African American men ages 45-70 were recruited from primary care practices to participate in a study evaluating a decision aid (DA). Immediately after using the DA, patients proceeded to primary care appointments. Afterwards, patients and physicians completed surveys assessing perceptions about SDM. Clinical visits were also audio-recorded and coded to assess SDM.
Mean scores on SDM measures among patients were 73.2 (SD = 27.5, 95% CI 55.71-90.62), 83.1 among physicians (SD = 7.8 95% CI 78.14-88.06), and 67.1 among objective raters (SD = 36.8 95% CI 43.72-90.45). Among patient-provider dyads, mean agreement was 49.9%.
Patients, physicians, and objective observers perceived SDM differently. Understanding discordant experiences of SDM is vital for improving clinical guidance about SDM especially among African Americans who have historically faced healthcare discrimination and mistrust. DAs, particularly for African American men, should incorporate strategies to empower patients to advocate for their communication needs and preferences.

Ventricular arrhythmias (VA) are one of the leading causes of mortality in HF subjects. Implantable cardioverter-defibrillators (ICDs) are currently the gold standard in treatment, preventing arrhythmic sudden cardiac death (SCD) episodes. However, the death rates related to HF remain elevated, as not all HF subjects benefit equally. Cardiac resynchronization therapy (CRT) has emerged as a novel approach for HF patients. check details These devices have been thoroughly investigated in major randomized controlled studies but continue to be underutilized in various countries. This review discusses the use of ICD in HF populations on top of treatments.Metformin has been successfully used as an anti-aging agent but exact molecular mechanisms of metformin in anti-aging remain unknown. Hyperglycemia during skin aging not only causes oxidative damage to cellular macromolecules, like dermal collagen, but also modulates the activation of transcription factor nuclear factor kappa B (NF-kB). We aimed to investigate in vitro effects of high glucose (HG) and metformin treatment on proliferation and apoptosis of human primary dermal fibroblasts (HDFs), and the expression of COL1A1, COL3A1, and RELA/p65 genes. Effects of normal glucose (5.5 mM) and HG concentration (50 mM HG) on HDFs, with two doses of metformin (50 μM and 500 μM), were investigated by immunostaining. Apoptotic levels were analyzed by flow cytometry. Expression of COL1A1, COL3A1, and RELA/p65 genes was measured by quantitative real-time PCR. The proliferation of HDFs was decreased significantly (P less then 0.01) and expression of COL1A1 was downregulated by HG without metformin, whereas proliferation was elevated and expression was upregulated with 500 μM metformin + HG compared to 5.5 mM glucose (P less then 0.05). The expression of COL3A1 and RELA/p65 were upregulated (P less then 0.01 for COL3A1), and percentage of late apoptotic cells increased significantly by HG without metformin (P less then 0.001) while it decreased in two concentrations of metformin dramatically compared with 5.5 mM glucose (P less then 0.01 for expressions and less then 0.001 for apoptosis). Metformin not only significantly downregulated RELA/p65 expression, but also inhibited the apoptosis of HDFs from aged human skin at toxic glucose concentrations which could be inversely mediated via COL1A1 and COL3A1 expression.After learning about facts or outcomes of events, people overestimate in hindsight what they knew in foresight. Prior research has shown that this hindsight bias is more pronounced in older than in younger adults. However, this robust finding is based primarily on a specific paradigm that requires generating and recalling numerical judgments to general knowledge questions that deal with emotionally neutral content. As older and younger adults tend to process positive and negative information differently, they might also show differences in hindsight bias after positive and negative outcomes. Furthermore, hindsight bias can manifest itself as a bias in memory for prior given judgments, but also as retrospective impressions of inevitability and foreseeability. Currently, there is no research on age differences in all three manifestations of hindsight bias. In this study, younger (N = 46, 18-30 years) and older adults (N = 45, 64-90 years) listened to everyday-life scenarios that ended positively or negatively, recalled the expectation they previously held about the outcome (to measure the memory component of hindsight bias), and rated each outcome's foreseeability and inevitability. Compared with younger adults, older adults recalled their prior expectations as closer to the actual outcomes (i.e., they showed a larger memory component of hindsight bias), and this age difference was more pronounced for negative than for positive outcomes. Inevitability and foreseeability impressions, however, did not differ between the age groups. Thus, there are age differences in hindsight bias after positive and negative outcomes, but only with regard to memory for prior judgments.
The ergogenic effects of dietary supplements on endurance exercise performance are well-established; however, their efficacy in hot environmental conditions has not been systematically evaluated.
(1) To meta-analyse studies investigating the effects of selected dietary supplements on endurance performance and core temperature responses in the heat. Supplements were included if they were deemed to (a) have a strong evidence base for 'directly' improving thermoneutral endurance performance, based on current position statements, or (b) have a proposed mechanism of action that related to modifiable factors associated with thermal balance. (2) To conduct meta-regressions to evaluate the moderating effect of selected variables on endurance performance and core temperature responses in the heat following dietary supplementation.
A search was performed using various databases in May 2020. After screening, 25 peer-reviewed articles were identified for inclusion, across three separate meta-analyses (1) exercise performance; (2) end core temperature; (3) submaximal core temperature.
