Objectives MRI is an integral part of breast cancer screening in high-risk patients. We investigated whether the application of the Kaiser score, a clinical decision-support tool, may be used to exclude malignancy in contrast-enhancing lesions classified as BI-RADS 4 on breast MRI screening exams. Methods This retrospective study included 183 consecutive, histologically proven, suspicious (MR BI-RADS 4) lesions detected within our local high-risk screening program. All lesions were evaluated according to the Kaiser score for breast MRI by three readers blinded to the final histopathological diagnosis. The Kaiser score ranges from 1 (lowest, cancer very unlikely) to 11 (highest, cancer very likely) and reflects increasing probabilities of malignancy, with scores greater than 4 requiring biopsy. Receiver operating characteristic (ROC) curve analysis was used to evaluate diagnostic accuracy. Results There were 142 benign and 41 malignant lesions, diagnosed in 159 patients (mean age, 43.6 years). A number of different methods have been used to generate isogenic control lines using CRISPR (clustered regularly interspaced short palindromic repeats)/Cas9 and non-homologous end-joining (NHEJ) by deleting the repeat region, with the risk of creating indels and genomic instability. In this study we demonstrate complete correction of an induced pluripotent stem cell (iPSC) line derived from a C9orf72-HRE positive ALS/FTD patient using CRISPR/Cas9 genome editing and homology directed repair (HDR), resulting in replacement of the excised region with a donor template carrying the wild-type repeat size to maintain the genetic architecture of the locus. The isogenic correction of the C9orf72 HRE restored normal gene expression and methylation at the C9orf72 locus, reduced intron retention in the edited lines, and abolished pathological phenotypes associated with the C9orf72 HRE expansion in iPSC derived motor neurons (iPSMNs). RNA sequencing of the mutant line identified 2220 differentially expressed genes compared to its isogenic control. Enrichment analysis demonstrated an over-representation of ALS relevant pathways, including calcium ion dependent exocytosis, synaptic transport and the KEGG ALS pathway, as well as new targets of potential relevance to ALS pathophysiology. Complete correction of the C9orf72 HRE in iPSMNs by CRISPR/Cas9 mediated HDR provides an ideal model to study the earliest effects of the hexanucleotide expansion on cellular homeostasis and the key pathways implicated in ALS pathophysiology.Background In the United States, human papillomavirus (HPV) vaccination has been recommended for young adult men who have sex with men (MSM) since 2011. Methods The Vaccine Impact in Men (VIM) study surveyed MSM and transgender women aged 18-26 years in 3 U.S. cities during 2016-2018. Self-collected anal swab and oral rinse specimens were assessed for 37 types of HPV DNA. We compared HPV prevalence among vaccinated and unvaccinated participants and determined adjusted prevalence ratios (aPR) and confidence intervals (CI). Results Among 1,767 participants, 704 (39.8%) self-reported receiving HPV vaccine. Median age at vaccination (18.7 years) was older than age at first sex (15.7 years). Quadrivalent vaccine-type HPV was detected in anal or oral specimens from 475 (26.9%) participants. Vaccine-type HPV prevalence was lower among vaccinated (22.9%) compared with unvaccinated (31.6%) participants; aPR for those who initiated vaccination at ≤18 years was 0.41 (95% CI 0.24-0.57) and at >18 years was 0.82 (95% CI 0.67-0.98). Vaccine effectiveness for at least one HPV vaccine dose at age ≥18 years or >18 years was 59% and 18%, respectively. Conclusions Findings suggest real-world effectiveness of HPV vaccination among young adult MSM. This effect was stronger with younger age at vaccination.A faunal study is presented to determine the species of ectoparasites found in the opossum Didelphis virginiana (Kerr) (Marsupialia Didelphidae). For this, Tomahawk traps were placed in the peridomiciles of a rural town in Yucatán, to capture individuals of this marsupial and proceed to the collection of their ectoparasites. A total of 3,023 arthropods were collected from 145 opossums. The most frequent ectoparasites were the acarines Ornithodoros (Alectorobius) nr. talaje (64.8%) (Argasidae); Ornithonyssus wernecki Fonseca (53.8%) (Macronyssidae) and Didelphilicus serrifer Fain (25.5%) (Atopomelidae); the ticks Amblyomma parvum Aragão (8.3%) and A. mixtum Koch (10.3%) (Ixodidae); and the fleas Ctenocephalides felis (Bouché) (20.7%) and Pulex simulans Baker (8.3%) (Pulicidae). It is concluded that the ectoparasite fauna of this marsupial is mainly composed of euryxenous organisms, which have been documented as vectors of diseases caused by rickettsial bacteria. Since D. virginiana is widely adapted to the peridomiciliary environment in the region, this study allows recognizing those that represent a potential risk for the transmission of vector-borne zoonotic diseases.
