Research and development of organisational interventions to prevent and minimise workplace aggression has also increased. However, it is not known if interventions prevent or reduce occupational violence directed towards healthcare workers. OBJECTIVES To assess the effectiveness of organisational interventions that aim to prevent and minimise workplace aggression directed towards healthcare workers by patients and patient advocates. Most institutions recommend the use of heparin to prevent occlusion; there is debate, however, regarding the need for heparin and evidence to suggest normal saline (0.9% sodium chloride) may be as effective. OBJECTIVES To assess the clinical effects (benefits and harms) of intermittent flushing of normal saline versus heparin to prevent occlusion in long-term central venous catheters in infants and children. SEARCH METHODS The Cochrane Vascular Information Specialist searched the Cochrane Vascular Specialised Register, CENTRAL, MEDLINE, Embase and CINAHL databases; World Health Organization International Clinical Trials Registry Platform and ClinicalTrials.gov trials register to 9 April 2019. We also unhe duration of catheter placement was reported to be similar for the two study arms in one study (203 participants; moderate-certainty evidence), and not reported in the remaining studies. AUTHORS' CONCLUSIONS The review found that there was not enough evidence to determine the effects of intermittent flushing with normal saline versus heparin to prevent occlusion in long-term central venous catheters in infants and children. It remains unclear whether heparin is necessary to prevent occlusion, CVC-associated blood stream infection or effects duration of catheter placement. Published by John Wiley & Sons, Ltd.BACKGROUND AND PURPOSE Angiotensin-converting enzyme inhibitors (ACEi) and angiotensin II receptor blockers (ARB) are first-line conventional drugs that are believed to reduce the progression of end-stage renal disease in diabetic patients. However, differences in the effects of ACEi and ARB are well not studied and, possible mechanisms are the subject of ongoing debate. EXPERIMENTAL APPROACH Male diabetic CD-1 mice were treated with ACEi, ARB, AcSDKP, ACEi+AcSDKP, ARB+AcSDKP, glycolysis inhibitors or non-treatment. Moreover, polyoligopeptidase inhibitor (POPi) injected male diabetic C57Bl6 mice were treated with ACEi, AcSDKP and, ARB or non-treatment. Western blot and immunofluorescent staining were used to examine key enzymes and regulators of central metabolism. KEY RESULTS The antifibrotic action of ACEi is due to an AcSDKP-mediated antifibrotic mechanism, which reprograms the central metabolism including restoring SIRT3 protein, and mitochondrial fatty acid oxidation whereas, suppression of abnormal glucose metabolism in the diabetic kidney. Moreover, POPi significantly blocked the AcSDKP synthesis, accelerated kidney fibrosis and, disrupted the central metabolism; ACEi party restored the kidney fibrosis and elevated the AcSDKP level whereas, ARB did not show in the POPi injected mice. ACEi and AcSDKP suppressed defective metabolism-linked mesenchymal transformations and reduced collagen-I and fibronectin accumulation in the diabetic kidneys. CONCLUSION AND IMPLICATIONS The present study envisages that AcSDKP is the endogenous antifibrotic molecule that controls the metabolic switch between glucose metabolism and fatty acid metabolism and that suppression of AcSDKP leads to disruption of kidney cell metabolism and activates mesenchymal transformations which ultimately leads to, severe fibrosis in the diabetic kidney.
