IgA level less than 10 mg/dl and IgG level less than 600 mg/dl were determined in 77.8% and 25.9% of patients, respectively. Positive HBT was detected in 40.7% (n=11) of the patients. In the positive HBT group, bloating, chronic diarrhea and abdominal pain were the most common lower GI manifestations. There was no significant difference in terms of age, BMI, IgA level, and duration of CVID between the positive and negative HBT groups. The significant association of co-occurrence of anemia and abdominal pain with positive HBT (positive predictive value 100%) might be considered as a clue to SIBO diagnosis. Conclusions.Regarding the high prevalence and non-specific manifestation of SIBO, it is suggested to consider concurrent symptoms in patients with CVID to manage the timely and precise diagnosis of SIBO.The political processes surrounding the Affordable Care Act (ACA) offer valuable lessons about race and politics in the United States. In particular, the ACA underscores a critical tension between politics and policy in a racialized polity even when policies are intended to target and address racial disparities, politics can undermine the steps necessary to winnow racial disparities. Close scrutiny of the ACA during its first decade reveals how race intersects with politics to render public policy less equitable and more vulnerable to erosion. Ultimately, this analysis points to the ways that racialized political processes are formidable barriers to equitable material outcomes. By examining such processes and making them visible, this article elucidates the possibilities, limits, and contours of public policy as a mechanism for achieving racial justice. Copyright © 2020 by Duke University Press.BACKGROUND Removing the jugular tubercle (JT) is regarded as an important step in the far-lateral approach; however, few cadaveric studies have objectively evaluated it. OBJECTIVE To quantitatively analyze the effect of JT removal in the far-lateral approach, using cadaveric computed tomography (CT) and magnetic resonance (MR) imaging. METHODS The far-lateral, supra-articular transcondylar transtubercular approach was employed on 23 sides of 13 formalin-fixed cadaveric heads. CT bone images were obtained before and after JT removal, and MR images were obtained before dissection and were merged with the CT bone images. The angles of attack used to approach the ventral region of the medulla, the distances between the medulla and the bony structure, and the volume of the paramedullary space were measured at the level of the JT on axial CT-MR fusion images. The values obtained after JT removal were compared with those obtained before JT removal. RESULTS All evaluated values were significantly increased after JT removal, including the angle of attack at the level of the JT (29.8 ± 7.4° vs 58.2 ± 15.5°, P less then .001), the distance between the olive and the JT (6.4 ± 2.0 mm vs 9.5 ± 5.0 mm, P = .01), and the volume of the space around the medulla (0.28 ± 0.04 cm3 vs 0.47 ± 0.09 cm3, P less then .001). CONCLUSION The paramedullary surgical working space widened by JT removal was quantitatively demonstrated in the cadaveric CT and MR imaging study. The measurement methods in this study can be applied to clinical cases and other skull base cadaveric studies. Copyright © 2020 by the Congress of Neurological Surgeons.BACKGROUND The survival benefit in maximizing resection in glioblastomas (GBMs) has been demonstrated by numerous studies. The true limit of infiltration of GBMs has been an overwhelming obstacle, and several technological advances have been introduced to improve the identification of residual tumors. OBJECTIVE To evaluate whether the integration of 5-aminolevulinic acid (5-ALA) with microbubble contrast-enhanced ultrasound (CEUS) improves residual tumor identification and has an impact on the extent of resection (EOR), overall survival (OS), and progression-free survival (PFS). METHODS A total of 230 GBM procedures were retrospectively studied. Cases were stratified according to the surgical procedure into 4 groups 5-ALA- and CEUS-guided surgeries, 5-ALA-guided surgeries, CEUS-guided surgeries, and conventional microsurgical procedures. RESULTS Patients undergoing conventional microsurgical procedures showed the worst EORs compared to the assisted techniques (5-ALA and CEUS procedures). Both 5-ALA and CEUS techniques improved the EOR compared to conventional microsurgical procedures. However, their combination gave the best results in terms of the EOR (P = .0003). The median EOR% and the number of supramarginal resections are hence superior in the 5-ALA + CEUS + group compared to the others; this observation had consequences on PFS and OS in our series. CONCLUSION In terms of the EOR, the best results can be achieved through a combination of both techniques, where the 5-ALA-guided procedure is followed by a final survey with CEUS. Compared with other intraoperative imaging techniques, CEUS is a real-time, readily repeatable, safe, and inexpensive technique that provides valuable information to the surgeon before, during, and after resection. Copyright © 2020 by the Congress of Neurological Surgeons.We examined whether routine fall yard maintenance, specifically depositing leaves removed from lawns and landscaping along forest margins, may increase densities of nymphal Ixodes scapularis Say and Amblyomma americanum (L.) ticks within these managed areas. Leaf blowing activities in fall 2017 and 2018 on residential properties in New Jersey, United States, significantly increased leaf litter depth in managed edge areas (range = 259.8-352.8 mm) compared to unmanaged edges (77.6-188.0 mm) and adjacent forest (39.4-166.2 mm). Drag sampling conducted on 20 and 30 dates in spring 2018 and 2019, respectively, yielded ≥3-fold more I. scapularis nymphs in managed edge plots compared to natural edge and forest plots in both years. In 2018, we collected more A. americanum nymphs from forest plots than from either natural or managed edge plots, but 2019 natural edge plots yielded the greatest number of the ticks. Nearly half of A. americanum adults were collected in forest plots in both years. Our data suggest that the acarological risk of human encounters with I.
