Like other infectious diseases, COVID-19 shows a clinical outcome enormously variable, ranging from asymptomatic to lethal. In Italy, like in other countries, old male individuals, with one or more comorbidity, are the most susceptible group, and show, consequently, the highest mortality, and morbidity, including lethal respiratory distress syndrome, as the most common complication. In addition, another extraordinary peculiarity, that is a surprising resistance to COVID-19, characterizes some Italian nonagenarians/centenarians. Despite having the typical COVID-19 signs and/or symptoms, such exceptional individuals show a surprising tendency to recover from illness and complications. On the other hand, long-lived people have an optimal performance of immune system related to an overexpression of anti-inflammatory variants in immune/inflammatory genes, as demonstrated by our and other groups. Consequently, we suggest long-lived people as an optimal model for detecting genetic profiles associated with the susceptibility and/or protection to COVID-19, to utilize as potential pharmacological targets for preventing or reducing viral infection in more vulnerable individuals.
Human neurodevelopment is complete by the 4th decade of life at which point brain atrophy ensues with variable rate and regionality into old age. Literally all regions of the brain experience atrophy with older age, however the pattern and rate of atrophy can dictate the behavioral consequences (i.e., cognitive impairment, Alzheimer's disease). Substantial research has aimed to discover the reasons why some people experience greater morphologic changes that produce undesirable consequences with aging and how it may be prevented. One possible explanation is diet, particularly fish consumption and the intake of omega-3 polyunsaturated fatty acids (omega 3) concentrated in fish oil. This narrative review examines the available evidence on the association between omega-3 and brain volume in non-demented older adults.

A PubMed search of the literature was conducted in search of studies that investigated the associations of omega-3 on brain morphology and volume in cognitively intact older adults. Inclusion cri. Current evidence is still insufficient to formulate recommendations for omega-3 intake to support brain health specifically.
Although intracranial aneurysms are increasingly treated endovascularly, microsurgical clipping has been the standard approach for middle cerebral artery (MCA) aneurysms. We compared microsurgical clipping and state-of-the-art endovascular treatment of unruptured MCA bifurcation aneurysms treated at a neurovascular center following a "coil-first" policy.

This single-center study included 148 patients treated for 160 unruptured MCA bifurcation aneurysms. Technical success, complications, clinical outcome, and angiographic results were retrospectively compared.

Microsurgical clipping was performed for 120 MCA aneurysms (75%) and endovascular treatment for 40 (25%; conventional coiling 8, stent-assisted coiling 16, balloon-assisted coiling 3, and flow-disruption 13). Technical treatment success was higher in the clipping group (100%) than in the endovascular group (92.5%, P= 0.015). Overall, complications occurred in 16.7% for clipping and in 20.0% for endovascular treatment (P= 0.631). Major ischemic stroipping proves to be the standard treatment option for MCA bifurcation aneurysms. However, endovascular treatment represents a safe and efficient alternative treatment option for patients.
Lumbar degenerative spondylolisthesis (LDS) is a common spinal disease. LDS has been differentiated into dynamic (unstable) and static (stable) spondylolisthesis. Standing flexion/extension lumbar spine radiographs are the best investigation to detect presence of dynamic spondylolisthesis. Magnetic resonance imaging is the investigation of choice to show lumbar canal stenosis and disc prolapse but it can miss dynamic LDS. Studies have shown good association between presence of facet fluid (FF) and dynamic spondylolisthesis.

A systematic review and meta-analysis were performed. All studies describing the relationship between FF and degenerative spondylolisthesis as measured on dynamic radiographs or kinematic magnetic resonance imaging were included.

Fourteen articles met the inclusion criteria. A total of 1065 patients were included in the meta-analysis. Of the patients with unstable spondylolisthesis, 71% had FF, whereas only 22% of the patients with stable spondylolisthesis had FF. The combined pooled odds ratio for unstable spondylolisthesis in the presence of FF was 7.55 (3.61-15.08; P <0.00001). The pooled standard mean difference in the FF size in the patients with unstable and stable spondylolisthesis was 0.97 mm (0.38-1.57; P= 0.001).

FF has positive correlation with the presence of dynamic LDS and the probability of dynamic LDS increases as the size of FF increases. The probability of having a dynamic spondylolisthesis in patients with FF >1 mm is 8 times that of patients with no FF. Standing flexion extension radiographs should be performed in patients with FF >1 mm.
