The aim Determine the changes in bone density that occur in the cortical layer of the tibia in patients with CPT after surgical treatment due to radiography data.
Materials and methods The study was performed on X-rays of the tibia of three patients with CPT, aged 5 to 7 years. X-rays were taken before surgery and 1 year after surgery. The optical density of the cortical layer of both tibia bones was determined, for which they were conditionally divided into 7 zones.
Results In all patients with CPT before treatment, there is significant decrease in the optical density of cortical bone tissue of the damaged tibia compared with contralateral. One year after surgery, there was an increase in the optical density of the cortical layer of both tibia of the studied patients. The increase in the density of the nonoperated tibia can be attributed to the active growth of patients. On the operated limb, the increase in the density of the cortical layer became statistically significant in all studied areas, which can be attributed not only to the growth of patients, but also to the possibility of active loading of the operated limb.
Conclusions In all patients with CPT before treatment, a statistically significant decrease in the density of cortical bone tissue of the damaged tibia was observed. As a result of the treatment there is an approximation of the bone density of the operated limb to the indicators of intact contralateral bone.

The aim To establish the clinical and pathogenetic role of glucocorticoid imbalance as an important link of impairment of the adaptive system homeostasis and to determine ways of its correction as a way to increase the effectiveness of the pulmonary tuberculosis treatment.
Materials and methods The effectiveness of glucocorticoids in the pulmonary tuberculosis treatment was studied in 304 patients, of which 134 patients (group 1) received only antibacterial therapy, 67 patients (group 2) were supplemented with glucocorticoids (20-30 mg of prednisolone, daily, in 3 doses, with their cessation by gradual reduction of dose). 103 patients (group 3) also received antibacterial therapy in combination with glucocorticoids (20-30 mg of prednisolone, in the morning, in a daily dose every other day, cessation was carried out simultaneously without reducing the dose).
Results The study found that the level of cortisol in patients with pulmonary tuberculosis was significantly higher than normal, did not have age and gender dependence, but showed a connection with the severity of clinical forms of tuberculosis, the duration of disease development and the presence of intoxication. buy CC-99677 It was established that the daily fluctuations of cortisol persist in tuberculosis patients, but its level significantly exceeds the norm, which indicates the functional stressing of the adaptive system.
Conclusions The article substantiates the homeostatic role of glucocorticoids in the complex treatment of tuberculosis patients when administered in a double physiological dose every other day, taking into account the daily biorhythm of the hypothalamic-pituitary-adrenal axis function.

The aim To analyze the features of changes in the functional state of the vascular endothelium of handball players in the dynamics of the training process, at different levels of the body's hypoxic state.
Materials and methods Theoretical methods, the method of Corretti et al. with the use of high-resolution ultrasound, Fisher test with the calculation of the Fisher criterion and the Bland-Altman method. The study of the vasomotor function of the vascular endothelium was carried out of young men 18-20 y.o., who did not go in for sports and which were systematically played handball. The brachial artery diameter, maximum linear blood flow velocity, volumetric blood flow velocity were registered in the state of relative rest after artificially created reactive hyperemia.
Results The primary results obtained showed that in the process of long-term adaptation to systematic muscular work, a pronounced vasodilation effect was observed. Subsequent analyze of changes in the functional state of the vascular endothelium of young sportsmen during the macrocycle preparation different levels of the body's hypoxic state manifested the following. The young men-athletes had more pronounced vasodilation effect, the values of the linear and volumetric blood flow velocity both in the state of relative rest and at the peak of the artificially created hyperemia were significantly higher than in the young men, who did not go in for sports.
Conclusions Suggested that the systematic muscular work contributes to a significant intensification of the oxidation pathway of nitric oxide formation from L-arginine with the participation of endothelial NO-synthase.

The aim To study the survival rate and count the number of complications in groups with NCRT, NCT and NCT + adjuvant radiotherapy.
Materials and methods The studied patients were divided into three groups with a total number of 304 people, Patients who received neoadjuvant chemotherapy-149 people, who received neoadjuvant chemotherapy and adjuvant radiation therapy-90 people. Neoadjuvant chemoradiotherapy was performed in accordance with Cisplatin/Docetaxel or Carboplatin/Paclitaxel 4 Mg and simultaneous radiation therapy with a total radiation dose 30 gray with a single dose of 2 Mg. After neoadjuvant treatment, patients of all groups underwent radical surgery. It consisted of lobectomy, bilobectomy and pneumonectomy. Postoperative complications and the duration of stay in the clinic after surgery were studied. Survival was assessed on the Kaplan and Mayer scale.
Results The study analyzed the main results of treatment, which showed that the 5 years survival in the main group was 28.1±5.9 %, in the control groups-the first control group (neoadjuvant chemotherapy) - 10.4±3.8 % and the second control group (neoadjuvant chemotherapy and adjuvant radiation therapy) - 5.8±2.0 %, respectively.
Conclusions 1. The overall survival rate of patients receiving NCT compared to patients receiving neoadjuvant chemotherapy is higher than in the neoadjuvant chemotherapy group and 13 months higher than in the adjuvant radiation therapy group. 2. Patients who had the phenomenon of a complete morphological response have a significantly higher survival rate (45 and 39 months, respectively).

The aim The purpose of the study is to improve the results of treatment of patients with acute intestinal obstruction of tumor origin by developing individualized surgical tactics considering the level of cryoglobulins.
Materials and methods 96 patients with ileus of tumor origin were studied. The mean age of patients was 54.7 ± 5.9 years. 30 patients were diagnosed with colorectal cancer, 35 patients - with sigmoid cancer, 13 patients - with cecum and ascending colon, 11 patients - with transverse colon cancer, and 7 patients with descending colon cancer. Isolation of cryoglobulins from blood serum was performed by the method of A. E. Kalovidoris with modifications. The content of Ig A, Ig M, Ig G, total Ig E in the serum was investigated using enzyme-linked immunosorbent assay systems "Granum-Ukraine", the content of allergen-specific Ig E was investigated using enzyme-linked immunosorbent assay systems produced by "Microgen".
Results As a result of treatment of 96 patients, it was found that the level of development of postoperative purulent complications was significantly influenced by the level of cryoglobulinemia and the volume of surgery (CMU, p <0.05). It was found that in patients with decompensated intestinal obstruction, the initial concentration of cryoglobulins was 16.4% higher than in the group with compensated intestinal obstruction (CMU, p <0,05).
Conclusions Determination of cryoglobulinemia on admission of patients with acute obstructive ileus of tumor origin is a simple and effective method for predicting the development of purulent-inflammatory complications in the postoperative period and can influence the choice of treatment tactics.

The aim To explore the possibilities of using the WHODAS 2.0 questionnaire in the field of medical and social expertise in Ukraine as an additional tool for the determination of the levels of human functioning in the main spheres of life.
Materials and methods A check-up of 125 disabled people was conducted, their scores were determined using the WHODAS as well as their degree of disability according to generally accepted criteria in Ukraine. The study object was the disability level. The study subject - levels of functioning and disability assessment with the WHODAS 2.0.
Results It is shown that most disabilities occur in the spheres of participation, life activity, and mobility, which is the biggest obstacle to the functioning of such people. An interval gradation of disability level determination is proposed based on the scores obtained by the respondent according to the WHODAS 2.0 with the selection of criteria for mild, moderate, severe, and very severe functional limitations.
Conclusions Correlation analysis of indicators in different sections of the WHODAS 2.0 questionnaire with the disability criteria used in the practice of medical and social examination proves that the disability level assessment according to the WHODAS 2.0 meets the generally accepted criteria, therefore, this disability scale can be an objective tool for functional level assessment and disability group determination.
