Timely recognition and diagnosis is imperative for successful treatment and outcome. The multi-organ involvement in the PPP syndrome requires close collaboration across different medical departments.Apprenticeships models are the cornerstone of learning in the medical profession for centuries now. Although we know that apprenticeships are effective in medical workplaces, its precise mechanisms remain tacit and implicit. However, decreasing exposure to relevant learning experiences forces us to develop an understanding of teaching and learning in apprenticeships. Central to the learning in apprenticeships are entrustment of autonomy and supply of expertise by the supervisors. Supervisors perform risk-assessments to estimate how much autonomy can be entrusted to their residents continuously. Such assessments are particular challenging to supervisors when the margins for error are narrow, for instance during complex tasks of surgical procedures. In this perspective we present our insights regarding the teaching and learning during surgical procedures. We demonstrate how supervisors and residents can apply insights in teaching and learning to optimize the learning process during complex tasks.The treatment of gout is subject to different national and international guidelines. Observational studies and limited trial data suggest that treatment with allopurinol may reduce the risk of cardiovascular events in patients with gout. However, at this moment allopurinol remains an unproven strategy. There is need for a large randomized placebo-controlled trial with sufficient power and duration of follow-up.
To describe trends and developments in the total duration of training medical specialists in the Netherlands, including their ages upon completion of the components of medical education undergraduate medical program, waiting time and further post-graduate specialist training.
Population cohort study.
From the combined data from Statistics Netherlands and the Registration Committee for Medical Specialists for the period 1986 to 2018 relevant populations were selected and training trajectories were mapped. The population size was 40,604 individuals for undergraduate medical programs, 41,885 for the duration of post-graduate specialist training, 31,915 for the waiting periods and 21666 for the total duration of the trajectory from the start of medical school until registration as a specialist.
The median duration of undergraduate medical programs was 7.1 years, which is longer than the nominal duration and this remained unchanged over the observation period. The average waiting time between graduating meage age at start of postgraduate specialist training has decreased, medical specialists are on average younger upon registration.Intermittent fasting (IF) is a broad concept and covers several fasting regimes. Studies of 'early time restricted feeding' and 'alternate day fasting' with energy restriction show a greater effect on weight and cardiometabolic health in overweight people in the short term, compared to a continuous caloric restriction (CCR). 'Late time restricted feeding' seems to have no or unfavorable effects. Long-term studies (up to 2 years) suggest that IF regimens are not superior to continuous caloric restriction. The few studies available show a similar compliance and metabolic adaptation between IF and CCR. There is insufficient knowledge about long-term safety in various groups of people, the influence of dietary quality and the practical feasibility of IF regimes. As a result, no recommendations can yet be made on the use of IF in the treatment of overweight and related diseases.Minimally invasive esophagectomy is increasingly becoming the surgical treatment of choice for esophageal cancer. The goal of this technique is to reduce the rate of respiratory complications associated with thoracotomy while taking advantage of the benefits of reduced mortality associated with minimally invasive techniques. However, minimally invasive esophagectomy is still not considered the gold standard for resectable esophageal cancer worldwide because it is a highly technical and complex procedure. The goal of this video tutorial is to present an easy step-by-step approach to a minimally invasive esophagectomy and to address technical considerations and potential pitfalls.Our group has previously described how dedicated practice outside the operating room can improve surgical technique and enhance intraoperative performance. We have also recently developed a "do-it-yourself" simulator to practice a variety of operative scenarios in cardiac surgery. This video tutorial demonstrates our Cannulation Module, which is designed to hone skills in aortic, right atrial, left ventricular vent, and bicaval cannulation for cardiopulmonary bypass.Recent advance in medical education is in correlation with the advances in information technology and thus computer-based learning is being increasingly employed. The objective of the present study was to design and evaluate an e-learning module in anatomy and assess the perceptions of students and faculty about this e-learning module. The participating students were randomized into three groups by block stratified randomization and Google groups were created for each of the three groups. The e-learning module was implemented in three sessions by rotating the three groups. Validated questionnaires were sent to faculty and participating students via Google forms to obtain feedback. The results of ANOVA showed that there was a significant difference among the groups in terms of marks obtained with conventional (F = 2.403, P = 0.103), online (F = 6.050, P = 0.005), and blended (F = 5.801, P = 0.006). Post hoc comparisons using the Tukey HSD test, about the gain of knowledge, indicated that the results were insignificant when comparing the conventional group with the online group, but were significant when comparing the blended group with the conventional and online group. The qualitative data regarding the perception of students toward e-learning were analyzed using thematic analysis. The introduction of an interactive e-learning module in anatomy was effective and well received by the students and faculty. The study showed that blended learning has a positive impact on the students' learning by improving cognitive gain and receptive perception for e-learning.
