art navigation systems do.Approximately 10% of the German population report chronic pain that has at least a moderate influence on everyday life. For instance, patients in Germany often experience an oversupply of diagnostics and therapy when suffering from mild back pain, while the care of patients with severe, chronic pain remains insufficient. Multimodal and interdisciplinary therapies are generally too seldomly used. Potentially preventive procedures are insufficiently investigated and used in current clinical practice and little is known about the mechanisms underlying chronification processes. D-Lin-MC3-DMA In Germany, pain is still largely neglected in current clinical and basic science, as well as research funding. Furthermore, existing projects are often insufficiently connected with each other, thereby missing the unique opportunity of building synergies. To improve medical pain care and to enable participation in activities of daily life with the maximum radius, interdisciplinary clinical programs and cross-linking of basic and clinical research is necessary. The main objectives are to (1) Strengthen basic research to investigate the underlying mechanisms of pain development and maintenance as well as of chronification processes; (2) improve patient care by adequate application of diagnostic, therapeutic, and preventive procedures; (3) consolidate interdisciplinarity in pain research and patient care; (4) maintain participation of patients in social and in everyday life; and (5) achieve and maintain high quality standards in pain patient care. The following article provides an overview of the measures and advances currently being taken in pain research in Germany to close the above-mentioned gaps. The focus lies on current and future developments in the field of pain prevention.
Current research on the treatment of chronic pain in children suggests an increasing trend internationally in the quantity as well as invasiveness of diagnostic and therapeutic interventions. The objective of this research was to examine the interventions received by patients before starting specialized inpatient pain treatment.

Aretrospective survey was conducted analyzing patient files from atertiary children's pain center from 2004, 2008, 2012 and 2016 (N = 585). In addition to diagnostic and therapeutic interventions, pain and patients' characteristics were collected. The identified measures were subsequently evaluated by an interdisciplinary expert panel regarding their invasiveness, potential risk and degree of mental burden.

An increase in diagnostic measures and medication was found up to 2012. Thereafter, adecreasing trend was identified (χ
(3) = 11.708; p = 0.008). Invasiveness (χ
(3) = 13.342; p = 0.004), risk (χ
(3) = 13.135; p = 0.004) and mental burden (χ
(3) = 14.403; p = 0.002) showed the same pattern of change. Patients with abdominal and limb pain are particularly at risk for highly invasive and high risk diagnostics.

Evidence for an increase in diagnostic and therapeutic measures in chronic pain was found up to 2012. Patients presenting with certain complaints receive comparably more invasive, risky and burdensome measures.

Nicotiana tabacum overexpressing CrSAMT from Citrus reticulata increased production of MeSA, which works as an airborne signal in neighboring wild-type plants, inducing PR1 and increasing resistance to the pathogen Xylella fastidiosa. Xylella fastidiosa is one of the major threats to plant health worldwide, affecting yield in many crops. Despite many efforts, the development of highly productive resistant varieties has been challenging. In studying host plant resistance, the S-adenosyl-L-methionine salicylic acid carboxyl methyltransferase gene (SAMT) from Citrus reticulata, a X. fastidiosa resistant species, was upregulated in response to pathogen infection. SAMT is involved with the catalysis and production of methyl salicylate (MeSA), an airborne signal responsible for triggering systemic acquired resistance. Here we used tobacco as a model system and generated transgenic plants overexpressing C. reticulata SAMT (CrSAMT). We performed an in silico structural characterization of CrSAMT and investigated ithowed significantly fewer symptoms when challenged with X. fastidiosa. Altogether, these data suggest that CrSAMT plays a role in host defense response and can be used in biotechnology approaches to confer resistance against X. fastidiosa.
To determine the effectiveness of Vacuum-Assisted Closure Device in the postoperative wound care for Fournier's gangrene METHODS We performed a systematic review in the following databases Medline (Ovid), EMBASE, and The Cochrane Central Register of Controlled Trials (CENTRAL), from inception to nowadays. We included RCTs and analytical observational studies. Meta-analysis was not possible given the clinical and methodological heterogeneity of the studies.

We included six studies that compared VAC treated patients and a control group. The length of stay of the VAC treated vs. the conventional dressing treated patients was higher for the VAC treated patients in all but one of the included studies. The VAC group had the highest number of surgical debridements requiring anesthesia. The conventionally treated patients had a higher number of daily dressings, and the need for additional dressing changes, without anesthesia. Two studies found significantly higher mean scores for VAS, requiring a higher need for daily analgesics for the control group patients.

VAC therapy is an effective method, but it is not better than conventional dressing treatment. VAC carries fewer dressing changes, less pain, and less need for analgesics, but it comes with a higher need for surgical interventions requiring anesthesia.
