While high serum phosphorus (SPhos) levels have been related to adverse outcomes in hemodialysis patients, further investigation is warranted in persons receiving peritoneal dialysis (PD).
Longitudinal data (2014-2017) from the PDOPPS, a prospective cohort study, were used to examine associations of SPhos with all-cause mortality and major adverse cardiovascular events via Cox regression adjusted for confounders. SPhos levels were parameterized by 4 methods (1) baseline SPhos; (2) mean 6-month SPhos; (3) number of months with SPhos>4.5mg/dL; and (4) mean area-under-the-curve of 6-month SPhos control.
The study included 5847 PD patients from 7 countries; 9% of patients had baseline SPhos<3.5mg/dL, 24% had SPhos≥3.5 to≤4.5mg/dL, 30% had SPhos>4.5 to<5.5mg/dL, 20% had SPhos≥5.5 to<6.5mg/dL, and 17% had SPhos≥6.5mg/dL. Compared to patients with baseline SPhos≥3.5 to≤4.5mg/dL, the adjusted all-cause mortality hazard ratio (HR) was 1.19 (0.92,1.53) for patients with baseline SPhos≥5.5 to<6.5mg/dL and HR=1.53 (1.14,2.05) for SPhos≥6.5mg/dL. Associations between SPhos measurements over 6 months and clinical outcomes were even stronger than for a single measurement.
SPhos>5.5mg/dL was highly prevalent (37%) in PD patients, and higher SPhos levels were a strong predictor of morbidity and death, particularly when considering serial phosphorus measurements. This highlights the need for improved treatment strategies in this population. Serial SPhos measurements should be considered when assessing patients' risks of adverse outcomes.

The characteristics of cells, tissues and gastric microbiota before or after key therapeutic points are critical for monitoring malignant transformation and early tumour reversal. This review summarises the pathological features of gastric mucosa, characteristics of gastric microbiota, specific microbial markers, microbe-microbe interactions and microbe-host interactions at different stages in Correa's cascade. The markers related to each Correa's cascade point were analysed in detail. We attempted to identify key therapeutic points for early cancer reversal and provide a novel approach to reduce the incidence of GC and improve precise treatment.We describe presenting clinical and imaging manifestations of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2)-associated Rhino-oculo-cerebral mucormycosis (ROCM) in a hospital setting during the second wave of SARS-CoV-2 pandemic in India. Data on the presenting manifestations were collected from 1 March to 31 May 2021. Associations between clinical and imaging findings were explored, specifically (1) the presence or absence of orbital pain and infiltration of a superior orbital fissure on imaging; (2) the presence of unilateral facial nerve palsy and pterygopalatine fossa infiltration and geniculate ganglion signal on contrast magnetic resonance imaging, and (3) vision loss and optic nerve findings on imaging. Orbital pain was reported by 6/36 subjects. A fixed, frozen eye with proptosis and congestion was documented in 26 (72%), complete vision loss in 23 (64%), and a unilateral lower motor neuron facial nerve palsy in 18 (50%). No association was found between the presence of orbital pain and superior orbital fissure infiltration on imaging. The ipsilateral geniculate ganglion was found to enhance more profoundly in 7/11 subjects with facial palsy and available magnetic resonance (MR) imaging, and the ipsilateral pterygopalatine fossa was found infiltrated in 14. Among 23 subjects with complete loss of vision, 9 (39%) demonstrated long-segment bright signal in the posterior optic nerve on diffusion MR images. We conclude that orbital pain might be absent in SARS-CoV-2-associated ROCM. Facial nerve palsy is more common than previously appreciated and ischemic lesions of the posterior portion of the optic nerve underlie complete vision loss.
The present study examines the association between rurality and positive childhood experiences (PCEs) among children and adolescents across all 50 states and the District of Columbia. Recent work has quantified the prevalence of PCEs at the national level, but these studies have been based on public use data files, which lack rurality information for 19 states.
Data for this cross-sectional analysis were drawn from 2016 to 2018 National Survey of Children's Health (NSCH), using the full data set with restricted geographic data (n = 63,000). Descriptive statistics and bivariate analyses were used to calculate proportions and unadjusted associations. Multivariable regression models were used to examine the association between residence and the PCEs that were significant in the bivariate analyses.
Rural children were more likely than urban children to be reported as having PCEs volunteering in their community (aOR 1.29; 95% CI 1.18-1.42), having a guiding mentor (aOR 1.75; 95% CI 1.45-2.10), residing in a safe neighborhood (aOR 1.97; 95% CI 1.54-2.53), and residing in a supportive neighborhood (aOR 1.10; 95% CI 1.01-1.20) than urban children.

Given the higher baseline rate of PCEs in rural than urban children, programs to increase opportunities for PCEs in urban communities are warranted. Accurate assessment of the Mpro activity is crucial for the detection of SARS-CoV-2. Herein, we report a nanopore-based sensing strategy that uses an enzyme-catalyzed cleavage reaction of a peptide substrate to measure the Mpro activity. The peptide was specifically cleaved by the Mpro , thereby releasing the output products that, when translocated through aerolysin, quantitatively produced the signature current events. The proposed method exhibited high sensitivity, allowing the detection of Mpro concentrations as low as 1 nM without the use of any signal amplification techniques. This simple, convenient, and label-free nanopore assay may expand the diagnostic tools for viruses.Both the U.S. Clean Water Act (CWA) and the Journal of Environmental Quality (JEQ) trace back to 1972. The journal has a strong history of publishing science in support of the CWA, from basic science used to develop best management practices to address water quality problems to special sections on hot issues of the day and publishing long-term data that provide evidence of success. The objective of this article is to provide a brief overview of how JEQ has provided a publication outlet for researchers involved in efforts focused on key CWA issues and to introduce a special section celebrating the golden anniversary of this blue policy. The molecular chaperone Hsp90 provides a promising target because it supports survival, virulence, and drug resistance in a variety of pathogens. Herein, we report on the structural optimization and structure-activity relationship studies of 3,4-isoxazolediamide analogs. As a new class of fungal Hsp90 inhibitor, compound B25 was found to have good synergistic effects with fluconazole and to avoid potential mammalian toxicity. It also showed remarkable metabolic stability in vitro. Collectively, B25 could be a promising lead compound for drug discovery targeting fungal Hsp90 and deserves further investigation.Large-scale hypothesis testing has become a ubiquitous problem in high-dimensional statistical inference, with broad applications in various scientific disciplines. One relevant application is constituted by imaging mass spectrometry (IMS) association studies, where a large number of tests are performed simultaneously in order to identify molecular masses that are associated with a particular phenotype, for example, a cancer subtype. Mass spectra obtained from matrix-assisted laser desorption/ionization (MALDI) experiments are dependent, when considered as statistical quantities. False discovery proportion (FDP) estimation and control under arbitrary dependency structure among test statistics is an active topic in modern multiple testing research. In this context, we are concerned with the evaluation of associations between the binary outcome variable (describing the phenotype) and multiple predictors derived from MALDI measurements. We propose an inference procedure in which the correlation matrix of the test statistics is utilized. The approach is based on multiple marginal models. Specifically, we fit a marginal logistic regression model for each predictor individually. Asymptotic joint normality of the stacked vector of the marginal regression coefficients is established under standard regularity assumptions, and their (limiting) correlation matrix is estimated. The proposed method extracts common factors from the resulting empirical correlation matrix. Finally, we estimate the realized FDP of a thresholding procedure for the marginal p-values. We demonstrate a practical application of the proposed workflow to MALDI IMS data in an oncological context.Potassium-ion batteries (PIBs) have been regarded as a competitive alternative for lithium-ion batteries, owing to the natural abundance, low cost, and similar rocking-chair working mechanism of potassium element. However, it is challenging to simultaneously prepare suitable potassium ion anode materials of low voltage plateau, high capacity, and long cycle life. In this work, onion-like soft carbon (OLSC) of high heteroatom content is prepared by using solvent-sensitive self-assembly properties of asphaltene molecules. The OLSC electrode exhibits a low voltage plateau because of a high degree of graphitization. Meanwhile, it possesses excellent cycling stability and rate capability due to the high stability of the onion-like structure and fast transport of potassium ions, the latter of which is caused by heteroatom-induced expanded interlayers as found by first-principle calculations. Compared with existing carbon materials, the OLSC synthesized in this study exhibits a high reversible capacity of 466 mAh g-1 at 20 mA g-1 , a reversible capacity of 222 mAh g-1 and capacity retention of 95% after 1600 cycles at 1 A g-1 .
