interstitial lung involvement occurs in 80% of chest CT scans of patients with systemic sclerosis (SS) and has a great impact on morbidity and mortality. The aim of the study was to describe factors associated with the development of interstitial lung disease (ILD) in patients diagnosed with SS in our division.
a retrospective case-control study of patients followed up between 2005-2021 who met the classification criteria for SS was performed. ILD was defined as the finding of interstitial manifestations on high-resolution chest tomography (HRCT) non-specific interstitial pneumonia pattern (NSIP) or usual interstitial pneumonia (UIP), and/or findings on pulmonary function tests (FVC less than 80% and DLCO less than 80%). Patients with ILD (cases) and without it (controls) were identified. Demographic, clinical and serological variables were analyzed. Percentage, mean (SD) and median (IQR) measurements were calculated for each variable. A univariate and multivariate analysis was performed using logistic regression to establish its association with ILD.
Seventy nine patients with SS were included, 31 with ILD. Univariate analysis showed that sclerosis subtype (according to Le Roy classification), lung function measures, and anticentromere antibody positivity were factors associated with ILD in a statistically significant way. In the multivariate analysis, only the presence of anti-centromere antibodies was statistically significant.
the analysis of risk factors to determine the development and progression of lung damage is of vital importance for an early implementation of treatment, which would impact the mortality rate of patients with SS.

Global consumption of vegetables and fruits is currently below recommendations, impacting the prevalence of obesity and non-communicable diseases. In addition to environmental factors, mothers' feeding practices influence children's eating patterns.
Questionnaires were conducted with mothers of children 7 months to 7 years of age (n = 727), in 12 Primary Health Care Centers in the capital cities of three Argentine provinces. Fruit and vegetable consumption was evaluated with the Children's Dietary Questionnaire (CDQ). We tested the association with maternal feeding practices distributed across three domains role modeling, accessibility, and socialization of the products in the home. We tested 11 practices separately and a variable summing the 11 practices.
The majority of children (86%) had low fruit and vegetable intake. In regression models, each of the 11 practices showed a significant association with children's fruit and vegetable intake (beta 0,6-1,8). However, the variable testing the sum of the 11 practices showed a larger association parameter with intake, even in multiple regression (beta 3,9). Proximity of sale points and cost of products also showed a significant association with intake.
Results provide information on home based strategies to promote children's fruit and vegetable intake that do not require complex resources and can be incorporated into community health education programs.

The problem of carbapenemase-producing Enterobacteriaceae (CPE) was exacerbated by the COVID-19 pandemic in countries with a previous high incidence, such as Argentina. This study describes the development and results of a CPE prevention program, mainly carbapenemase-producing Klebsiellas (KPC), in three critical units of two public hospitals during 6 months of the pandemic.
The objective was to reduce the incidence of KPC in clinical and colonization samples. This quasi-experimental study was based on a cycle of improvement and implementation of three measures hand hygiene, environmental hygiene, and periodic surveillance with rectal swabs.
Regarding the measures, all the units optimized active surveillance, and two of these also improved hand and environmental hygiene. Comparing the pre- and post-intervention periods in the three units, no significant change was observed in the rate of KPC positive clinical samples. KPC colonization was significantly reduced in two units (unit 2 51.6-18.5 p 0.0004, unit 3 62.5-5.2 p < 0.0000001). All units showed a downtrend in both rates towards the end of the study.
Containing or reducing the advance of the KPC in our region is possible even in difficult scenarios such as the pandemic. More studies are needed in low- and middle-income countries to demonstrate the impact of KPC prevention programs in these situations.

Achieving glycemic control in type 2 diabetes(T2D) prevents or delays chronic complications and extends survival.
to analyze metabolic control goals in adults with T2DM, treated by specialists in their usual practice in Argentina, and their relationship with adherence to prescribed medications, different treatment strategies, time of diagnosis, quality of life variables and presence of chronic complications of T2DM.
cross-sectional design; multicenter study conducted in 28 centers in Argentina during 2018. The sample was selected randomly and systematically. Morisky-Green-Levine and WHO-5 adherence questionnaire for quality of life was applied. History of disease and laboratory markers were analyzed.
of 1329 DMT2 cases included, 60.2% achieved the goal. Regarding pharmacological treatment in diabetes 5.1% were without medication, 47.5% with 1 drug, 34.5% with 2, 12.9% with 3 or more; in turn, with insulin (alone or combined) 38.1%. According to the questionnaire, 68.4% adhered to treatment and 72.6% rth age = 65 years (p less then 0.0001), higher adherence score (p less then 0.0001), private health insurance (p less then 0.0001), and physical activity (p less then 0.02). The worst metabolic control was associated with time of DMT2 diagnosis (p less then 0.0001), insulin therapy (p less then 0.0001) and symptoms of depression (p less then 0.002) Conclusions The youngest and most vulnerable presented lower values for adherence; intervening and intensifying the treatment earlier in them would allow better results.Fine-needle aspiration (FNA) for the histological diagnosis of occupying lesions in the pancreas as opposed to tru-cut needle biopsy to obtain tissue for analysis has been associated with a lower incidence of post-procedure complications, with almost immediate recovery and no need for hospital stay. MLN8237 mw Nevertheless, the question of the diagnostic effectiveness of percutaneous computed axial tomography (CT)-guided FNA in solid pancreatic lesions has been raised. The aim of this study was to confirm the diagnostic effectivity of percutaneous CT-guided FNA in pancreatic space-occupying lesions and to assess short-term complications. All percutaneous CT-guided FNA with real-time monitoring, performed between April 2010 and December 2015, were retrospectively analyzed. In all cases 21-gauge needles were used. All FNA were performed in the presence of a pathologist who immediately stained and reported as adequate for analysis in all cases. The diagnosis was confirmed by histopathological evaluation. Of 54 FNA performed, final histopathological evaluation revealed neoplastic cells compatible with adenocarcinoma in 52 patients (96%) and was negative for neoplastic cells in two patients (4%). The sensitivity was 94%, and the specificity 100%. Post-FNA morbidity was observed in four patients, consisting of epigastric pain in two and abdominal wall hematoma in two other patients. Percutaneous CT-guided FNA of pancreatic space-occupying lesions was found to be a good, minimally invasive and safe method with low morbidity. The presence of the pathologist in the procedure allowed for immediate cytological diagnosis.Hepatocellular carcinoma is the most common primary liver tumor, with 905 677 diagnosed cases and 830 180 deaths, in 2020 worldwide. In Argentina, it accounts for the 9th cause of death for cancer in men and the 10th in women. Unlike other highly-prevalent tumors, scientific evidence for most therapeutic options is limited mainly to small cohorts and retrospective studies. The aim of this study is to characterize and describe epidemiologically patients with diagnosis of hepatocellular carcinoma in the Italian Hospital of Buenos Aires during a 12-year period. Overall survival for our cohort was 58%, 46%, and 36% at 1, 3 and 5 years respectively. Average survival for patients receiving palliative treatment was 5 months, while for those who received either non-curative or curative treatment was 23 and 75 months respectively. Recurrence-free survival for those patients who underwent a curative treatment was 89%, 76% y 61% at 1, 3 and 5 years. A thorough analysis of etiology, risk factors, incidence, mortality and treatment was made. The study's importance lies in its large sample size, quantity and quality of data, and will most certainly stimulate the development of local studies in hepatocellular carcinoma.
COVID-19 develops severe inflammatory responses that can lead to death. It is essential in a pandemic to have accessible instruments to estimate the prognosis of the disease. The lymphocyte-to-C-reactive protein ratio (LCR) is a predictive biomarker studied in oncology, which could have some advantages in COVID-19 patients in the early stages of the disease. Our objective was to estimate the risk of LCR < 100 and mortality in hospitalized patients with COVID-19.
hospitalized patients with COVID-19 seen between March to October 2020 were included. The patients were grouped according to LCR < 100 and LCR > 100. A Cox regression model was performed to estimate the association between LCR < 100 and mortality.
we included 730 patients with COVID-19. The mean age at diagnosis was 49.9 years (SD 16.8) and 401 (55%) were men. Cox regression model showed an association between LCR <100 and mortality (HR 6.2; 95% CI 1.6 to 23.5; p 0.008), adjusting by age. severe pneumonia, intensive care requirements, and comorbidities.
