Succinylcholine is a preferred muscle relaxant for rapid sequence intubation. Postoperative myalgia (POM) is one of its commonest adverse effects with unknown pathogenesis. Various modalities were examined to reduce POM. We hypothesized that duloxetine may reduce the incidence and severity of fasciculation or succinylcholine-induced POM in outpatient surgeries.
This randomized double-blinded trial involved controlled 70 adult participants scheduled for elective direct microlaryngoscopic surgeries. NVP-AUY922 Before induction of general anesthesia by 2h, 35 patients received duloxetine 30mg orally (group D) and 35 patients received similar oral starch placebo capsules (group C). Fasciculations, POM, sedation score, time to first rescue analgesia, total analgesic consumption 24h after surgery, patients' satisfaction, and adverse effects were recorded.
Incidence of fasciculation was 77.1% and 94.3% in groups D and C (p value = 0.04), whereas its severity was not significant between groups (p value = 0.09). Incidence effective in reducing postoperative rescue analgesic requirement with better patients' satisfaction and no serious adverse effects.Sulfur and selenium containing sugars have gained prominence in the last two decades because of their importance in several biological applications. These type of carbohydrate scaffolds are also challenging targets for synthesis. In this personal note, we have summarised the results of our investigation over the last 20 years on the use of two reagents, benzyltriethylammonium tetrathiomolybdate and tetraethylammonium tetraselenotungstate, in efficient transfer of sulfur and selenium respectively to the synthesis of a number of carbohydrate derivatives.
Neuropathic pain (NP) is a complex condition that impairs the patients' quality of life. Registries are useful tools, increasingly used as they provide high-quality data. This article aims to describe the Greek Neuropathic Pain Registry (Gr.NP.R.) design, the patients' baseline data, and real-world treatment outcomes.
The Gr.NP.R. collects electronically, stores, and shares real-world clinical data from Pain and Palliative Care centers in Greece. It is a web-based application, which ensures security, simplicity, and transparency. VAS, DN4, and Pain Detect were used for pain and NP assessment.
From 2016 to 2020, 5980 patients with chronic pain, of cancer or non-cancer origin, were examined and 2334 fulfilled the NP inclusion criteria (VAS >5, DN4 >4, and Pain Detect ≥19). At the first visit, the mean age was 64.8years, 65.5% were female patients, and 97.9% were Greek. The mean (SD) time from pain initiation to visiting the pain clinics was 1.5 (3.8) years. Most patients were undertreated. Following the patients' registration, the national guidelines were implemented. The majority of the prescribed medications were gabapentinoids (70.2%), especially pregabalin (62.6%), and opioids (tramadol, 55.3%). At visits 1 and 6, mean VAS was 7.1 and 5, and mean DN4 score was 5.6 and 3.5, respectively.
The Gr.NP.R. provides information on the demographics, clinical progress, treatment history, treatment responses, and the drugs of choice for patients with cancer and non-cancer NP. The collected data may help physicians plan the management of their patients.

In this review, we analysed 80 papers reporting the effects of volcanic eruptions in freshwater environments and on freshwater organisms. An increase in water turbidity is the most common reported physical effect while increases in concentrations of inorganic elements, many representing nutrients for primary producers, are the most common chemical effects. Bacterial growth is usually stimulated, while autotrophs can be either positively or negatively affected depending on the type of impact. A persistent effect reported in the biota is changes to the assemblage, which could generate further changes in terms of ecosystem functions. This analysis also identifies some information gaps, particularly involving the effects of eruptions on heterotrophic biofilms in streams and on invertebrates and fish in lakes. Most studies were carried out soon after the volcanic eruption, so it is difficult to assess the recovery of the ecosystems. Eruptions present unique opportunities for scientific discovery, although such studies are often hindered by a lack of pre-eruption data, which would allow for a more comprehensive assessment of the effects.Heparan sulfate (HS) is a complex, polyanionic polysaccharide ubiquitously expressed on cell surfaces and in the extracellular matrix. HS interacts with numerous proteins to mediate a vast array of biological and pathological processes. Inhibition of HS-protein interactions is thus an attractive approach for new therapeutic development for cancer and infectious diseases, including COVID-19; however, synthesis of well-defined native HS oligosaccharides remains challenging. This has aroused significant interest in the development of HS mimetics which are more synthetically tractable and have fewer side effects, such as undesired anticoagulant activity. This account provides a perspective on the design and synthesis of different classes of HS mimetics with useful properties, and the development of various assays and molecular modelling tools to progress our understanding of their interactions with HS-binding proteins.
Paranoia describes unfounded and distressing interpersonal threat beliefs. Secure attachment imagery has been shown to attenuate paranoia and anxiety in non-clinical and clinical groups, but little is known about the differential effects of anxious and avoidant imagery or mechanisms of change. In this study, we tested the impact of secure, anxious, and avoidant attachment imagery on paranoia, anxiety, and help-seeking intentions. We also examined hypothesized mechanisms of change, specifically whether cognitive fusion and negative self- and other-beliefs mediate these relationships.
This study utilized an experimental, cross-sectional design.
A large (N=303), international general population sample with high levels of non-clinical paranoia completed a series of measures before and after engaging in secure, anxious, or avoidant imagery.
Relative to anxious and avoidant attachment imagery, secure attachment imagery reduced paranoia and anxiety and increased help-seeking intentions. Cognitive fusion and negative self- and other-beliefs mediated the impact of attachment imagery on paranoia and anxiety, but not help-seeking.
