As the progress of population aging in China, the proportion of elderly population is increasing. Both chronic diseases and infectious diseases can threaten the health of the elderly. There are many kinds of infectious diseases, including vaccine preventable infectious diseases affecting the health of adults, such as influenza, pneumococcal diseases and herpes zoster. In addition, the newly emerged COVID-19 has caused a pandemic in the world, resulting the highest proportion of deaths occurred in the elderly and posing a serious threat to the health of the elderly. This paper mainly summarizes the prevention and control of vaccine preventable diseases and COVID-19 to which the elderly are susceptible, analyzes the infectious disease problems affecting the health of elderly population, and recommends countermeasures for the prevention and control of these diseases in elderly population.Hepatocellular carcinoma (HCC) caused by hepatitis B virus (HBV) is the first cause of cancer-caused death in China. This study, from the perspective of HBV genomic epidemiology, systematically investigated the evolution of HBV and their interaction with host genetic factors in each stage of HBV carcinogenesis and comprehensively analyzed the role of apolipoprotein B mRNA editing enzyme catalytic polypeptides (APOBEC) family in the inflammation-to-cancer transformation. Based on our findings and related studies, we proposed "cancer evolution and development" (Cancer Evo-Dev) theoretical system suitable for most cancer types. Cancer Evo-Dev lays a theoretical foundation for understanding the mechanism of inflammation to promote the development of cancer, which is of great significance for the specific prevention, prediction, early diagnosis, and targeted therapy of malignant diseases.Recent years with the rapid progress in high-throughput omics techniques, the accumulation of population cohorts and biobanks, great advances in internet and information technology, and the emerging tools for big data utilization, health care big data provide abundant data resources and broad research platforms for epidemiological research. We aimed to review the opportunities and challenges of epidemiological research in the era of big data, and explore the future development of epidemiology. Epidemiology should seize the opportunities, open up new directions, develop new methods, and promote the translation of research results into public health and clinical medicine, which will eventually realize the vision of "Healthy China".One of the main aims of chronic disease epidemiology is to explore the etiological factors of diseases. Multi-omics includes all genes (genomics), extensive changes in gene expression (epigenetics), ribonucleic acids (transcriptomics), and proteins (proteomics) generated during the process of DNA replication, transcription, translation, and post-translational modification, as well as small molecule metabolites downstream (metabolomics). Multi-omics platforms provide technical support for assessing omics biomarkers including genomics, transcriptomics, and proteomics, while systems epidemiology provides theoretical and methodological support for using multi-omics to conduct etiological research. Multi-omics research not only reveals the interaction network between molecules, but also contributes to causal inference from the molecular level. With the global exponential growth of publicly available data, analysis platforms, and consortia, resources for multi-omics research will become more abundant, and the depth and breadth of research will be greatly expanded. This article will review the applications of multi-omics approach in the etiologic research on non-communicable diseases, representative research in the past three years, opportunities and challenges for large-scale cohort studies, advantages and issues of multi-omics research in the Chinese population, and future perspectives.
Ebstein's anomaly (EA) is a kind of congenital heart disease, which is currently widely treated by cone reconstruction. However, the prediction of postoperative recovery is still challenging.
A retrospective analysis was performed on EA cases undergoing cone reconstruction from January 2010 to January 2016. Univariate and multivariate logistic regression analyses were performed, with postoperative adverse events defined as dependent variable and pre-and intraoperative parameters defined as independent variables. The predictive capacity of preoperative percutaneous oxygen saturation (SPO
) and Great Ormond Street (GOS) score was evaluated using areas under the curve of the receiver operating characteristic (ROC).
Preoperative SPO
was 95.7 ± 5.20%. Cardiopulmonary bypass, aortic cross-clamp, postoperative mechanical ventilation, and hospitalization time were 101.7 ± 28.26 min, 60.9 ± 18.04 min, 16 h (8, 22), and 8 days (7, 11), respectively. The incidence of total postoperative adverse events, including low cardiac output syndrome, mechanical ventilation more than 3 days, postoperative hospitalization more than 2 weeks, postoperative reintubation, extracorporeal membrane oxygenation assistance, and death, was 13.1% (n = 13). Low preoperative SPO
(p = .001,odds ratio [OR] = 0.834), GOS score (p = .021, OR = 0.368), and cardiopulmonary bypass time (p = .034, OR = 1.021) were risk factors for adverse events. Multivariate logistic regression analysis showed that low preoperative SPO
(p = .002, OR = 0.846) and GOS score (p = .043, OR = 0.577) were independent risk factors for adverse events. The areas of SPO
and GOS score under the ROC curve were 0.764 and 0.740, respectively.
Low preoperative SPO
and GOS score were predictors of adverse events after cone reconstruction, and SPO
was more convenient and objective than the GOS score.
Low preoperative SPO2 and GOS score were predictors of adverse events after cone reconstruction, and SPO2 was more convenient and objective than the GOS score.En bloc heart-lung transplantation still represents definitive therapy for end-stage cardiopulmonary failure. selleck chemicals However, patients may critically decompensate while awaiting suitable donor organs and necessitate veno-arterial extracorporeal membrane oxygenation. In this article, we describe the combined use of central cannulation with the Berlin Heart EXCOR ventricular assist device cannulae and the CentriMag centrifugal pump as an extended bridge to heart-lung transplantation in three pediatric patients.
