The limitation or loss of internal rotation (IR) after the surgery is a major problem in reverse shoulder arthroplasty (RSA). The particular factors associated with postoperative IR remain unclear. We aimed to analyze the predictors of IR after RSA.
We included primary RSA patients with the following implants and a minimum of 1-year follow-up Wright Aequalis (n=25), DJO Encore (n=29), Biomet Comprehensive (n=40), and Exactech Equinoxe (n=29). Age, sex, dominant hand, primary diagnosis, implant type, preoperative critical shoulder angle, pre- and postoperative acromiohumeral distance, lateral offset of implant, glenosphere inclination, peg-glenoid rim distance (PGRD), glenosphere overhang, scapular notching, subscapularis (SSc) repair, pre- and postoperative ROM, and functional scores were assessed. IR was assessed using a 10-point scale based on the following anatomical levels from the greater trochanter to the buttocks (2 points), from the sacrum to L4 (4 points), from L3 to L1 (6 points), from T12 to T8oor preoperative IR. Preoperative diagnosis, implant type, SSc repair, implant lateralization, glenosphere inferior placement, and scapular notching were not identified as independent predictors of IR after RSA in our sample size.
Preoperative IR was the only independent factor for IR after RSA. Patients with excellent preoperative IR had better postoperative IR than those with poor preoperative IR. Preoperative diagnosis, implant type, SSc repair, implant lateralization, glenosphere inferior placement, and scapular notching were not identified as independent predictors of IR after RSA in our sample size.
To investigate the feasibility of three-dimensional (3D) reconstruction with an interactive Hisense computer-assisted system (CAS) for preoperative planning and intraoperative guidance during laparoscopic-assisted upper pancreatic lymph node dissection in distal gastrectomy for gastric cancer.
This study included 28 patients who underwent preoperative 3D reconstruction of the upper border of the pancreas using Hisense CAS (3D reconstruction group) for preoperative planning and intraoperative navigation. To determine its efficacy, the clinical data of these patients were compared with those of 28 patients who did not undergo 3D reconstruction (control group).
Fifty-six cases of laparoscopic-assisted distal gastrectomy were performed. Three-dimensional reconstruction was successful in all the patients in the 3D reconstruction group, and real-time navigation was performed during the operation. The rate of correspondence between the 3D reconstruction images and intraoperative findings was 100%. The time takis technique is helpful for the implementation of the precise dissection of lymph nodes at the upper edge of the pancreas and improves the quality and safety of the surgery.Successful biologics must satisfy multiple properties including activity and particular physicochemical features that are globally defined as developability. These multiple properties must be simultaneously optimized in a very broad design space of protein sequences and buffer compositions. We highlight the emerging applications of ML in biologics discovery and development, focusing on protein engineering, early biophysical screening, and formulation. We discuss the power of ML in extracting information from complex datasets and in reducing the necessary experimental effort to simultaneously achieve multiple quality targets. We finally anticipate possible future interventions of AI in several steps of the biological landscape.
Evaluate aromatherapy for postoperative abdominal pain in hospitalized patients.
A randomized controlled trial design.
Study participants (n= 172) were randomized to receive either standard care or standard care and aromatherapy (AT) for postsurgical pain up to 24hours after admission to a nonintensive care surgical unit. A convenience sample was recruited before surgery and given instructions on self-rating pain intensity. The AT group was topically administered a drop of lavender essential oil after medication and at random for pain. Pain scores and medications data were collected.
Of the evaluable patients (n= 147), demographic data were similar (standard care and AT groups). The use of aromatherapy showed no substantial benefit at improving pain scores or reducing medication use (the primary objectives of the study). A subgroup analysis of patients who received a regional nerve block for pain management, however, showed more than fivefold improvement in pain scores after the use of aromatherapy. The AT group used more medications at baseline (P=.032), whereas 70% less medications were used (P=.031) by 24hours.

In patients who received a regional nerve block, significant improvement in pain level occurred as effects of the block diminished.We have recently proposed a new approach to evaluate 2D femoral neck (FN) structure, named the Minimal Model (MM), that comprised FN areal bone mineral density (FNaBMD) and FNWidth and 2 new internal structural measures; (1) the standard deviation of normalized mineral mass projection profile distribution (FNSigma), and (2) the displacement between center-of-mineral mass and geometric center of mineral mass projection profile (FNDelta). The contralateral hip of 67 FN fracture Chinese male patients had a QCT scan shortly after fracture and was compared to 156 community participants without hip fracture. The QCT scans were analyzed using Mindways software to enable DXA-equivalent 2D images to be obtained; MM variables were calculated from these images. In FN fracture and nonfracture participants, the 4 MM variables as well as age, weight and height were compared. Compared to nonfracture, fracture participants were older, weighed less and were taller. After adjustment for these differences FN fracture participants compared to nonfracture had mean ± SD lower FNaBMD 0.
