outweigh the disadvantages of SBCS when compared with 'delayed bilateral cataract surgery' (DBCS). SBCS is favored over DBCS in pediatric population and in uncooperative, mentally retarded and physically disabled adults needing general anesthesia to reduce the risks and complications of general anesthesia. Considering such factors and review of available literature strongly support that SBCS has a definite role where indicated, under certain circumstances, and in certain select group of patients in both developing as well as developed countries. For many decades only a few formulas have been available to calculate the intraocular lens (IOL) power for patients undergoing cataract surgery the Haigis, Hoffer Q, Holladay 1 and 2 and SRK/T. In recent years, several new formulas for IOL power calculation have been introduced with the aim of improving the accuracy of refraction prediction in eyes undergoing cataract surgery. Entinostat molecular weight These include the Barrett Universal II, the Emmetropia Verifying Optical (EVO), the Kane, the Næser 2, the Olsen, the Panacea, the Pearl DGS, the Radial Basis Function (RBF), the T2 and the VRF formulas. Although most of them are unpublished so that their structure is unknown, we give an overview of each formula and report the results of the studies that have compared them. Their performance in short and long eyes is provided and a special focus is given on the issue of segmented axial length, which is a promising method to obtain more accurate outcomes in short and long eyes. Here, the group refractive index originally developed for the IOLMaster may not represent the best method to convert the optical path length into a physical distance. The issue of posterior and total corneal astigmatism (TCA) is discussed in relation to toric IOLs; the latest formulas for toric IOLs and their results are also reported.Cataracts, the leading cause of vision impairment worldwide, arise from abnormal aggregation of lens proteins. According to the World Health Organization, cataracts cause more than 40% of blindness cases. As the population ages, the prevalence of cataracts will increase rapidly. Although cataract surgery is regarded as effective, it still suffers from complications and high cost, and could not meet the increasingly surgery demand. Therefore, pharmacological treatment for cataracts is a cheaper and more readily available option for patients, which is also a hot topic for years. Anti-cataract drug screening was previously mainly based on the specific pathogenic factors oxidative stress, excess of quinoid substances, and aldose reductase (AR) activation. And several anti-cataract drugs have been applied in the clinic, while the effect is still unsatisfied. Makley and Zhao recently identified two kinds of novel pharmacological substances (25-hydroxycholesterol, lanosterol) that can reverse lens opacity by dissolving the aggregation of crystallin proteins, indicating that protein aggregation is not an endpoint and could be reversed with specific small-molecule drugs, significantly boosting the development of the cataract pharmacopeia and being regarded as a new dawn for cataract treatment. Our team built a novel optimized platform and had screened several potential therapeutic agents from a collection of lanosterol derivatives. In this review, we would mainly focus on the advancement of cataract pharmacotherapy based on the targets for anti-cataract drugs.Where and when cataract surgery started have been a mystery. Indian tradition and the Persian author Zarrin-Dast attributed the procedure to the Indians, while pseudo-Galen suggested an Egyptian origin. Certain idiosyncratic practices are common to early Greek and Sanskrit descriptions of cataract couching, e.g., the requirement for maturity of the cataract, the preference for patients of intermediate ages, comparison of some eyes to glass, rubbing the eye, having a wide portion of the couching instrument shaft, pars-plana puncture with avoidance of the vein, and immediate vision testing. In ancient Greece and India, the words describing the color of a healthy blue eye (glaukos and nīla, respectively) could also characterize a poorly-seeing eye not curable by surgery. In both regions, the lens (or pupillary region) was compared to a lentil, and colored entoptic phenomena were noted. The sitting posture of the patient, ocular convergence towards the nose, the more systematized integration of the humoral theory with cataract surgery, and possibly blowing on the eye and putting cotton on the eye are all consistent with an Indian origin for the procedure. On the other hand, the emphasis on surgical ambidexterity could suggest an origin close to the Mediterranean. Thus, the question of where cataract surgery started has not been resolved. Various authors have suggested that multiple types of cataract surgery were practiced in the ancient and medieval periods (I) couching, (II) discission (division), (III) aspiration through a tube, (IV) extraction through a limbal incision, and (V) expulsion of lens remnants around an embedded probe. We review the evidence in favor (and against) each of these types of surgery.Cataract extraction has become a much safer procedure with the development of phacoemulsification, extending its application in ocular conditions such as glaucoma for better disease control. This review aims at summarizing the effect of lens extraction with or without combined glaucoma surgeries in different types of glaucoma and describing the intraoperative techniques and changes of glaucoma care postoperatively. A comprehensive literature search was performed through Medline and PubMed, and 67 studies were selected for this review. In primary angle closure (PAC) diseases, studies have revealed significant intraocular pressure (IOP) and medication requirement reduction after lens extraction. Fewer studies described its application in primary open angle glaucoma (POAG) and ocular hypertension (OHT), but literature available suggests that it can also lead to better disease control. Likewise, lower postoperative IOP and pressure fluctuations have been shown in normal tension glaucoma (NTG). Advanced glaucoma, shallow anterior chamber and pseudoexfoliation glaucoma (PXG) are three difficult scenarios that are commonly encountered in cataract operations.
