A 58-year-old man presented with entrapment of the flexor hallucis longus (FHL) tendon between fractured fragments of the talar body. The patient was successfully treated with atypical operative approaches to release the tendon and reduce the fracture. At the 1-year follow-up, the patient remained asymptomatic with the ability to extend and flex of all toes. This case illustrates a unique surgical approach for this presentation.

An osteoporotic and kyphotic 72-year-old woman presented with sternal pain, severe chest instability, and shortness of breath secondary to a traumatic sternal nonunion resulting from an unhealed sternal fracture sustained in a motor vehicle accident several months earlier. In this study, a meta-analysis was performed to evaluate the clinicopathologic and prognostic value of SNHG7 in cancers. Electronic databases of PubMed, Web of Science, Cochrane Library and Embase were used to search relevant studies. A combined hazard ratio (HR) and its corresponding 95% confidence interval (CI) were used to assess the association between SNHG7 expression and prognosis in cancer patients. Pooled odds ratio (OR) and 95% CI were calculated to elaborate the association between SNHG7 expression and clinicopathological features in cancers. Besides, the data from The Cancer Genome Atlas (TCGA) dataset was used to validate the results. In total, eighteen studies compromising 1303 participants were enrolled in this analysis. The pooled results showed increased SNHG7 expression could predict unfavorable overall survival (OS) (HR = 1.75, 95%CI = 1.52-2.02, P = 0.000). Analysis stratified by follow-up time, cancer types, analysis types, sample sizes and cut off further verified the prognostic value of SNHG7. Additionally, elevated SNHG7 expression was correlated with TNM stage (OR 3.31, 95%CI = 2.29-4.80, P = 0.000), lymph node metastasis (OR = 3.32, 95%CI = 1.61-6.83, P = 0.004), and tumor differentiation (OR = 1.92, 95%CI = 1.22-3.03, P =0.005) in patients with cancers. Excavation of TCGA dataset valuated that SNHG7 was upregulated in some cancers and predicted worse OS, which partially confirmed our results in this meta-analysis.
Homozygous and heterozygous variants in PPP2R3C are associated with syndromic 46,XY complete gonadal dysgenesis (MEGD syndrome), and impaired spermatogenesis, respectively. This study expands the role of PPP2R3C in the aetiology of gonadal dysgenesis (GD).

We sequenced the PPP2R3C gene in four new patients from three unrelated families. The clinical, laboratory and molecular characteristics were investigated. We have also determined the requirement for Ppp2r3c in mice (C57BL6/N) using CRISPR/Cas9 genome editing.

A homozygous c.578T>C (p.L193S) PPP2R3C variant was identified in one 46,XX girl with primary gonadal insufficiency, 2 girls with 46,XY complete GD, and one undervirilized boy with 46,XY partial GD. The patients with complete GD had low gonadal and adrenal androgens, low AMH, and high FSH and LH concentrations. All patients manifested characteristic features of MEGD syndrome. link2 Heterozygous Ppp2r3c knockout mice appeared overtly normal and fertile. Inspection of homozygous embryos at 14.5, 9.5 and 8.5 days post coitum revealed evidence of dead embryos. We conclude that loss of function of Ppp2r3c is not compatible with viability in mice and results in embryonic death from 7.5 dpc or earlier.

Oral solution and tablet formulations of Levo-Thyroxine (L-T4) are both used in the treatment of Congenital Hypothyroidism (CH). However, few studies and with a limited follow-up period have been published comparing these two formulations in children.

The aim of this multicenter study was to compare the effectiveness of L-T4 oral solution (with ethanol as excipient) and tablet formulation in children with CH up to 3 years of age.

Children diagnosed with CH between 2006 and 2015 were enrolled and divided into two groups according to the L-T4 formulation used solution in drops (Group D) or tablets (Group T). Auxological parameters, TSH and FT4 values and L-T4 dose were collected at diagnosis and at 15 days, 1, 3, 6, 12, 24 and 36 months of treatment. The Developmental Quotient at 1 and 3 years of age was evaluated using Griffiths' Scale.

254 children were enrolled. 117 were treated with solution and 137 with tablets. Auxological parameters, dose and thyroid function values at diagnosis, 3, 6, 12, 24, 36en starting and adjusting the dose. No negative effects in cognitive development were observed. The data obtained are encouraging but long-term follow-up is needed.Context Women with polycystic ovary syndrome (PCOS) have an increased risk of metabolic syndrome (MetS). Both PCOS and MetS are associated with excess weight. Objective To examine the effect of a three-component lifestyle intervention (LSI) with or without short message service (SMS+ or SMS- respectively) on the prevalence and severity of MetS and metabolic parameters, compared to care as usual (CAU). Design Randomized controlled trial. Methods Women diagnosed with PCOS and a BMI >25 kg/m² (n = 183) were either assigned to a one-year three-component (cognitive behavioural therapy, diet, exercise) LSI, with or without SMS support, or to CAU which provided weight loss advice only. Main outcome measures included changes in the prevalence of MetS, the continuous MetS severity z-score (cMetS z-score), metabolic parameters, and the impact of weight loss. Results After one year the decrease in the cMetS z-score was greater in the SMS+ group than the CAU group (-0.39, p=0.015). The prevalence of MetS changed with -21.6% (p=0.037), -16.5% (p=0.190) and +7.0% (p=0.509) in both LSI groups and CAU group, respectively. A post hoc analysis for both LSI groups combined vs CAU resulted in a MetS difference of -25.9% (p=0.046). Moreover, weight loss per se resulted in significant favourable effects on all metabolic parameters. Conclusions This three-component lifestyle intervention was more successful in improving metabolic health compared to CAU. Therefore we recommend this intervention to women with PCOS and excess weight, provided that a clinically relevant weight loss is being pursued.
To evaluate intradiskal pressure (IDP) in the C6-7 intervertebral disk (IVD) after destabilization and distraction-fusion of the C5-C6 vertebrae.

7 cadaveric C4-T1 vertebral specimens with no evidence of IVD disease from large-breed dogs.

Specimens were mounted in a custom-made 6 degrees of freedom spinal loading simulator so the C5-C6 and C6-C7 segments remained mobile. One specimen remained untreated and was used to assess the repeatability of the IDP measurement protocol. link3 Six specimens underwent 3 sequential configurations (untreated, partial diskectomy of the C5-6 IVD, and distraction-fusion of the C5-C6 vertebrae). Each construct was biomechanically tested under neutral, flexion, extension, and right-lateral bending loads. The IDP was measured with a pressure transducer inserted into the C6-7 IVD and compared between the nucleus pulposus and annulus fibrosus and across all 3 constructs and 4 loads.

Compared with untreated constructs, partial diskectomy and distraction-fusion of C5-C6 decreased the mean ± SD IDP in the C6-7 IVD by 1.3 ± 1.3% and 0.8 ± 1.3%, respectively. During motion, the IDP remained fairly constant in the annulus fibrosus and increased by 3.8 ± 3.0% in the nucleus pulposus. The increase in IDP within the nucleus pulposus was numerically greatest during flexion but did not differ significantly among loading conditions.

To characterize the anatomic location of the esophageal ostium relative to the rima glottidis in adult Labrador Retrievers with the use of CT.

A search of the medical records database identified records of Labrador Retrievers that underwent CT of the head and neck between January 1, 2015, and December 31, 2018. Evaluators, blinded to each other's results, reviewed CT images and measured esophageal area at the level of the rima glottidis. For each dog, the left esophageal percentage (LEP) was calculated as the esophageal area left of the rima glottidis midline divided by the overall esophageal area at that level. Variables (age, sex, patient position, intubation status, and maxillary support during CT) were evaluated for association with LEP. The CT images of dogs that had multiple scans were assessed for within-patient variance.
