Extensive summer grazing is a dairy herd management practice frequently adopted in mountainous areas. Nowadays, this activity is threatened by its high labour demand, but it is fundamental for environmental, touristic and economic implications, as well as for the preservation of social and cultural traditions. Scarce information on the effects of such low-input farming systems on cattle health is available. Therefore, the present case study aimed at investigating how grazing may affect the health status of dairy cows by using milk traits routinely available from the national milk recording scheme. The research involved a dairy herd of 52 Simmental and 19 Holstein × Simmental crossbred cows. The herd had access to the pasture according to a rotational grazing scheme from late spring up to the end of summer. Therefore, the proper management of cows on pasture can be a valuable solution to preserve the economic, social and environmental sustainability of small dairy farms in the alpine regions, without impairing cows' health.Background and Objectives Musculoskeletal dysfunction can induce several types of chronic pain syndromes. It is of particular interest to elucidate the pathomechanism of different forms of chronic pain. It is possible that patients who have developed chronic widespread pain (CWP) may endure different pathomechanisms as compared to those who suffer from local pain (osteoarthritis, OA) and regional pain (chronic low back pain, cLBP), especially with regard to pain regulation and its related biomediators. The aim of this study was to determine the differences in pathomechanisms among these patients by measuring pain-related biomediators, particularly brain-derived neurotrophic factor (BDNF). Additionally, subpopulations of immune cells were determined in parallel. Materials and Methods Patients and healthy subjects (HSs) were recruited (age and gender-matched). BDNF was measured from serum samples of patients and HSs and the data of body composition parameters were recorded. Additionally, both patients and HSs were asked to fill in questionnaires related to pain intensity, anxiety, and depression. Results Our results highlight that the levels of both free and total BDNF are significantly lower in pain patients compared to HSs, with p values of 0.041 and 0.024, respectively. The number of CD3- CD56bright natural killer (NK) cells shows significant differences between the groups. Comparing all chronic pain patients with HSs reveals a significantly lower number of CD4+ CD8+ T cells (p = 0.031), CD3- CD56bright NK cells (p = 0.049) and CD20+ CD3- cells (p = 0.007). A survey was used to examine whether having access to cooperative programs and social support impacted symptoms of depression among dairy farmers. However, no optimal comorbidity score is established for this purpose. Therefore, this study aimed to compare the predictive value of the Charlson comorbidity index for other-cause mortality with the use of a simple comorbidity count and to assess the impact of frequently occurring comorbidities. Methods Surgically treated patients with stages I-III breast cancer aged ≥70 years diagnosed between 2003 and 2009 were selected from the Netherlands Cancer Registry. Competing risk analysis was performed to associate 5-year other-cause mortality with the Charlson index, comorbidity count, and specific comorbidities. Discrimination and calibration were assessed. Results Overall, 7511 patients were included. Twenty-nine percent had no comorbidities, and 59% had a Charlson score of 0. After five years, in 1974, patients had died (26%), of which 1450 patients without a distant recurrence (19%).
