The treatment of intercondylar distal femur fractures requires anatomic reduction of intra-articular fragments and absolute fixation. Preoperative planning is necessary to understand fracture morphology. All fracture lines need to be recognized as the primary implant may not capture all articular fragments, mainly when coronal plane fractures are present. Oftentimes, independent interfragmentary compression screws are necessary. No recent studies have visually mapped out the distal femur articular fracture fragments necessary for absolute fixation. The objectives of this study are to determine the frequency of coronal plane fractures in intercondylar distal femur fractures and describe the pattern of intra-articular fracture fragments.
The hospital's trauma registry was queried for distal femur ORIF CPT codes logged in the past four years. A retrospective chart review was performed using the EMR and CT scans. Demographics and mechanisms of injury were analyzed. Fracture fragments were surveyed and drawn ogments of either the medial condyle, lateral condyle, or both condyles. Coronal plane fracture fragments must be identified to obtain absolute fixation. Our study found a higher coronal plane fracture line frequency (52.7%) than prior commonly cited studies. Surgeons must be on the lookout for anterior fracture fragments, posterior fracture fragments, and articular comminution when treating intercondylar femur fractures.
Distal femur intra-articular coronal plane fractures can yield large anterior and posterior condylar fracture fragments of either the medial condyle, lateral condyle, or both condyles. Coronal plane fracture fragments must be identified to obtain absolute fixation. Our study found a higher coronal plane fracture line frequency (52.7%) than prior commonly cited studies. Surgeons must be on the lookout for anterior fracture fragments, posterior fracture fragments, and articular comminution when treating intercondylar femur fractures.Climate change is an urgent public health problem that has looming implications and associated deleterious health consequences. The intersection of climate change and health has broad implications for health professionals in a variety of settings but especially for ED settings. Climate change is already affecting human health and health systems-which includes impacts on ED care. Disaster response and emergency preparedness are critically important public health interventions in our climate-changing world, and the contributions of emergency nurses are essential. Disaster preparedness, environmental emergency response, and health emergency management are important elements of emergency nursing and are explicated in Sheehy's Emergency Nursing Principles and Practices, 7th Edition. The purpose of this article is to present an overview of a clinical tool and mnemonic, A CLIMATE, developed by the authors with application to a case review. It is imperative that the nursing profession-particularly emergency clinicians-address the intersection of climate and health to engage in the assessment, intervention, management, evaluation, education, and referral of those who present to emergency departments with potential climate-related health impacts.Observational studies have shown that gut microbiota-dependent metabolites are associated with the risk of Alzheimer's disease (AD). However, whether such association reflects a causality remains unclear. Apocynin We conducted a bidirectional Mendelian randomization analysis to examine the causal relationships between gut microbiota-dependent metabolites trimethylamine N-oxide (TMAO) or its predecessors and AD. We observed that genetically predicted TMAO (odds ratio 0.99, 95% confidence interval 0.89 to 1.09 per 10 units, p = 0.775) or its predecessors including betaine (1.06, 1.00 to 1.12 per 10 units, p = 0.056), carnitine (1.05, 0.98 to 1.12 per 10 units, p = 0.178), and choline (1.01, 0.92 to 1.10 per 10 units, p = 0.905) were not associated with the risk of AD. Our Mendelian randomization estimates from AD to metabolites showed that genetically predicted higher risk of AD was also not causally associated with TMAO, betaine, carnitine, and choline levels. Our findings support that gut microbiota-dependent metabolites TMAO or its predecessors do not play causal roles in the development of AD.This article investigates a coordinated optimal guaranteed cost control (DCOGCC) for a multi-motor roll-to-roll web machine with time-varying and uncertain parameters. The state-space oriented DCOGCC law is designed to ensure that each subsystem of the web machine is stable and the upper bound of performance index is minimum. Linear matrix inequality (LMI) conditions to guarantee the stability of the systems with DCOGCC are derived. A decentralized model is adopted by regarding the entire machine as a synthetic system, then the original decentralized model is transformed into an equivalent one. By choosing appropriate information of consecutive subsystems as coordination variables, the DCOGCC scheme may attenuate the interaction effects between subsystems. The results of some simulations and experiments are given to verify the feasibility and effectiveness of the DCOGCC.This paper proposes an adaptive neural network based output feedback backstepping fast terminal sliding mode control (NN-BFTSMC) for continuous polymerization reactor with external disturbances and parametric uncertainties. Firstly, neural networks (NN) are employed to approximate the uncertain nonlinear functions. Next, the average molecular weight and the reactor temperature tracking controllers are designed based on the finite-time NN-BFTSMC. In addition, the NN-BFSMC effectively estimates and compensates the upper bounds of the external disturbances. Moreover, the chattering effect is eliminated without losing the robustness accuracy. The finite-time stability of the closed-loop system is proved by Lyapunov theory. At last, numerical simulations and comparative studies are introduced to illustrate the superior performance of the proposed method. In this article, we develop an original high order sliding mode control (SMC) method which can be used to control single input single output (SISO) system and multi-variable system. The method has the benefit of resisting unmodelled dynamics and external disturbance. However, there exist the chattering effect in the conventional control methods. For decreasing the chattering phenomenon, a new high order SMC method is raised. Firstly, the new methodology is designed. Secondly, the method is employed to control the single input single output system with two examples. Thirdly, this new scheme is applied to control the multi-variable system. At the same time, two examples are presented to testify the validity of the raised control method, then the raised new control method has been utilized to the load frequency control. Additionally, in comparison with other recently existed high order control method, the raised control method has demonstrated better control performance in terms of fast convergence and chattering phenomenon.
