% CI 1.6 to 2.0) when compared to persons without either comorbidities. Risk of 30-day rehospitalization was significantly higher among patients with heart failure and underlying cirrhosis. Adult male Wistar rats following a 5/6 Nx were treated with enalapril (NxE group, 10 mg/kg/day, n = 9) or vehicle (Nx group, n = 8) for two weeks. On the 15th day after Nx, rats were anaesthetized and submitted to mean arterial pressure (MAP) determination before and after receiving two intravenous injections of saline (vehicle, n = 9) or NPCdc (0.3 μg/kg dissolved in saline, n = 18) separated by a 20-min interval. The kidneys were submitted to oxidative stress analysis. The basal MAP of the NxE group was nearly 20% lower (P less then 0.05) than non-treated rats. NPCdc administration decreased the MAP in both groups; however, in the NxE group, the effects were observed only in the second injection.
