Treatments are associated with high rates of relapse, and thus there exists an unmet medical need for new, improved therapies. Mogamulizumab is a novel defucosylated monoclonal antibody targeting C-C chemokine receptor 4 that eradicates malignant cells via antibody-dependent cellular cytotoxicity (ADCC).Areas covered Phase I-III clinical trials involving mogamulizumab demonstrated its significant efficacy and tolerability. In the phase III MAVORIC study, mogamulizumab exhibited greater efficacy than vorinostat (response rate, 28% versus 5%; median progression-free survival, 7.7 versus 3.1 months, respectively) for advanced, relapsed/refractory MF/SS. Responses were durable (median, 14.1 months), and response rates were highest in SS (37%) and within the blood compartment (68%). Common adverse events include infusion reactions and drug eruptions. The drug also increases the risk of immune-mediated complications such as autoimmune disease and acute graft-versus-host disease following transplantation.Expert opinion Mogamulizumab represents a valuable therapy for advanced MF/SS as it can produce prolonged responses, particularly within the peripheral blood. Since it eliminates malignant T cells via ADCC, combining mogamulizumab with immunotherapeutic agents such as interferons, interleukin-12, and Toll-like receptor agonists may further enhance its efficacy.Purpose Adolescent and young adult patients of the Milan's Youth Project developed a new communication project aimed at young people with cancer in a series of video tutorials called Tumorial, they talk about their day-to-day experiences, offering "survival tricks" to their peers. Methods The project was developed during group meetings taking place every week in a dedicated room near the ward. WNK-IN-11 chemical structure Each meeting focused on a single topic and was led by the patients themselves, who talked about their experiences; staff members moderated the discussion and took notes, which was used as script for a video, recorded by one patient as a spokesperson. All the videos had English subtitles. Results The project was implemented between March 2018 and June 2019 and involved 53 teenagers and young adults (15-27 years), 33 receiving treatment and 20 in follow-up when the project began. It appears of great importance considering that social networks-and YouTube in particular-frequently provide unreliable or useless information. In making the project, patients told their innermost feelings, promoting cohesion among them. Patients and caregivers developed the project together in a significant example of cooperation.Introduction Patients in intensive care units (ICU) are often developing severe infections in which are associated with significant mortality rates. A number of novel technologies for the rapid microbiological diagnosis of these infections have been developed, introducing the era of 'fast microbiology.' Treatment of bacterial and fungal infections in ICU is however complicated by alterations in the pharmacokinetics of antimicrobial agents.Areas covered We review novel pharmacologic tools that can be used to optimize anti-infective therapies and patient management in ICU. A MEDLINE Pubmed search for articles published from January 1995 to 2019 was completed matching the terms pharmacokinetics and pharmacology with antimicrobial agents and ICU or critically ill patients. Moreover, additional studies were identified from the reference list of retrieved articles.Expert opinion Several tools are in development for the full automation of the analytical methods used for the quantification of antimicrobial concentrations within a few hours after sample collection. Ad hoc software with adaptive feedback is also available for appropriate dose adjustments based on both individual patient covariate data and therapeutic drug monitoring (TDM) data when available. The application of these technological improvements in the clinical practice should open the way to a 'fast pharmacology' at the bedside.Pharyngeal ectopic thymus is a rare cause of pharyngeal masses and is rarely considered in the differential diagnosis of neck and head masses in children. In this paper, the case of an infant with a pharyngeal ectopic thymus is presented and our intraoral surgical approach in the patient's treatment is described.Spontaneous cerebrospinal fluid otorrhea is very rare. Because of nonspecific symptoms, it is usually diagnosed when complications such as meningitis occur. Cerebrospinal fluid leak may be caused by cochlea malformation, which permits nonphysiological communication between subarachnoid space and tympanomastoid cavity. Nearly 20% of congenital sensorineural hearing loss is connected with inner ear bone malformation. We present a case of 40-year-old man suffering since early childhood from recurrent meningitis and right ear deafness, caused by congenital internal ear malformation. For many years, patient with sensorineural hearing loss had not undergone diagnostic radiology; computed tomography scans of the temporal bone had not been performed. Developing meningitis in early childhood was regarded as the reason for deafness.
