Physicians, pharmacists, medical students, and pharmacy students were asked to participate in the e-learninsroom of Kampo learning.Introduction Subthalamic nucleus (STN) deep brain stimulation (DBS) is an important option in the treatment of motor symptoms and fluctuations in patients with advanced Parkinson's disease (PD). In addition to the improvement in motor symptoms, many studies have reported changes in some non-motor symptoms (NMS) after STN DBS.Method 61 patients (42 males) who underwent STN DBS with advanced PD and 24 healthy controls (15 males) were included in the study. Autonomic symptoms (orthostatic hypotension, sweating, salivation) were assessed with a semi-structured questionnaire. Sympathetic skin responses (SSR) were studied by electrophysiological examination within 3-6 months after STN DBS.Results SSR latency and amplitude were found between the control group and preoperative patients (P ≤ 0.01; p = 0.01, respectively), and between preoperative and postoperative patients (P ≤ 0.01; P ≤ 0.01, respectively). There was a statistically significant difference between the control group and postoperative patients (p = 0.005; p = 0.029, respectively). Orthostatic hypotension (29%) and sweating (48%) improved, but there was no change in the salivation.Discussion We think that STN DBS applied in PD has not only motor symptoms and fluctuations, but also corrects autonomic dysfunctions such as sweating disorders and orthostatic hypotension; SSR is more sensitive and reliable in detecting autonomic dysfunction, independent of motor symptoms and fluctuations.To assess the prevalence of diabetic retinopathy (DR) among persons with diabetes and prediabetes participating in the 2018 Population Health Survey in Greenland (B2018), a follow-up survey of three previous health surveys. Participants were invited to a diabetes complication screening. We assessed the prevalence of DR using Optos Daytona Ultra-wide field fundus camera and assessed differences in prevalence according to demographic and clinical characteristics using chi square test and a t-test and assessed DR based on ethnicity. The overall prevalence of DR was 2% (10/483). Among participants with HbA1c ≥48 mmol/mol (6.5%) DR prevalence was 9% (9/91), compared with less then 1% (1/382) among participants with HbA1c less then 48 mmol/mol (6.5%). All participants with DR lived in towns. The mean Inuit genetic admixture was lower among participants with DR. The prevalence of DR is low in Greenland and almost non-existent among persons with HbA1c below the diabetes threshold.Key areas of sports science research investigate the functional role of muscle activations within human movement. Even within relatively constrained movements like cycling, significant variability is observed in muscle activation strategies. Particular attention has been given to particular muscles, despite Soleus and Tibialis anterior muscles presenting a potentially functionally relevant split between monomodal and bimodal activation strategies. The current study (N = 54) investigated the prevalence and functional implications of these different strategies and identified, in addition to monomodal [Soleus N = 24, Tibialis anterior N = 7] and bimodal [Soleus N = 12, Tibialis anterior N = 31] strategies, a third group switching between strategies [Soleus N = 16, Tibialis anterior N = 13]. The combined Soleus group showed significantly higher Index of Force Effectiveness, lower negative work and lower radial forces than the bimodal group. Furthermore, bimodal Soleus strategies produced a period of significantly greater plantar flexion during the upstroke. No differences were found between the Tibialis anterior groups. These data show an identifiable group of cyclists utilising a combination of monomodal and bimodal strategies potentially benefiting mechanical effectiveness. Awareness of such functional implications can aid researchers and practitioners when interpreting cycling biomechanics data or intervention responses. Fourteen softball pitchers (17.9 ± 2.3 years, 166.4 ± 8.7 cm, 72.2 ± 12.6 kg) completed baseline isokinetic strength assessment for knee, hip, trunk and pitching elbow flexion and extension as well as trunk rotation. Seven days later, participants pitched a simulated game consisting of 105 fastballs prior to repeating all strength assessments. Changes in muscular strength were assessed using paired samples t-tests, with significance set a priori as p ≤ 0.05. Normalised (%BW) stride leg knee extension peak torque was significantly higher (p = 0.020) post-simulated game (75.1 ± 24.6%BW) as compared to baseline (64.0 ± 19.5%BW) and trunk flexion peak torque was significantly higher (p = 0.009) post-simulated game (84.8 ± 47.0%BW) as compared to baseline (63.5 ± 47.1%BW). This study showed an increase in knee extension and trunk flexion strength after an acute bout of pitching. The findings give insight into muscular changes following pitching which can assist in appropriate softball training and recovery.
Gut dysbiosis may be implicated in the pathogenesis of IgA nephropathy (IgAN) through immune and/or metabolite pathways. Fecal microbiota transplantation (FMT) could reestablish the micro-ecological balance in IgAN, although this has never been attempted before. We explored whether FMT could be efficacious in treating IgAN in two patients with refractory IgAN.

Two Chinese female patients with IgAN failed to achieve clinical remission after receiving several rounds of immunosuppressive therapy and suffered from unbearable adverse effects due to immunosuppressants. Both patients received intensive fresh FMT conducted through transendoscopic enteral tubing (TET) regularly for 6-7 months, and were followed up for a further 6 months. Partial clinical remission was achieved in both patients, evidenced by a decrease in the 24-h urinary protein (24-hUP) to less than half of baseline during FMT treatment or follow-up, along with increased serum albumin (sAlb) and stable kidney function. The gut microbiota of both patients was distorted with lower biodiversity and altered composition, which was reversed following FMT. Phylum

increased during and after FMT. The intensive fresh FMT was well-tolerated, and no severe adverse events occurred.

Therefore, it is important to determine the effect of different topical agents on corneal wound healing response.Background The aim was to compare the effects of three different eye drops on corneal epithelial wound healing in an experimental model.Methods Twenty-four eyes of 24 female BALB/c mice were included. A 2 mm central corneal epithelial defect was created. Topical Coenzyme Q10 + Vitamin E D-α-TPGS 4 × 1 was applied to Group A (n = 6), topical Sodium hyaluronate + Xanthan Gum + 0.3% Nethylmicine 4 × 1 to Group B (n = 6) and topical Sodium hyaluronate 4 × 1 to Group C (n = 6). Group D (n = 6) was the control group without treatment. Clinical scoring according to corneal fluorescein staining and histopathological evaluations was performed.Results Clinical scores according to corneal fluorescein staining were similar in all groups on days 1 (p = 0.05), 2 (p = 0.15) and 3 (p = 0.62). Electron microscopy revealed disruption of intercellular junctions between corneal epithelial cells and intracellular vacuole formation in all groups except Group A. Corneal epithelial thickness and superficial epithelial microvillus arrangement were close to normal in Group A.Conclusion Although there was no difference in clinical scores between groups, electron microscopy revealed a better organised epithelium with normal configuration of microvilli and less vacuolisation in Group A.Despite recognition that farming has substantial economic uncertainty, few recent studies have investigated how economic stress may impact the family dynamic among family farmers. This study sought to address this gap in the literature by examining both personal and global economic stress as a predictor of family distress and work-family conflict among a sample of farm couples. Participating in the study were 217 husband-wife dyads (434 individuals) in which farming was the primary occupation of the husband. A cross-sectional survey design was used to assess economic stress, family distress, and work-family conflict. Spouses completed separate online surveys to ensure independent responding. Both individual (spillover) and dyadic (crossover) effects were examined. Results demonstrated farmers' personal economic stress was positively related to individual and spousal family distress. Furthermore, family distress was a mediator of the relationship between personal economic stress and work-family conflict for both farmers and their spouses. In sum, economic stress is an important variable to consider when examining the work-family interface of farm couples. Moreover, personal economic stress had a larger impact on family distress and work-family conflict than global economic stress. By providing a better understanding of the family mechanisms by which economic stress may impact farm couples, it is hoped the current results can be used to assist in the development and application of mental health resources for farm families.The aim of this review is to discuss sexual dimorphism of energy metabolism, and to describe the impact of women's hormonal status on substrate oxidation during exercise. Many evidences indicate that sex steroids play a pivotal role in the sex-related differences of body composition and energy substrate storage. Compared with men, women rely more on fat and less on carbohydrates at the same relative exercise intensity. Scientific data suggest that 17-β oestradiol is a key hormone for the regulation of body composition and substrate metabolism. However, in women, measurements with stable isotopic tracers did not highlight any difference in whole-body substrate oxidation rates between the follicular and luteal phases of the menstrual cycle during endurance exercise. The remaining discrepancies about the effect of menstrual cycle-related hormone fluctuations on substrate oxidation could be partly explained by the exercise intensity, which is an important regulator of substrate oxidation. Due to their specific nature and concentration, the synthetic ovarian hormones contained in oral contraceptives also influence substrate metabolism during endurance exercise.
