This paper describes the occupations in the U.S. that involve close contact with others and whether the work is outdoors or indoors (risk factors for COVID-19), including the distribution of Black and Hispanic workers over these occupations.

U.S. data released from 2014 to 2019 on employment, proximity to others at work, outdoor or indoor work, and Black and Hispanic worker percentages for occupations were used. Occupations were assigned to 6 categories defined as a low, medium, or high physical closeness (proximity) at work and outdoor or indoor work. A total of 3 of the 6 categories represent a higher risk for exposure to SARS-CoV-2 medium-proximity indoor, high-proximity outdoor, and high-proximity indoor exposure.

A high proportion of U.S. workers may be at higher risk for exposure to SARS-CoV-2 because their occupations involve either high proximity to others indoors and outdoors (25.2%, 36.5 million workers) or medium-proximity indoors (48%, 69.6 million workers). There is a differential distribution of proximity and outdoor/indoor work by occupation, which disproportionately affects Black and Hispanic workers in some occupations.

Implementation of COVID-19 preventive measures in work settings should be based on occupation-specific risk factors, including the extent of proximity to others and whether the work is conducted outdoors or indoors. It is important that communication messages are tailored to the languages and preferred media of the workforce.

The objective of this study is to identify county-level characteristics that may be high-impact targets for opioid and antibiotic interventions to improve dental prescribing.

Prescriptions during 2012-2017 were extracted from the IQVIA Longitudinal Prescription database. Primary outcomes were yearly county-level antibiotic and opioid prescribing rates. Multivariable negative binomial regression identified associations between prescribing rates and county-level characteristics. All analyses occurred in 2020.

Over time, dental opioid prescribing rates decreased by 20% (from 4.02 to 3.22 per 100 people), whereas antibiotic rates increased by 5% (from 6.85 to 7.19 per 100 people). selleckchem Higher number of dentists per capita, higher proportion of female residents, and higher proportion of residents aged <65 years were associated with increased opioid rates. Relative to location in the West, location in the Northeast (59%, 95% CI=52, 65) and Midwest (64%, 95% CI=60, 70) was associated with lower opioid prescribing rates. Higher clinician density, median household income, proportion female, and proportion White were all independently associated with higher antibiotic rates. This case study presented a complex pulmonary artery stenosis initially treated with bilateral pulmonary artery stenting. The patient was found to have aortic valve endocarditis and recurrent pulmonary artery stenosis. The aortic valve was replaced and the pulmonary valve was repaired, followed by interposition of a Y-shaped graft from the sinotubular junction to both pulmonary arteries. The patient had an uneventful recovery and was discharged home asymptomatic after endocarditis antibacterial treatment.
In people with a Fontan circulation, serial cardiopulmonary exercise testing (CPET) to evaluate change in peak exercise capacity has been increasingly recognised as a useful prognostic tool; a decline is associated with adverse clinical outcomes. The aim of this study is to describe the "natural" history of exercise capacity in the Australian and New Zealand (ANZ) Fontan cohort and to identify factors associated with a decline.

The ANZ Fontan registry was retrospectively reviewed for adolescent and adult patients (≥16 years) with serial CPET results performed on a cycle ergometer ≥6 months apart. Patients were excluded if they underwent a surgical procedure or fenestration closure in-between tests or if the tests were considered as submaximal effort.
