Copyright © 2020 Gengenbacher et al.Nacubactam is a novel, β-lactamase inhibitor with dual mechanism of action as an inhibitor of serine β-lactamases (classes A, C, and some class D) and an inhibitor of penicillin binding protein 2 in Enterobacteriaceae The safety, tolerability, and pharmacokinetics of intravenous nacubactam were evaluated in single and multiple ascending dose, placebo-controlled studies. Healthy participants received single ascending doses of nacubactam 50 to 8,000 mg, multiple ascending doses of nacubactam 1,000 to 4,000 mg every 8 hours (q8h) for up to 7 days, or nacubactam 2,000 mg plus meropenem 2,000 mg q8h for 6 days after a 3-day lead-in period. Nacubactam was generally well tolerated, with the most frequently reported adverse events (AEs) being mild-to-moderate complications associated with intravenous access and headache. There was no apparent relationship between drug dose and the pattern, incidence, or severity of AEs. No clinically relevant dose-related trends were observed in laboratory safety test results. No serious AEs, dose-limiting AEs, or deaths were reported. After single or multiple doses, nacubactam pharmacokinetics appeared linear, and exposure increased in an approximately dose-proportional manner across the dose range investigated. Nacubactam was excreted largely unchanged into urine. Co-administration of nacubactam with meropenem did not significantly alter the pharmacokinetics of either drug. We show here that these compounds act synergistically with imipenem and cefoxitin in vitro and increase the bactericidal activity of the β-lactams against M. abscessus In addition, compound 12 also displays synergism with imipenem and cefoxitin within infected macrophages. Copyright © 2020 American Society for Microbiology.Objective To evaluate the pharmacokinetics of amikacin in Mexican patients with different renal function receiving once-daily dosing regimens and the influence of clinical and demographical covariates that may influence in the optimization of this antibiotic.Patients and methods A prospective study was performed in a total of 63 patients with at least one determination of amikacin plasma concentration. Population pharmacokinetic parameters were estimated by nonlinear mixed effects modeling; validations were performed for dosing recommendation purposes based on PK/PD simulations.Results The concentration-versus-time data were best described by a one-compartment open model with proportional interindividual variability associated with amikacin clearance (CL) and volume of distribution (Vd); residual error followed a homoscedastic trend. Creatinine clearance (CLCr) and ideal body weight (IBW) demonstrated significant influence on amikacin CL and Vd, respectively. Final model [CL (L/h) = 7.1 *(CLCr/130)0.84 and Vd (L) =20.3 * (IBW/68)2.9] showed a mean prediction error of 0.11 mg/L (95% CI -3.34, 3.55) in the validation performed in a different group of patients with similar characteristics.Conclusion There is a wide variability in amikacin pharmacokinetic parameters in Mexican patients. This leads to inadequate dosing regimens specially in patients with augmented renal clearance (CLCr > 130 ml/min). Optimization based on final population pharmacokinetic model in Mexican patients may be useful since reliability and clinical applicability have been demonstrated in the current study. Such infections are difficult or impossible to treat, as the enterococcal strains responsible are often resistant to multiple antibiotics. One intrinsic resistance trait that is conserved among E. faecalis and E. faecium is cephalosporin resistance, and prior exposure to cephalosporins is one of the most well-known risk factors for acquisition of an enterococcal infection. Cephalosporins inhibit peptidoglycan biosynthesis by acylating the active-site serine of penicillin-binding proteins (PBPs) to prevent the PBPs from catalyzing crosslinking during peptidoglycan synthesis. For decades a specific PBP (known as Pbp4 or Pbp5) that exhibits low reactivity towards cephalosporins has been thought to be the primary PBP required for cephalosporin resistance. We analyzed other PBPs and report that in both E. faecalis and E. check details faecium, a second PBP, PbpA(2b), is also required for resistance; notably, the cephalosporin ceftriaxone exhibits a lethal effect on the ΔpbpA mutant. Strikingly, PbpA(2b) exhibits low intrinsic reactivity with cephalosporins in vivo and in vitro Unlike the Δpbp5 mutant, the ΔpbpA mutant exhibits a variety of phenotypic defects in growth kinetics, cell wall integrity, and cellular morphology, indicating PbpA(2b) and Pbp5 (4) are not functionally redundant and that PbpA(2b) plays a more central role in peptidoglycan synthesis. Collectively, our results shift current understanding of enterococcal cephalosporin resistance and suggest a model in which PbpA(2b) and Pbp5 (4) cooperate to coordinately mediate peptidoglycan crosslinking in the presence of cephalosporins.
