Off-road riding of quad bikes and motorcycles is common among children across rural and remote Australia, but is a significant source of injury and hospitalisation. An in-depth analysis of paediatric off-road vehicle crashes was undertaken to inform injury prevention countermeasures by characterising injury patterns and sources of injury.

What is the central question of this study? The current literature indicates that oxidative stress plays a major role in iron overload. Although exercise is a well-established approach to treat/prevent cardiovascular diseases, its effects on iron overload are not known. What is the main finding and its importance? Moderate-intensity aerobic training had benefits in a rodent model of iron-overload cardiomyopathy by improving the antioxidant capacity of the heart. After further confirmation by translational and clinical studies, we should consider using this non-pharmacological, highly accessible and easily executable adjuvant approach allied to other therapies to improve the quality of life of iron-overloaded patients.
Iron is an essential micronutrient for several life processes, but its excess can damage organs owing to oxidative stress, with cardiomyopathy being the leading cause of death in iron-overloaded patients. Although exercise has long been considered as a cardioprotective tool, its effects on issociation with an increase in the endogenous antioxidant enzyme catalase. Due to the efficacy of tropomyosin receptor kinase (TRK) inhibitor therapy in tumours with rearrangements of neurotrophic tyrosine kinase receptor (NTRK) genes, there has been a surge in demand for NTRK fusion screening. To date, most studies involving mesenchymal tumours have been focusing on soft tissue tumours, while data on bone tumours are sparse. Hence, we aimed to explore the frequency of NTRK fusions in a large series of primary bone tumours.
Immunohistochemical expression of pan-Trk was successfully assessed in 354 primary bone tumours using tissue micro arrays (TMAs). In a selection of positive cases, additional molecular analysis for NTRK fusion analysis was performed using anchored multiplex PCR-based targeted NGS.
Positivity was encountered in nineteen cases (5%) and included Ewing sarcoma (n=6, 33%), osteosarcoma (n=11, 13%) and giant cell tumour of bone (n=2, 3%). In all, except one case, cytoplasmic staining was observed. Weak staining was most often observed (n=13), while five cases showed moderate staining and one case showed focal strong staining. Molecular analysis was successful in six cases, which were all negative for NTRK fusions.
The likelihood of finding a NTRK fusion in bone tumours in clinical practice is extremely low. This may imply that, if more comprehensive large scale molecular studies confirm this, routine predictive NTRK testing in bone tumour patients with advanced disease may be reconsidered.

Central neuromodulators are an effective treatment for irritable bowel syndrome (IBS) but may be used less frequently than other therapies.
To survey gastroenterologists in the United States (US) about their use of neuromodulators in patients with IBS.
A 23-question survey was distributed to gastroenterologists in the United States. Comparisons in prescribing practices were conducted between (a) gastroenterologists who were "high prescribers" versus "low prescribers" of neuromodulators in patients with IBS and (b) gastroenterologists and "gastroenterology experts" in the use of neuromodulators using descriptive statistics and multivariable logistic regression analyses.
The 525 gastroenterologists who were surveyed used neuromodulators for a median range of 21%-30% of their patients with IBS. Neuromodulators were described as extremely/very important in managing IBS by 55% of clinicians. Significant predictors of high-prescribing behaviour were academic versus clinical practice setting (odds ratio [OR]ractice, focus in IBS and perceive central neuromodulators as effective.
