in English, German EINLEITUNG Schaftprothesen stellten bei der Endoprothetik des Schultergelenkes über Jahrzehnte den Goldstandard dar. In den letzten Jahren wurden für das Schultergelenk knochensparende Endprothesendesigns entwickelt. Aktuelle Studien konnten zeigen, dass die native Krafteinleitung auf den proximalen Humerus am besten durch schaftfreie Implantate imitiert werden kann. Es bestehen jedoch Bedenken bezüglich der Langzeit-Performance solcher schaftfreier Schulterprothesendesigns. Diese Studie hatte zum Ziel, die im Langzeitverlauf entstehenden knöchernen Veränderungen des proximalen Humerus nach schaftfreier anatomischer Schultertotalendoprothesenversorgung zu untersuchen. MATERIAL UND METHODEN Zwischen 2008 und 2010 wurden 48 Schultern bei 43 Patienten mit einer anatomischen schaftfreien press-fit Schultertotalendoprothese (TESS, Biomet) versorgt. 30 Schultern bei 25 Patienten mit einem Durchschnittsalter von 65,7 ± 9,9 (34 bis 82) Jahren standen für eine klinische und radiologische Auswertung das klinische Ergebnis hatten. Das schaftfreie Press-fit-Prothesendesign wies eine 8-Jahres-Performance auf, die mit jener konventioneller Schaftprothesen vergleichbar ist.The Dark Triad of personality is a cluster of three socially aversive personality traits Machiavellianism, narcissism and psychopathy. These traits are associated with a selfish, aggressive and exploitative interpersonal strategy. The objective of the current study was to establish relationships between the Dark Triad traits (and their dimensions) and momentary affect. Machiavellianism, grandiose narcissism, vulnerable narcissism and the dimensions of the Triarchic model of psychopathy (namely, boldness, meanness and disinhibition) were examined. We used the Day Reconstruction Method, which is based on reconstructing affective states experienced during the previous day. The final sample consisted of 270 university students providing affective ratings of 3047 diary episodes. Analyses using multilevel modelling showed that only boldness had a positive association with positive affective states and affect balance, and a negative association with negative affective states. Grandiose narcissism and its sub-dimensions had no relationship with momentary affect. The other dark traits were related to negative momentary affect and/or inversely related to positive momentary affect and affect balance. As a whole, our results empirically demonstrated distinctiveness of the Dark Triad traits in their relationship to everyday affective states. These findings are not congruent with the notion that people with the Dark Triad traits, who have a dispositional tendency to manipulate and exploit others, are generally cold and invulnerable to negative feelings. The associations between the Dark Triad and momentary affect were discussed in the contexts of evolutionary and positive psychology, in relation to the role and adaptive value of positive and negative emotions experienced by individuals higher in Machiavellianism, narcissism and psychopathy.Spinal cord stimulation (SCS) is receiving increasing interests for treating pain and gait disorders in patients with Parkinson's disease (PD). In an SCS study, it is hard to apply a double-blind approach, especially at low frequencies, as the stimulation normally induces paresthesia which can be perceived by the patient. We herein demonstrate a case treated with SCS in which a blinding design was accomplished by an accidental dislocation of a stimulation lead. A 73-year-old man with PD was admitted to our hospital because of relapsed low back pain. This was due to the dislocation of a previously implanted SCS lead, which caused a decrease in its effectiveness in alleviating pain (from 81 to 43% measured by King's Parkinson's Disease Pain Scale) and improving gait (from 35 to 28% measured by the timed up and go test). A second SCS surgery using a paddle lead solved this problem, with improvements in pain and gait rebounded to 81 and 45%. In this case, the paresthesia induced by SCS (using either a paddle lead or percutaneous leads) was below the threshold of perception when the patient was sitting and standing, and a dislocation of one previously implanted percutaneous lead did not cause evident changes in his sensation of paresthesia. At last follow-up, the patient's quality of life had improved by 40% as measured by the 8-item Parkinson's Disease questionnaire (PDQ-8). This study could serve partly as a proof that low-frequency SCS is effective in improving pain as well as gait problems in PD patients, which was unlikely a result of a placebo effect. © 2020 S. Karger AG, Basel.OBJECTIVE Precise diagnosis of allergy requires knowledge of the population's food allergy (FA) spectrum and predictors. METHODS Medical charts of Turkish children aged 0-2 years with FA and/or atopic dermatitis (AD) were reviewed. RESULTS A total of 1,389 patients, 912 with FA and 1,140 with AD, were included. read more In the FA group, the most frequently diagnosed FAs were egg white (75.9%), cow's milk (55.7%), tree nuts (31.5%) and sesame (20.6%). The detection of FA in 99% of children with any kind of FA necessitate testing with egg white, cow's milk, hazelnut, sesame, walnut, cashew, and pistachio. In the FA group, 72.7 and 56.8% had AD and multiple FA respectively. Multiple FA (56.8 vs. 49.8%) and hen's egg allergy (85.5 vs. 50.2%, p less then 0.005) were more common and cow's milk allergy (51.4 vs. 67.1%, p less then 0.005) less common in the AD subgroup of the FA group than in the non-AD subgroup. Multiple FA likelihood increases parallel to the severity of AD (p less then 0.05). In the AD group, 58.2% had an immunoglobulin E-mediated FA. The risk of concomitant FA increased as the age at symptom onset of AD decreased (OR 0.800 [95% CI 0.731-0.875]; p less then 0.001) and the severity of AD increased (OR 2.350 [95% CI 1.898-2.911]; p less then 0.001). CONCLUSION Although severe and early-onset AD is a predictor of the presence and magnitude of FA in infancy, the spectrum of FA is a reflection of cultural characteristics. The clinical presentations of both AD and FA may in fact be an expression of the extent of the immune dysregulation underlying atopy and allergy. © 2020 S. Karger AG, Basel.
