Faecal microbiota transplantation (FMT) is the transfer of screened and minimally processed faecal material from a 'healthy' donor to 'diseased' recipient. It has an established role, and is recommended as a therapeutic strategy, in the management of recurrent Clostridioides difficile infection (CDI). Recognition that gut dysbiosis is associated with, and may contribute to, numerous disease states has led to interest in exploiting FMT to 'correct' this microbial imbalance. Conditions for which it is proposed to be beneficial include inflammatory bowel disease, irritable bowel syndrome, liver disease and hepatic encephalopathy, neuropsychiatric conditions such as depression and anxiety, systemic inflammatory states like sepsis, and even coronavirus disease 2019. selleck products To understand what role, if any, FMT may play in the management of these conditions, it is important to consider the potential risks and benefits of the therapy. Regardless, there are several barriers to its more widespread adoption, which include incompletely understood mechanism of action (especially outside of CDI), inability to standardise treatment, disagreement on its active ingredients and how it should be regulated, and lack of long-term outcome and safety data. Whilst the transfer of faecal material from one individual to another to treat ailments or improve health has a history dating back thousands of years, there are fewer than 10 randomised controlled trials supporting its use. To bring together understanding of the role that beavers may play in the management of water resources, freshwater, and terrestrial ecosystems, this article reviews the state-of-the-art scientific understanding of the beaver as the quintessential ecosystem engineer. This review has a European focus but examines key research considering both Castor fiber-the Eurasian beaver and Castor canadensis-its North American counterpart. In recent decades species reintroductions across Europe, concurrent with natural expansion of refugia populations has led to the return of C. fiber to much of its European range with recent reviews estimating that the C. fiber population in Europe numbers over 1.5 million individuals. As such, there is an increasing need for understanding of the impacts of beaver in intensively populated and managed, contemporary European landscapes. This review summarizes how beaver impact (a) ecosystem structure and geomorphology, (b) hydrology and water resources, (c) water quality, (d) freshwater ecology, and (e) humans and society. It concludes by examining future considerations that may need to be resolved as beavers further expand in the northern hemisphere with an emphasis upon the ecosystem services that they can provide and the associated management that will be necessary to maximize the benefits and minimize conflicts. During the COVID-19 pandemic, emergency surgery was modified in line with Royal College guidance to accommodate the evolving climate. This study compared management of appendicitis before and during the pandemic by assessing disease presentation severity, modes of investigation, surgical management and patient outcomes. Outcomes assessed included length of stay, readmissions and rates of postoperative wound infections.

We collected data on appendicitis patients managed at a district general hospital over two distinct 8-week periods; 42 patients before and 36 patients during the COVID-19 pandemic respectively. The study included clinically or radiologically diagnosed appendicitis patients.

Our study found patients during the COVID-19 pandemic had higher inflammatory markers (CRP 103 vs 53mg/L; p=0.03) and more severe disease on histological examination of the appendix than pre-pandemic. Patients were nearly twice as likely to undergo CT diagnosis of appendicitis during the pandemic than before. During the pandemic, only half of the cohort underwent laparoscopic appendicectomy in contrast with greater than 85% of the pre-COVID-19 cohort (p=0.0005). Patients in the COVID-19 era cohort recorded shorter lengths of hospital stay (2.6 vs 3 days; p=0.35); however, had higher reattendance rates (12 vs 25%; p=0.15) and surgical site infections (p=0.0443). Finally, the study reported shorter median time to theatre (0 vs 1 days) during the pandemic than before.

In addition to reiterating the benefits of laparoscopic versus open surgery and quicker diagnostic methods, this study also implies that though patients during COVID-19 era presented with more severe disease, their treatment was in a more efficient service.
